Due to the know-how on the development and production of aids for children, adolescents and also adults collected over many years and firmly anchored within our company, and our personal consultation service, we repeatedly succeed in setting new standards. 4 GMFCS The GMFCS classification differentiates between five stages for patients with cerebral palsy based on their movement possibilities. The basis of the distribution according to classification stages are the functional skills of the child and the need for aids including means of mobility. According to the GMFCS classification, "Stage I" includes those children who can walk without support but who cannot master complex motor tasks. "Stage V accords with a child who has extremely limited independent methods of movement, even when using aids, with low head and torso control. GMFCS Levels I - V This system is frequently also used in other areas, not just for children and teenagers with cerebral palsy. We have assigned the respective GMFCS level to each of our products: please understand this classification as an orientation aid - there may be individual reasons to deviate from it. GMFCS between the 6th and 12th birthday: GMFCS Level I The children can walk freely at home, in school, outdoors and in the surrounding area. The children can climb over and get down from kerbstones without additional physical aids, and climb stairs without holding on to railings. Running and hopping is possible, but speed, balance and coordination are limited. The children can participate in physical activities and sports depending on their personal requirements and the environmental conditions. GMFCS Level II In most situations, the children can walk freely, but experience difficulties over long distances and on uneven ground, in case of slopes, in crowds of people, on narrow paths or when carrying objects. The children can climb up and down stairs by holding onto railings, or can achieve this with physical assistance if no railings are available. Outdoors and in their surroundings, they are able to walk with physical support or using a walking aid, or they use a wheelchair over longer distances. Running and jumping are at best restricted. The restrictions in the execution of physical-motor tasks can make adaptations necessary which permit children to participate in physical activities and sports. GMFCS Level III The children are usually able to walk indoors with a walking aid. When the children are seated, they may require a seat strap to straighten the pelvis and for balance. When transitioning from sitting to standing and from the floor to standing, physical assistance is required from a person or a supporting surface. Over longer distances, the children use a form of rolling mobility aid. Children may be able to go up and down stairs by holding onto railings under supervision or with physical support. The limitations of walking may make adaptations necessary which first permit participation in physical activities and sports, including an active wheelchair or an electric wheelchair. GMFCS Level IV The children use methods of movement which usually involve physical support or electrical mobility aids. The children require adapted seat devices for torso and pelvic control; physical support is also required for most transfers. At home, the children move along the floor (rolling, creeping or crawling), walk for short distances with physical assistance or use electrical mobility aids. When the children are brought into a standing position, they can use a walker at home or in school. At school, outdoors and in the surrounding area, children are pushed in an active wheelchair or they use an electric wheelchair. Due to their restrictions in movement, adaptations are required which first permit physical activities or participation in sports, including physical support and/or electrical mobility. GMFCS Level V The children are transported in all situations in a wheelchair. The children are restricted in the possibilities for holding their head and torso upright against the force of gravity and in their arm and leg movements. Supporting technologies are used in order to improve straightening of the head, sitting, standing and/or mobility, but the existing limitations cannot be compensated for by the additional aids used. Transfers require the physical support of an adult. At home, the children are able to move short distances along the floor or are carried by an adult. The children may be able to move independently with an electric wheelchair through comprehensive adaptation of the sitting position and the controls. 5 GMFCS between the 12th and 18th birthday: GMFCS Level I The teenagers walk freely at home, at school, outdoors and in their surroundings. The teenagers are in a position to climb up and down kerbstones without physical assistance and to manage stairs without using the railing. Running and jumping are possible, but speed, balance and coordination are restricted. The teenagers can participate in physical activities and sports depending on their personal wishes, environmental conditions and possibilities. GMFCS Level II In most situations, the teenagers can walk freely. Environment factors (such as uneven terrain, slopes, long distances, time pressure, weather, the acceptance of those of the same age) and personal preferences influence the decision on which type of mobility is chosen. At school or at work, the teenagers are able to walk using a walking aid for security. Outdoors or in their surroundings, the teenagers are able to use a wheelchair over long distances. The teenagers are able to go up and down stairs holding onto railings or with physical support where there are no railings available. The restrictions in the execution of physical-motor tasks can make adaptations necessary which permit the child to participate in physical activities or sports. GMFCS Level III The teenagers can walk using a walking aid. Compared with teenagers in other stages, teenagers in stage III show increased variability in the methods of movement used, depending on the physical possibilities, the surroundings and personal factors. When they are seated, they may require a seat strap to straighten the pelvis and for balance. When transitioning from sitting to standing and from the floor to standing, physical assistance is required from a person or a supporting surface. At school, the teenagers use an active wheelchair or an electric wheelchair. Outdoors and in the surrounding area, the teenagers are pushed in a wheelchair or they use an electric wheelchair. The teenagers are able to go up and down stairs by holding onto railings under supervision or with physical support. The limitations of walking may make adaptations necessary which permit participation in physical activities and sports, including the independent use of an active wheelchair or an electric wheelchair. GMFCS Level IV In most situations, the teenagers use an active wheelchair. They require seat adaptations for pelvic and torso control. Physical support from one or two people is required for transfers. The teenagers can take on their own weight in order to assist when being transferred into a standing position. Indoors, teenagers may be able to walk short distances with physical assistance, and to use a wheelchair or a walking aid which helps to support the torso when they are brought into a standing position. The teenagers can use an electric wheelchair independently. If an electric wheelchair is neither expedient nor available, the teenagers are transported in an active wheelchair. The restrictions in mobility make adaptations necessary in order to participate in physical activities or sports including physical support and/or electrical mobility. GMFCS Level V The teenagers are transported in a wheelchair in all situations. The teenagers are restricted in their possibilities for holding their head and torso upright against the force of gravity and for controlling their arm and leg movements. Aids are used in order to improve head and torso control, sitting, standing and mobility, but the existing limitations cannot be fully compensated for by the aids used. Physical support from one or two people or a mechanical lift is required for transfers. The teenagers may be able to achieve independent mobility through an electric wheelchair subject to comprehensive adaptations of the seat and controls. The restrictions in mobility make adaptations necessary for the children to participate in physical activities and sports, including physical support and the use of electrical mobility aids. 7 SEATING + positioning The sitting position is one of our basic postures and the most frequent posture adopted by humans. Therefore a physiological, normal posture is decisive for the development of our bones and musculature as the initial basis for activities such as the search for food or for play. Learning to sit in a correct position early on helps to avoid possible damage through poor posture. Children with special needs sit more frequently in comparison to children without disabilities. For this reason, it is important that their chairs offer variable positions. The option of altering the pelvic angle is a good example: the adjustment of the angle from a relaxed sitting position to sitting up straight can be used as a training aid in order to encourage a child to hold their torso and head in one of these positions for short periods of time. Sitting is frequently combined with further activities: fine motor skills are for example required during meals; social functions are developed during play or interaction with other children. Our SMILLA sitting aids and the MADITA series support these activities and can be adapted to every situation. Through the correct adjustment to table height, the appropriate distance between mouth and hand is achieved and facilitates the guidance of cutlery; during play, it helps a child to maintain eye contact with friends. Our aids for sitting and positioning are tailored to the requirements of users, carers and supervisors. They all provide a high level of modularity, convenience and functionality, and can be especially adapted if required. This facilitates the removal of physical and psychological tension for the children through movement. The dynamic back and the dynamic seat tilt angle is controlled via a gas pressure spring. 35 TINA The cradle subframe with cradle system GMFCS Levels IV - V max. 100 kg (-)0-50 (-) cm TINA is a cradle subframe which can be equipped with a cradle system and a vacuum mattress. Even users with extreme contractions and sensitivity to pressure points can thus be provided with individual positioning. The basic model with universal mounting points also serves as a subframe for individual cradle special designs. 33

36 TINA The cradle subframe with cradle system The angle adjustment of the head section and foot section allow a temporary position change up to a sitting position. The height adjustment of the lying surface for transfer and therapy takes place electrically, as does the angle adjustment of the head section. The leg section can also be adjusted in angle via a gas pressure spring. Using an electric pump, the mattress is ventilated so that the user can sink into the material. After they have been positioned, the mattress is vacuumed and provides pressure relief on the contact surface with optimum stabilisation of the user. 34

39 Transport + travelling Aids for travelling and transport guarantee the mobility of people with disabilities. Specially-equipped rehabilitation car seats are therefore essential for everyday use, as they permit participation in social life. This applies both for children and teenagers as well as for their families, as a limited movement radius though a lack of suitable aids unfortunately frequently results in isolation long-term. Safety aspects such as the maintenance of ISO and ECE regulations are especially important in this field. Our product for transport the car seat has of course been tested and inspected acc. ECE 44/04 so that safe transport can proceed unhindered. In addition, functions such as the angle adjustment provide a comfortable and pleasant ride for the children. Parents or drivers also find it easier to position the children in the car seat using accessories such as swivel devices. Based on our SASCHA car seat, additional special constructions are possible if the accessory options and the equipment for the versions generally available in the trade prove insufficient. The maximum load of 60 kg and a seat width of up to 56 cm permit use right into adolescence before it becomes necessary to use other transport systems. Our FREDDY, FLOYD, FRANKA and the FRIDOLIN series subframes are available for more specific requirements. The right subframe is available for each area of application, whether indoor or outdoor. These all have one thing in common: they have been produced from robust and durable materials. Of course, all the subframes can be adjusted in height and seat angle, and thus used in different everyday situations. 41 SASCHA The swivelling car seat and restraint system GMFCS Levels III - V max. 60 kg / cm cm The SASCHA car seat has been inspected in accordance with the requirements laid down in the ECE-44/03 Standard for child car seats, and is particularly suitable for special constructions. The belts installed in the seat serve to support and secure the child - the restraint function is fulfilled through the 3-point seat belt installed in the car. Diverse accessories are available as individual equipment for the seat and restraint system. A side guide and torso support pad guide the upper thighs and straighten the torso. The footrest is adjustable in height and can be removed by pressing a button. 39

60 WALKING + DRIVING Walking is of central importance to humans: stability and balance capabilities as well as an upright body posture are required for the purpose. Illnesses can lead to difficulties with independent walking. Walking aids can help here: they can guarantee individual mobility and permit a more independent way of life. The diverse options range from walking sticks to walking trainers which provide full support. In order to select the correct walking assistance concept for a child or teenager, it is necessary to consider the modes of action (strain and relief) of the walking aids, and to define the assistance objectives (gait pattern, balance, mobility etc.) Anterior walkers place different physical demands on the user. In particular the user's torso has to work to stabilise the body. The reason for this lies in the relationship between the support surface of the walker and the position of the body's centre of gravity. Because the patient stands behind the walking aid, this mainly assists with balance when the patient supports themselves on the handles, and provides less relief when standing and for the related muscle activity in the upper body. Because the torso has to perform more stabilisation on an anterior walking aid, the muscle tension in the arms and legs increases accordingly. This causes increased energy consumption and fatigue. However, precisely this increased strain represents an important indication for an anterior walking aid seen from a medical-therapeutic point of view. The training effect on the torso muscles can thus be substantially increased. In addition, this type of walking aid is more compact and manoeuvrable, and is generally connected with the achievement of increased own mobility. On a posterior walking aid, the force line of action of the patient's body centre of gravity lies in the centre of the walking aid frame. This means that the user can also support themselves on the handles, and the walking aid adsorbs body weight, thus relieving strain on the legs and meaning that the torso musculature has less balancing work to do. The mode of action for which a posterior walking aid is suitable is indicated if the torso musculature is weakened or if the user finds it difficult to coordinate their torso musculature. The walking trainer also provides high lateral stabilisation of the user, in particular when the steering stop is locked. In addition, this type of walking aid has a positive influence on erectness and gait patterns using accessory options such as pelvis and buttocks support pads or forearm supports. 58

93 MOMO electric drives Mobility through support Motorised assistance when using a tricycle or therapy bike makes sense for many users and is sometimes necessary due to the indication. The start-up situation is often the only obstacle for many users for independent use of the bicycle. However, permanent support to compensate for the relative loss of power in the muscles is required for conditions such as muscular dystrophy. We have developed an innovative electric drive to make the medical benefits of cycling accessible to these people. Based on the hardware from the Canadian manufacturer of e-bikes, BionX, we have developed customised setups especially for the requirements described above. All bikes are equipped with a 250 Watt, brushless hub front motor, a 6.6 Ah lithium-ion battery and a digital display. The display can be optionally mounted for the rider or the accompanying escort. The level of assistance can be adjusted to four levels in all setups. The assistance results from the pedal pressure and a factor of approx. 1.4 to 3 times depending on the level. A selection can be made between the following setups: Start-up assistance With this setup, the rider experiences direct assistance from the motor up to 3 km/h, which then becomes ride assistance up to 6 km/h, when applying light pressure to the pedal and minimum start-up. Start-up assistance Plus With this setup the rider experiences direct assistance from the motor up to 4 / 6 km/h for 16" + 20" and up to 8 km/h for 24" + 26", which then becomes ride assistance up to 10 km/h when applying light pressure to the pedal and minimum start-up. Mobile With this setup the rider experiences direct assistance from the motor up to 4 / 6 km/h for 16" + 20" and up to 10 km/h for 24" + 26", which then becomes ride assistance up to 10/12.5 km/h with 16" + 20" and up to 17.5 km/h with 24" + 26", when applying light pressure to the pedal and minimum start-up. Higher speeds are only possible with muscle power in all of the different setups. The drive performance support is controlled digitally and is available in three different setups. The drive can be controlled via a display - this can either be mounted on the handlebars or on the luggage rack. 91

95 STANDING Children and teenagers who are unable to stand independently are forced to spend a lot of time sitting or lying down. In seated position, the hips and knees are generally bent, which unfortunately means that the hip flexors and hamstring muscles shorten. If these muscles are seldom stretched, the shortening can become permanent and extremely uncomfortable. For the maintenance of functional movement ranges, early intervention with supporting upright trainers is therefore to be recommended. Research has shown that regular standing over the course of a day can improve the muscular development and bone density. This helps to develop strong and sturdy joints, in particular in the hip, knee and ankle areas. In many cases, upright training has a positive influence on the muscle tone, which can strongly support motor functions. The metabolism is activated and so is the function of the respiratory, circulation and digestive systems. Upright trainers permit a secure standing position and the correct physiological orientation of the lower extremities. In addition, contact to other children at eye level is facilitated and social integration is thus made easier. Through the expansion of the field of view, visual perceptions are promoted, and cognitive functions are improved due to the reception and processing of environmental influences. 105 BATH + TOILET Children and teenagers with mental or physical disabilities frequently require support with bodily hygiene, whether when washing, cleaning teeth, showering, bathing or going to the toilet. We wash ourselves on average at least once per day; go three to eight times per day to the toilet and clean our teeth up to three times per day. These figures quickly indicate that the demands on bath and hygiene aids are of course particularly high. They have to be robust, water-resistant, comfortable and at the same time variable and adjustable. The frames of our aids for the sanitary area have all been produced from rustproof, corrosion-free stainless steel and are therefore ideal for use in wet areas. Skin-compatible covers with high abrasion resistance ensure comfort and prevent unpleasant pressure points or an uncomfortable feeling in the water. 127 Cleaning guidelines by material Wood: Wooden parts should be cleaned with a damp cloth. When using detergents, please consult the manufacturer's instructions regarding detergent concentration. Caustic household detergents or corrosive substances are not suitable. Never leave natural wood in a damp or wet condition over long periods, as it will take on water and can change in shape (swelling). Therefore, only wipe and clean damp (i.e. with wrung-out cloths). Test a small, non-visible area for compatibility. Powder-coated parts: Aluminium parts should be cleaned with a sponge or damp cloth using normal antibacterial household detergents. Stainless steel: Stainless steel parts should be cleaned using a sponge or damp cloth with a mild soap. Bleach must not be used. Covers / Fabric: Covers and fabric should be washed at 30 C in the washing machine using normal detergent. Do not tumble dry. Plastic: Plastics should be cleaned with a sponge or damp cloth. Disinfect surfaces using the non-alcohol-based disinfection solution "mikrozid sensitive liquid" as a spray disinfectant or a foam from Schülke & Mayr GmbH. Please note the solution instructions if using other solutions! How? To CLEAN, wipe all accessible surfaces with a damp sponge cloth and a fat-dissolving washing-up liquid if required. Then dry the surfaces with a cotton or disposable cloth. Hard-to-access places can also be dried using a hairdryer on its lowest setting. To DISINFECT apply the "mikrozid sensitive liquid" undiluted on the surfaces and leave for 1 minute (leave for 30 minutes if there is suspicion of contamination with the Noro virus).
