Evelyn Dudley addressed requests that were received to make P&T recommendations for the Committee available to the public in advance of the meeting. She stated this is being discussed internally and no decisions have been made at this time. Evelyn motioned to remove P&T committee member Angelo Defazio from the committee due to lack of response and attendance to meetings. The committee was unanimously in favor. Vacancies on the committee were announced and current members were encouraged to recruit potential replacements. Evelyn announced that the State intends to extend Step-Therapy prior authorization criteria to additional classes this year, targeting the Antimigraine Triptans and Lipotropics Statins therapeutic classes. DSS and HP solicited the P&T Committee to provide input on specific criteria.

Therapeutic Class Reviews: Note: Brand name drugs listed in bold font. Antibiotics, Inhaled Kathy Novak from Magellan presented the recommendations for this class. Dr. Craig Lapin provided testimony in favor of adding Cayston to the formulary. Beth D Ambrosio from Novartis provided information on the benefits of TOBI Podhaler. Hilda Slivka motion to accept the recommendations as presented, along with the addition of Cayston and TOBI Podhaler to the PDL. Her motion was seconded by Carl Sherter and passed unanimously. ON Medicaid PDL: BETHKIS (INHALATION), CAYSTON (INHALATION), TOBI PODHALER (INHALATION), TOBI (INHALATION) Pancreatic Enzymes The recommendations were presented, and there were no changes from the previous year. A motion to accept the recommendations as presented was unanimously passed by the committee. ON Medicaid PDL: CREON (ORAL), PANCRELIPASE (AG) (ORAL), ZENPEP (ORAL) Anticoagulants The recommendations were presented to the committee, and there were no changes from the previous year. Scheduled speakers waived their right to speak. Stella Cretella motioned to accept the recommendations as presented, and was seconded by Charles Thompson. The committee passed the motion unanimously.

Stella Cretella motioned to accept the recommendations as presented. Kevin Chamberlin seconded and the motion passed unanimously. ON Medicaid PDL: NATROBA (TOPICAL), PERMETHRIN CREAM (TOPICAL), PERMETHRIN OTC (TOPICAL), PIPERONYL BUTOXIDE/PYRETHRINS SHAMPOO OTC (TOPICAL), SKLICE (TOPICAL), ULESFIA (TOPICAL) Beta Blockers The recommendations were presented, and Dr. Komsu Mamuya provided public testimony in support of adding Bystolic to the PDL. Emmett Sullivan motioned to accept the recommendations as presented, along with the addition of Nadolol. His motion was seconded and passed unanimously. ON Medicaid PDL: OXYBUTYNIN ER (ORAL), OXYBUTYNIN SYRUP (ORAL), OXYBUTYNIN TABLET (ORAL), TOVIAZ (ORAL), VESICARE (ORAL) Colony Stimulating Factors The recommendations were presented, and Tammy Curtice of Amgen provided testimony for the inclusion of Neulasta on the PDL. Jeffrey Kamradt discussed Neulasta being the standard of care and ease of administration. He motioned to accept the recommendations as presented, with the addition of Neulasta. 5 Growth Hormone The recommendations were presented and Aniello Marotta from Pfizer provided testimony for the inclusion of Genotropin. Hilda Slivka motioned to approve the recommendations as presented, with Emmett Sullivan seconding. The motion passed unanimously. ON Medicaid PDL: NORDITROPIN PEN (INJECTION), NUTROPIN AQ CARTRIDGE (INJECTION) Hepatitis C Agents The recommendations were presented, and public testimony was provided by Dr. Hoffman and Arlene Price of Janssen in support of Sovaldi and Olysio, respectively. The committee deliberated and a motion was made to accept the recommendations as presented along with the addition of Olysio and Sovaldi. The motion passed unanimously. ON Medicaid PDL: INCIVEK (ORAL), OLYSIO (ORAL), PEGASYS KIT (SUBCUTANE.), PEGASYS PROCLICK (SUB-Q), PEGASYS SYRINGE (SUB-Q), PEGASYS VIAL (SUBCUTANE.), RIBAVIRIN TABLET (ORAL), SOVALDI (ORAL), VICTRELIS (ORAL) Hypoglycemics, Incretin Mimetics/Enhancers The recommendations were presented, and Dr. Bogdanovics provided testimony for the inclusion of the GLP-1 agonists on the PDL. Angela Gaskins-Younger also advocated for the GLP-1 Agonists to be on the PDL. Ron Jones of AstraZeneca and Justin Bakshai of Novo Nordisk provided testimony in support of their companies drugs in this class. The committee discussed the different GLP-1 agonists at length. Hilda Slivka motioned to accept the recommendations as presented, along with the addition of Bydureon and Victoza. The motion was seconded by Charles Thompson. Emmett Sullivan and Kevin Chamberlin opposed this motion, but it was upheld by a 5-2 vote. ON Medicaid PDL: BYDUREON (SUBCUTANE.), BYETTA PENS (SUBCUTANE.), JANUMET (ORAL), JANUMET XR (ORAL), JANUVIA (ORAL), JENTADUETO (ORAL), TRADJENTA (ORAL), VICTOZA (SUBCUTANE.) Hypoglycemics, SGLT2 The recommendations were presented for this class, with neither of the two agents being recommended as preferred. Ron Jones spoke in support of Farxiga, and Arlene Price in support of Invokana. Kevin Chamberlin motioned to accept the recommendations as presented, and was seconded by Jeffrey Kamradt. ON Medicaid PDL: AMITIZA (ORAL), LINZESS (ORAL) Lipotropics, Other The recommendations were presented and Dr. Drew Revel from Genzyme spoke in support of Kynamro. Emmett Sullivan motioned to accept the recommendations as presented and the motion passed unanimously. ON Medicaid PDL: CHOLESTYRAMINE/ASPARTAME (ORAL), CHOLESTYRAMINE/SUCROSE (ORAL), COLESTIPOL TABLET (ORAL), GEMFIBROZIL (ORAL), NIACOR (ORAL), NIASPAN (ORAL), TRICOR (ORAL), TRILIPIX (ORAL) Multiple Sclerosis Agents The recommendations were presented for this class and Kathy Novak clarified that Extavia and Betaseron were the same ingredient and manufactured in the same facility. Elizabeth Destafney from Teva spoke in support of Copaxone 40mg, Beth D Ambrosio from Novartis for Gilenya, Pamela Salisbury from Genzyme for Aubagio, and Elaine Alexander from Acorda for Ampyra. Kevin Chamberlin motioned to accept the recommendations as presented and the motion passed unanimously.

Hilda Slivka asked details about usage of the drug in pregnancy. A motion was made to accept the recommendations as presented, and it was passed unanimously. ON Medicaid PDL: NALTREXONE (ORAL), SUBOXONE FILM (SUBLINGUAL) PAH Agents, Oral and Inhaled The recommendations were presented and there were no changes from the previous year. Casimir Zygmunt from Bayer spoke in support of Adempas, and Raymond Mastriani from Actelion spoke in support of Opsumit. Stella Cretella motioned to accept the recommendations as presented and the motion was passed unanimously.
