5 VERNON YOUTH FOOTBALL & CHEERLEADING Physical & Parents Code of Conduct PARTICIPANTS OF ALL VYF&C FOOTBALL AND CHEERLEADING TEAMS, AT ALL LEVELS, MUST HAVE A PHYSICAL EXAMINATION IN ORDER TO BE ELIGIBLE TO PRACTICE AND PLAY. The physical examination must take place after January 1 and prior to August 1 of the current year. A statement signed and dated by the physician indicating that the child is physically sound to participate in the sport must be provided to the coach prior to the first practice All athletes should arrive to games and practices on time and should be picked up promptly. 2. It is the parent s responsibility to supervise any siblings brought to any VYF&C functions. Siblings MAY NOT be dropped off with participant. 3. Proper sportsmanship is required at all times by the participant, parents and all other spectators. ANY ABUSE OF THE OFFICIALS, COACHES, or PLAYERS - WILL NOT BE TOLERATED. 4. SMOKING, ALCOHOL, AND ILLEGAL DRUGS ARE STRICTLY PROHIBITED AT ALL VYF&C EVENTS. 5. The OFFICIALS have the right to eject anyone from a VYF&C event. Failure to comply will result in POLICE involvement. 6. Any violation of the above VYF&C Code of Conduct will be subject to disciplinary action by the VYF&C and/or the N.J.Y.F.L., which may include but not limited to, the removal of and/or suspension from further events. (Please sign and hand in the bottom portion at time of registration) The person signing here acknowledges that they are the representative for the participant and his/her family and friends, and by signing here you agree that you, and the participant s family and friends are aware of the VYF&C Code of Conduct. It is also agreed that each participant must have a physical examination after January 1 st and prior to August 1 st, and will submit a signed physician s note to the coach at the first practice.
