2 Page 2 Strikers AllStar Cheer Team Season Welcome to the Strikers AllStar Team and we are excited to have you as a part of our family. We are entering our 5 th season and working towards a winning season. In order to accomplish this, there are some basic rules that need to be followed by all families. We appreciate you taking the time to read this packet and completing the forms necessary for participation. Please review with your son/daughter as they will be equally responsible for the rules listed below. Good sportsmanship, polite manners and a kind disposition are mandatory in the gym. *All cell phones and electronics must be turned off or left outside of practice areas Injuries Parents need to note that cheerleading is a highly competitive and dangerous sport. The stunts and tumbling could lead to injuries. These include but are not limited to bruises, pulled and strained muscles, torn or strained ligaments, broken bones, or dislocations, paralysis or other major injuries.. We at Strikers Allstars INC take every precaution to limit these injuries, but unfortunately, we cannot prevent them all. Our staff is CPR and concussion certified. In the event that your child is injured we will take every necessary step to ensure your child s wellbeing. Athletes need to inform staff immediately of all injuries. We as a staff will do our best to contact you as a parent. If you do not hear from our staff please feel free to contact your child s coach Tardiness and Absences Tardiness is defined by not reporting to any and all locations at specified time. For every minute late there will be a 5 lap penalty. Excessive tardiness or absences will result in dismissal from the team. Please plan vacations or other activities during scheduled team breaks. Any athlete absent or tardy will need to notify his/her team mom. Gym Dress Code All athletes should be dressed in required team practice attire. This will be communicated by team mom. During group outings all athletes must wear the team t-shirt and a bow of their choice. NFINITY VENGEANCE are required to be worn during all practices and competitions. NOTE: Competition shoes are never to be worn outdoors.

3 Page 3 A Cheer Team Competition Uniform is a required purchase by each athlete. Details regarding purchase and sizing will be provided by Spirit Wear Coordinator. Please note that the competition uniform and shoes are purchases that are made individually by each athlete and are in addition to any registration or team fees. NO JEWELRY! Including ear, nose, tongue, eye brow or belly button rings. Wearing these items could result in serious injury to your child or to another child. Wearing jewelry is also illegal and could result in a major penalty at a competition. Fingernails must be kept short. No fake fingernails. Schedule/Holidays/Practice changes Team moms will relay all added practices. Parents should plan on extra practices 2 weeks leading into competitions. Strikers AllStar team may not always follow the Fulton County School Schedule with regard to the holiday schedule. We need to get in as many practices as we can. If you are unable to make a practice during a Holiday, this will be an excused absence, but please make sure your team mom is aware that you will not be attending. Parents and Relatives Only cheerleaders and coaches are allowed on the mat unless invited by a coach. No one is allowed to yell on to the floor or speak to any team member or coach while class is in session. Please do not gossip about other children in your class. Please do not gossip about coaches. The coaches reserve the right to close practices at any time. Please feel free to speak to your coach about anything; just remember to do it at the appropriate times. For example, approaching a coach in the middle of practice would not be an appropriate time. General Information If you ever have a problem, please do not hesitate to contact Coach David. Please keep in mind that he has limited time with the team so if you need to speak with him, please try to arrange a time to meet with him that does not take away from the limited time he has to coach. Communication of absences or any other issues should be communicated to the Team Mom first who will notify Coach David. Remember, Strikers is our kids cheer home. Please be respectful of the facility at all times. No profanity or abusive language will be tolerated at any time. No MENTAL BLOCKS. Mental blocks are evil and contagious. All class decisions are left to the discretion of the coaches.

4 Page 4 Every team/every year will go through losses and additions of athletes. The dismissal or addition of anyone is at the coach s discretion. Practices may be changed and/or added throughout the year. Parents need to check s and our website regularly. Please encourage ALL athletes throughout the season. All cheerleaders and families will show good sportsmanship at all times. All decisions will be made by the owner of Strikers Allstars Inc. These rules may be modified at any time. Costs (Annual and Monthly) All team participants must have an updated payment information in our system. v MONTHLY TUITION FEES: $150 Competitive Cheer $70 Tumbling Class v ANNUAL FEES PER ATHLETE: (NO refunds given once payment made) All fees are to be paid on the date due. Payments will be drafted on the 5th of each month v TEAM FEES PER ATHLETE: (NO refunds given once payment made) Team fees for Vipers and Cobras are $1750 for cheerleaders ( uniform, backpack, and windsuit are additional) Payment schedule is: 5/10 $350 6/10 $350 7/10 $350 8/10 $350 9/10 $350 v Uniform Fee: Each team has a custom uniform designed for their season. All athletes are responsible for this cost. This cost ranges between $250-$500 v Post season Competitions:We will honor the highest paid bid provided. This will be determined by the gym owners and coaches. A paid bid refers to the athletes registration fees. All other fees are the responsibility of the parent * The above summary does not include travel costs and Disney registration fees. Each parent will be responsible for the cost of their child (and anyone accompanying their child) to travel to and from each competition. Please note that most competitions will have an entrance fee of up to $20 per person at the door (Competing Athletes do not have to pay this fee).

5 Page 5 Monthly Tuition is due by the 5 th of each month. Any payment not received by the 5 th of that month will incur a $15.00 late fee. The Annual Fees payment schedule is due at sign up. FAQ s Q: When can my child move up another level? A: We reserve the right to move a child (up or down a level) at any time. Q: Why do some parents sit in the lobby and bad mouth the coaches, the kids, or other teams? A: We have no idea. Strikers Allstars Inc reserves the right to amend our handbook as deemed necessary. We will notify all parents of any amendments.

Please help us by following the guidelines set forth in this packet. It is much easier for the coaches and cheerleaders to do their job when they do not have to worry about outside issues. When you have read and understand everything in this packet, please do the following: ü Sign and date this page (below) and turn in at tryouts. ü Pay the annual registration fee and 1 st month tuition. Payment must be received at the time of tryout. ü Complete and sign the Registration and Release Form ü Make a copy of your athlete s medical insurance card and give it to the Team Mom ü Complete and sign the Parent/Athlete Agreement Form ü Complete and sign the Vacation Request Form ü Register your Athlete(s) at USASF and get the Athlete ID number to the Team Mom ü Get a copy of the Athlete s birth certificate to the Team Mom ü If you feel overwhelmed, take a deep breath - it really will be okay! STATEMENT OF UNDERSTANDING Parent/Legal Guardian signature indicates receipt and acceptance of the Strikers AllStar Inc. Cheer Team Information Packet. No prior determination of life threatening emergency or danger of serious or permanent injury resulting from treatment need be made under this authorization. Exceptions to this authorization are as follows: I am fully aware that any activity involving motion, height, or athletic activities creates the possibility of serious injury, and I further agree to hold Strikers Tumble & Cheer, LLC and its staff and officers harmless for any injury or resulting expense(s). I release and discharge all rights and claims against Strikers Tumble & Cheer, LLC and its parties. Strikers Tumble & Cheer, LLC strives to provide the maximum in safety procedures and guidelines, and cannot assume responsibility for any accidents, injury, or illness that may occur. I authorize Strikers Tumble & Cheer, LLC to use photographs, video and/or other likenesses of my child for use in its promotional materials or sales and waive any rights of compensation or ownership thereto. 8 Page 8 PARENT/ATHLETE AGREEMENT FORM I (the parent/guardian s name) and my child have read and understand the information packet. I will commit to have my child at practice and stay up to date on my monthly payments and pay the competition expenses in a timely manner. My child will commit to attending all practices and will work with his/her teammates to reach all goals set forth. Any parent or athlete not following the financial agreements or team commitments for the season may be subject to financial penalties or dismissal from the program.
