2 REGISTRATION: This is a Wheelchair and Ambulatory Sports USA (WASUSA) sanctioned qualifying meet for athletes in both the Adult and Junior divisions. Each competitor must be a member of WASUSA. If you plan to compete in more than one sport, you must join each respective National Governing Body (NGB)/ Sports Technical Committee (STC) prior to competition. You can obtain a membership application through Wheelchair and Ambulatory Sports USA: Website: director@wasusa.org Phone: (636) FEES: The fees include: registration, lunch, medals, qualification for nationals & meet sanctioning. All registration forms and payment must be received postmarked by April 30 th, 2010 to avoid a late fee of $15. Applications or event add-ons will not be accepted if postmarked after May 7, Please note that the fees are non-refundable. 12 Directions to JFK Pediatric Rehabilitation Dept Driving North on the Garden State Parkway: 1. Take Exit 131. (Please note that you will pass exits 131B and 131A). Turn right at end of exit ramp. 2. Proceed to second traffic light, Wood Avenue (two gas stations on right). 3. Turn right onto Wood Avenue and proceed to first traffic light, Oak Tree Rd. (Burger King on left) 4. Turn left onto Oak Tree Rd. and proceed through 3 traffic lights. 5. Approximately 0.8 miles past the third traffic light (Grove Ave), you will see a large red brick building on the right hand side. It is a former school. Sign on the lawn says Hartwyck at Oak Tree, Pediatric Rehabilitation". 6. Turn right into driveway. 7. Follow directions for Access to Building below. Driving South on the Garden State Parkway: 1. (Gas station should be on the right after making turn.) 3. Proceed to first light (Central Avenue). Make left onto Central Avenue. (Central Ave becomes Plainfield Rd.) 4. Proceed to fourth traffic light, Oak Tree Rd. 5. Turn right onto Oak Tree Rd. After Commerce Bank, we are the second building on the left (red brick). It is a former school. Sign on the lawn says "Hartwyck at Oak Tree, Pediatric Rehabilitation". 6. Turn right into driveway. 7. Follow directions for Access to Building below. Driving North on Route 1: 1.At Menlo Park Mall, take exit for Parsonage Road on the right. 2.Continue straight on Parsonage Road through 4 traffic lights passing a Rite Aid on your left and a Dunkin Donuts on your right. Parsonage Rd becomes James St. You will also pass JFK Medical Center on your left. Follow James St. to end and make a right at light onto Grove Ave. 3.At first light (Oak Tree Road) make a left. Follow Oak Tree Road for approximately 0.8 miles. You will see a large red brick building on the right hand side. It is a former school. Sign on the lawn says Hartwyck at Oak Tree, Pediatric Rehabilitation". 4.Turn right into driveway. 5.Follow directions for Access to Building below. Access to building: Proceed straight along entrance driveway toward back of building turning right at sign "2050 Johnson Rehabilitation Institute". Proceed to the covered, handicapped accessible entrance with double glass doors marked Pediatric Entrance. Please ring bell to be let in to the building. 12 of 14

13 Directions to Thomas Edison State Park From the NJ Turnpike: Take the NJ Turnpike to Exit 10. After the tollbooth, follow signs for Route 514 (County). Get onto 514 West. You will go past Raritan Center (on your left). At approximately miles past the Center, you will see Middlesex County College on your left. Go past the first light opposite the college entrance and look for the next light at Mill Road. Make a left onto Mill Road. Thomas Edison Park is on Mill Road, just past the college, on the left. Look for a stone sign at ground level that says Thomas Edison Park. Look for the track area on your left once inside the park. From the GS Parkway: Take the Garden State Parkway to Exit 129- NJ Turnpike. Take the Turnpike South to Exit 10. After the tollbooth, follow signs for Route 514 (County). Get onto 514 West. You will go past Raritan Center (on your left). At approximately miles past the Center, you will see Middlesex County College on your left. Go past the first light opposite the college entrance and look for the next light at Mill Road. Make a left onto Mill Road. Thomas Edison Park is on Mill Road, just past the college, on the left. Look for a stone sign at ground level that says Thomas Edison Park. Look for the track area on your left once inside the park. 13 of 14

14 Directions From hotels and park to the Banquet at the JFK Conference Center The banquet will be held at the JFK Conference Center at 70 James St., Edison, NJ (across the street from JFK Medical Center). From the Hilton Hotel: Make a right turn upon leaving the hotel parking lot onto Wood Ave. After passing the over-pass, make a left at the light onto Route 27 (Lincoln Highway). Make a right onto James Street (Dunkin Donuts on the corner). Go to the second right turn and turn right into the parking lot. The Conference Center is the white building. From the Sheraton: Take Route 1 South. Make a right onto Parsonage Road. Menlo Park Mall is on the left). Continue on Parsonage Road past the underpass to the traffic light at Route 27. There will be a Rite- Aid on the left and a Dunkin' Donuts on the right. Go straight, crossing Route 27. Parsonage Road becomes James St. at this point. Go to the second right turn and turn into the parking lot. The Conference Center will be the white building. From the Clarion: Follow Route 27 (Lincoln Highway) north as it winds to the left and then to the right. At the Rite-Aid (Dunkin' Donuts is across the street), make a left onto James St. Go to the second right turn and turn into the parking lot. The Conference Center will be the white building. From the Red Roof Inn: Make a left turn upon leaving the hotel and bear right at the Shell station. Stay on this road as it winds around and becomes Route 27 (Lincoln Highway). Continue straight on Route 27 and turn left at the fifth light onto James Street (there will be a Dunkin' Donuts and a Rite-Aid opposite of each other). Go to the second right turn and turn into the parking lot. The Conference Center will be the white building. From Thomas Edison Park: Make a right onto Mill Road. Just before the second light, take the right fork to 514 East. Just before the third traffic light, take the jug handle to 531 North (there is also a sign Route 1 Metuchen ). Cross 514 and take 531 north for approximately 0.7 miles and make a right onto the ramp for Route 1 north. Take Route 1 North for approximately 1.9 miles and take the exit/jug handle for Parsonage Road. Cross Route 1 and stay on Parsonage Road (Menlo Park Mall will be on your left). Continue on Parsonage Road past the underpass to the traffic light at Route 27. There will be a Rite-Aid on the left and a Dunkin' Donuts on the right. Go straight, crossing Route 27. Parsonage Road becomes James St. at this point. Go to the second right turn and turn into the parking lot. The Conference Center will be the white building. 14 of 14
