1 EASY APPLICATION Community Traffic Safety Awards Program 2017 Application AAA Community Traffic Safety Awards are presented to communities for their efforts to improve local traffic safety for all modes of travel. Award levels are Bronze, Silver, Gold and Platinum. The AAA Community Traffic Safety Awards Program is not a competition; any community that completes the application and earns the required number of points will receive an award. Points are earned for having an active Traffic Safety Committee; participation in AAA Traffic Safety Programs, Three E programs and projects; and statistical measures of success. General Instructions All information should be for the year Instructions and point values for each of the categories are included for your information. Applications can be completed online, but must be printed out and faxed or mailed to the address below. 2 CATEGORY 1. Traffic Safety Committee (Max. 20 points) Community input should be obtained from a traffic safety committee, comprised of individuals who represent various local organizations and modes of travel. Their input can be used to identify traffic safety issues and to formulate solutions. Instructions: Check the boxes that apply to your community. Does your community have a Traffic Safety Committee? Monthly Every other month (6 times a year) Four times a year Three times a year Less often, as needed Please list the organizations represented on the committee: AAA Scoring: Maximum of 20 points for this section. 10 points for communities with a Traffic Safety Committee. 5 points if it s composed of at least 3 organizations. 5 points for monthly meetings, 4 points for bi-monthly, 3 points for quarterly, and 2 points for 3 times a year, 1 if less often. CATEGORY 2. Participation in AAA Traffic Safety Programs (Max. 25 points) AAA offers many programs that help educate the public on safe driving, bicycling, child passenger safety, and being a safe pedestrian. AAA encourages communities to take advantage of these programs as a supplement to your own efforts. Instructions: Check which AAA Traffic Safety programs or events your community has participated in 2016: AAA School s Open-Drive Carefully Campaign AAA Halloween Safety Campaign AAA School Safety Patrol AAA Bike Rodeos AAA Safety Events (fairs, car seat checks, CarFit events, etc.) AAA School Assembly Programs or Workshops AAA literature distribution Note: Your organization may do many of these kinds of programs on your own, without assistance from AAA. This section is only for programs in which you, or another organization in your community (e.g. schools, libraries, etc.), have solicited AAA s involvement. AAA Scoring: Maximum of 25 points for this section. 5 points for each AAA program they participate in.

3 CATEGORY 3a. Three E s Programs & Projects (Max. 75 points) Communities should implement programs or projects designed to address local traffic safety issues for all modes of travel (e.g. motor vehicle, bicycle, pedestrian, child passenger safety, motorcycles, etc.) through the Three E s of Education, Enforcement, and Engineering. Here is an opportunity to indicate the programs, projects and enforcement programs your community utilizes to facilitate safer traffic. In order for a program or project to earn points towards your award, it must meet the following qualifications: It must have been conducted, implemented or enacted in (For this section, the program does not need to be new you only need to have utilized it in New programs will be covered in Category 3b.) It is not part of standard enforcement or maintenance. It has provided an identifiable safety benefit. It has benefited a significant segment of the community. 4 CATEGORY 3b. 3 E S WORKSHEET (for Platinum Points) Here is your opportunity to let your community shine by describing your NEW in 2016, innovative and possibly unique educational programs, enforcement programs or engineering projects that have positively impacted traffic safety in your community. Instructions: Reproduce this page and complete for each NEW program for Attach documentation, including photos, if applicable. Be sure to include the traffic issue addressed, your solution/program, and how you have evaluated it. AAA Scoring: Platinum Points will be awarded based on description, documentation and merit of the program or project. Up to 3 points awarded per new program or project. Include initial problem, how your program/project addressed the problem, and how you have evaluated it.

5 CATEGORY 4. Statistical Measures of Success (Max. 30 points; 12 Platinum Pts.) Communities should track local statistics each year to evaluate and measure the success of their programs and projects, as well as to determine areas for needed improvement. Hopefully, the data will demonstrate a trend of improvement. Instructions: In the chart below, provide statistics for measures that your community tracks. Use n/a if data is not available. In the space below, indicate data sources used to obtain statistical measures. This must be included to receive points. AAA Scoring: 2 points will be awarded for each measure with entries for all five years. 1 point will be awarded if only a 2016 entry is provided. Platinum Points will be given for either a five-year trend of improvement or for a 2016 seat belt use entry that is substantially better than the statewide or national data. Communities with no pedestrian fatalities in 2016 will receive an award. Maximum of 30 points for annual points, Maximum of 12 platinum points. Data source must be listed to receive points.
