2 NCMS Cheerleading Handbook Contract NCMS Cheerleading Contract with Student & Parent/Guardian I have read the NCMS Cheerleading Handbook and agree to all rules and regulations set forth within. As a cheerleader, I understand that I am responsible for all the rules outlined in the constitution, including the following: Maintaining a position of honor and responsibility. Representing NCMS, myself and my family in the most positive way possible at all times. Exhibiting a positive and cooperative attitude. Attending all practices at scheduled times. Being in uniform and supporting all school events as assigned by coach/sponsor. Working early mornings, afternoons, evenings and some weekends during the school year and the summer. Preparing for all pep rallies, performances, etc., ahead of time. Making cheerleading and its responsibilities my priority to ensure that the job is properly completed. Doing my share of work at all times, as cheerleading is a team effort. Maintaining proper class conduct grades in order to participate. Having at least a 70 in every subject throughout the school year in order to participate. Making sure my grades do not interfere with cheering. Cheerleading follows all UIL rules (NO PASS/NO PLAY). I understand I can be relieved of my cheerleading responsibilities if I fail to do my job or abide by the regulations established by the coach/sponsor. I agree to the cell phone/electronic use policies outlined in the handbook Furthermore, I understand that I will be relieved of my cheerleading responsibilities if I am placed in ISS for any incident other than dress code or for being tardy. Handbook Agreement I give permission for my child to try-out for the 2014/2015 squad. I understand that I am financially responsible for the cheer responsibilities outlined in the NCMS Cheerleading Handbook. 3 NCMS Cheerleading Parent Responsibility Contract NCMS Parent Responsibility Guidelines The NCMS Spirit program fosters a positive and respectful environment for students, parents and coaches/sponsors. As a parent/guardian I understand I am instrumental in the social development of my child. Parent behavior may not be distracting or disrespectful. The following behaviors will not be tolerated: loud outbursts, profanity (verbal & written which includes internet communication and social networking), disruptive behavior at any function, other inappropriate conduct In order to create a mutually respectful and positive environment I agree to: Remember that my child participates for her enjoyment, not mine. Demand that my child treat all teammates, coaches and fellow parents with respect. Remember that children learn by example and I must be a role model for proper and courteous social interaction at all events and in times of conflict. Not encourage unsportsmanlike conduct against any coach or fellow teammate. Not encourage gossip among parents or cheerleaders. Leave coaching to the coaches. Communicate with coaches in a respectful tone and manner at all times. Resolve conflict on an adult level that does not interfere with the continuity of the cheerleading program. Contact coach regarding any issue/concern in written/ communication not through phone contact. I acknowledge that failure to comply with set parent behavior guideline may lead to a disciplinary action that may include, but is not limited to, the following: Verbal warning by coach or school administrator Written warning by coach or school administrator Temporary parental suspension from events/activities/direct communication with coaches (does not negate parent responsibility to assure cheerleader fulfills obligations) Repeated offenses will result in removal of child from squad Acceptance of Parent Responsibilities I agree to the aforementioned conditions and agree to be a supportive and positive influence on the NCMS Cheerleading program.

4 NCMS Cheerleading Parent Financial Contract NCMS Parent Financial Responsibilities The NCMS Cheer Program is a fee based program. The approximate expense of the program is outlined below. Additional costs may be incurred throughout the year for additional events or activities. A fundraiser will be available to each individual cheerleader/mascot to reduce their individual expenses. I/we understand the expenses involved in cheerleading and understand orders are placed as soon as possible. I/we also understand that once the orders are placed, there are no refunds or exchanges. In addition, once the orders are placed, I/we agree to pay the full amount even if my/our child moves away, is ineligible or changes their mind. I/we are fully aware of all of the expenses that will occur throughout the year and agree that any remaining balance of $50.00 or less will be considered a contribution to the NCMS Cheerleader Activity Fund. I understand that I am financially responsible for the cheer equipment listed in this handbook and aware that if the FINAL PAYMENT is not received by April 16 th, that will forfeit her position on the squad and full amount will remain due.

5 Parent/Guardian Acknowledgement of Risk Inherent Risk of Cheerleading Cheerleading is a sport and with any sport there is risk of injury. Cheerleading is an anaerobic/aerobic activity that includes jumping, stunting, motions, and tumbling. Coaches should be informed of any injury or chronic conditions. Although the probability of injury is minimized if you practice correctly, there is always the possibility of one occurring. Injuries that can occur in cheerleading include, but are not limited to, the following: blisters, muscle strains, ligament sprains, joint and muscle soreness, abrasions, contusions, stress fractures, broken bones, spinal cord injuries involving paralysis and even death. However, if you take certain precautions, the possibility of such injuries may be largely decreased. Be sure to abide by the following: Never stunt or tumble unless a coach is present. Always practice in the presence of a qualified coach. Always warm-up appropriately before cheering (practice and games) by jogging and stretching. Do not attempt a stunt that you do not know how to perform safely and that has not been cleared by the coach. Always use attentive spotters when stunting. Always use mats or a grassy area when stunting during practice. Always cheer in an area free from obstructions. Do not stunt on uneven ground, wet surfaces, and concrete. Do not stunt in extreme cold or rainy weather. Never talk, laugh, or mess around when performing a stunt or learning a stunt. Report injuries as soon as they occur. Follow all trainer and doctor recommendations. Lift weights to increase strength and guard against injuries. Always wear shoes and clothing appropriate for cheerleading. Never wear jewelry of any kind or chew gum when cheering including practices and games. Always have your hair pulled back from your face and shoulders. Eat nutritious meals and get plenty of rest. Always ask for assistance or advice at any time. Do not stunt or tumble when game is in process. Take all activities seriously. Do not participate in cheerleading if you are sick Acceptance of Inherent Risk of Cheerleading I have read the preceding warning. I thoroughly appreciate and understand the assumption of risks inherent in cheerleading participation. The risk of injury includes minor and catastrophic injuries. I acknowledge that my child is physically fit and voluntarily participating in the activity of cheerleading. I understand these risks and will not hold New Caney Independent School District or any of its personnel responsible in the case of accident or injury at any time. I understand that the school and/or its personnel assume no financial obligation for any injury that might occur. Student Signature

6 NCMS Cheerleading Fundraiser Contract Fundraising Cheerleader Responsibilities In order to keep the price of the middle school cheerleading program affordable, the cheer organization sponsors a fall and spring fundraiser. Money from the fundraisers helps to subsidize the cost of any activities that they will participate in throughout the school year. All cheerleaders are required to participate in ALL team fundraising activities. The purpose of fundraising is to subsidize any additional activities the cheerleaders will participate in throughout the year. Cheerleaders are responsible to sell the product or participate in fundraising event and turn in all money or unsold products in a timely manner. Cheerleaders are responsible for the dollar amount of the product they were issued if the fundraiser was a product fundraiser Failure to turn money in will result in disciplinary action and will be reported as a theft of the organization. Acceptance of Fundraising Responsibilities I have read the preceding information regarding fundraising. I understand that if I make the cheerleading squad I am required to participate in all fundraisers. I understand I am responsible for the money collected from product sales and am required to turn the money in to sponsors when due. Student Signature

7 NCMS Cheerleading Cell Phone/Electronic Use Contract NCMS Cheerleader Guidelines In order to create a distraction free and safe practice/ performance environment, use of cell phones will be prohibited unless granted explicit permission due to an emergency. I agree to the following conditions concerning cell phones/electronic devices: I will not bring my cell phone to class/practice. My phone/device will not be out during games/performances, including halftime, unless I am given explicit permission. I will not take unauthorized photos or video of any team member, coach or cheer parent. I will not distribute unauthorized pictures or video on any social networking site nor distribute any material through any other electronic means (including text, , etc.) If given permission to use phone, I will use the phone/device the explicit reason and return my phone to my bag promptly. I will follow the AUP outlined the New Caney ISD Student Handbook. If I violate the above guidelines I will be subject to the following consequences: 1 st occurrence verbal warning 2 nd occurrence cell phone will be confiscated and a parent must pick the phone/device up. Incident will be documented. 3 rd occurrence cell phone will be confiscated and a parent must pick the phone/device up. Incident will be documented and cheerleader/mascot will be benched for next performance. 4 th occurrence removal from squad Demerits will be issued based on each individual incident If the incident violates the district AUP policy, administrative discipline according to the policy will occur, which may include suspension or permanent removal from the squad. If any incident violates the privacy of a fellow teammate or coach, the cheerleader/mascot will be immediately removed from the squad. Acceptance of Cheerleading Cell Phone/Electronic Use Policy I agree to the aforementioned conditions and agree to the consequences set forth if I violate the cell phone/electronic use policy. Cheerleaders must have a positive attitude at all times and will assume the role of a student ambassador representing the best of NCMS. There are many privileges with becoming a cheerleader, and with those privileges comes many responsibilities.
