1 Information Packet The mission of PPMH Cheerleading is to promote school spirit and to serve as ambassadors between our school and community. Both in school and at competitions, we strive to represent the Pinecrest Crocs with pride and outstanding character. It is our goal to be role models and lead by example in and out of the classroom. We understand that we are students before we are athletes and that this relationship is reciprocal. As a team, we will persevere to improve academically, physically, and mentally everyday with the intention of being revered and reaching our ultimate goal of the being the best cheer team we can possibly be. Practices will be at the Magic City All-Stars Gym in Doral. Exact address will be announced before our first practice. Practices start on Monday June 17th! Summer: Monday & Wednesday 6:00pm - 9:00pm at gym. Regular: Monday & Wednesday 6:45pm - 9:00pm at gym. 5 PINECREST PREP CHEERLEADING RULES AND REGULATIONS Please note that each time PPMH is mentioned throughout this contract it is meant to be all inclusive of the Pinecrest Prep Middle High Cheerleading Program, its managers, coaches, and employees. GENERAL 1. Only registered athletes are allowed in the practice area or floor. 2. All spectators must remain in the designated area and keep the noise level down at all times during practice. 3. Any person that disrupts a practice may be asked to leave the premises. 4. Absolutely NO food, drinks, or gum are permitted in the practice area. 5. All trash must be disposed in the trash receptacles before AND after practice. 6. Cell phones must be put on silent mode or left outside of the practice area. 7. PPMH is not responsible for any personal items that are lost or stolen. 8. All completed forms, payments and/or information needed by PPMH must be turned in to Coach Nilse or Ms. Torrado as requested. 9. ALL CHEERLEADERS MUST MAINTAIN A 2.0 GPA IN ORDER TO PARTICIPATE IN THE PPMH CHEERLEADING PROGRAM!

6 TEAM PPMH retains the right to: 1. Move, replace, add, suspend, or even dismiss an athlete for a period of time or indefinitely from a team or the entire program based on criteria including but not limited to: Attendance, GPA, Finances, Conduct, and Parent Conflicts. 2. Decide if an athlete may or may not compete. 3. Decide the roles and/or positions an athlete will have/play on the team. (E.g. base, flyer, back spot, tumbler, alternate, etc.) 4. Extend practices longer than the scheduled time or add additional practices if necessary and as long as it does not conflict with school, homework or exams. ATTENDANCE/INJURIES All athletes must: 1. Attend and be prepared to participate in all PPMH Cheerleading activities (practices, competitions, fundraisers, meetings, school events, etc) including those unexpectedly added throughout the season unless otherwise recommended by a doctor through valid written documentation. 2. Notify coaches immediately when an injury occurs. 3. Arrive at least 15 minutes early to all PPMH Cheerleading activities. 4. Schedule all vacations so as not to interfere with any PPMH Cheerleading competitions. 5. Notify coaches in writing prior to all expected tardiness or absences. 6. Notify coaches by calling or text messaging immediately, of any unexpected tardiness or absences.

7 DRESS CODE All athletes must: 1. Be dressed as specified below for every practice unless otherwise instructed by the coach. All items must be official PPMH Cheerleading items or approved by the coaches. Practice top Practice shorts Practice bow (high ponytail with no bangs in face) Cheerleading or athletic sneakers 2. Maintain a well-groomed appearance and good personal hygiene at all times. 3. Keep hair and bangs out of the face (in a high ponytail) at all times during events. 4. Keep nails at fingertip length to avoid injury to themselves and/or teammates. 5. Remove all jewelry during all practices, competitions, events etc. 6. Wear appropriate athletic sneakers to practice. (NO CONVERSE OR VANS; these are prone to cause injuries). 7. Replace all lost or noticeably damaged uniform/practice items. It is the athlete's responsibility to notify coaches if uniforms items are damaged prior to any event or activity.

8 COMPETITION DRESS CODE By the scheduled meeting time for any competition or event: 1. Athletes must be dressed as specified below unless otherwise instructed by their coach. All items must be official PPMH Cheerleading items or approved by the coaches. Uniform top Bloomers or Spankies Bow Socks Competition sneakers Warm up pants Warm up jacket Uniform skirt must be inside cheerleading backpack 2. All rollers must be taken out and hair and make-up must be complete as per coach s request. 3. All non-uniform items such as sunglasses, cell phones, CD players, and ipods must be put away. 4. Any braces or tape needed to perform must be supplied by the athlete and put on before going to the warm up mat. 5. All jewelry and colored nail polish must be completely removed. 6. During awards ceremonies, athletes must be in full competition uniform and may not wear backpacks, warm ups or any other items unless otherwise approved or requested by the coaches.

9 AFTER AWARDS/ CERMONIES: 1. Athletes may only change into their official PPMH competition t-shirt and warm ups. 2. No skirts may be worn over warm up pants. 3. Socks and competition sneakers must be worn at all times (ABSOLUTELY NO CROCS, SANDALS, FLIP-FLOPS ETC. ARE ALLOWED AT ANY COMPETITION OR EVENT) SPORTSMANSHIP/CONDUCT All athletes and parents must always: 1. Set a positive example for others to follow by being respectful and courteous to everyone. 2. Privately speak with coaches to discuss any issues that may arise. 3. Refrain from gossiping or any form of verbal or physical confrontation. 4. Refrain from celebrating the misfortune or defeat of another person, team, or program. 5. Accept team placements and awards with dignity and class at all times. HEALTH All athletes must: 1. Inform PPMH of all medical conditions that may limit or prevent their ability to participate in any PPMH Cheerleading activities. 2. Notify PPMH of any injuries sustained as a result of their participation in any sanctioned PPMH Cheerleading activities. 3. Provide valid written documentation from a doctor explaining the reason(s) and the duration for which they may be limited or unable to participate in any PPMH Cheerleading activities.

10 TRAVEL FOR COMPETITIONS A separate packet will be issued for each competition stating specific information, transp ortation/hotel costs and itineraries which all athletes and their families must abide by. FINANCIAL OBLIGATIONS All athletes and parents understand that: 1. They assume full responsibility for all costs incurred as a member of our PPMH Cheerleading program including but not limited to: gym registration, monthly payments, practice outfits, uniforms, competitions, travel expenses, or any other item(s). 2. Monthly costs do not fluctuate based on the number or duration of practices in any month unless otherwise decided by coaches. 3. All payment due dates must be met. Gym and School payments will be charged a $10.00 late fee after the 5 th of each month. 4. An athlete s account must be current and in good standing to participate in practices/competitions/special events. 5. PPMH reserves the right to remove an athlete from the team or make them an alternate at any time for failure to keep up with financial obligations. 6. All payments/fees must be current before an athlete may collect any clothing/uniform or other apparel items. 7. If an athlete chooses to leave at their own will for no excusable reason before the season is over, any and all funds are completely non-refundable.

11 RULES AND REGULATIONS CONTRACT I have read the PPMH Cheerleading Rules and Regulations Contract in its entirety and understand its contents. I understand the responsibility my child is undertaking by becoming a PPMH Cheerleading team member and agree to fully support and encourage them to fulfill their commitment. Furthermore, I understand that being a PPMH Cheerleading team member is a commitment on the part of the parent as well. I realize that when representing PPMH I must always conduct myself with class and responsibility. I understand that any athlete or parent that does not abide by the rules and regulations contained in this contract, that is consistently negative, or acts in a manner that jeopardizes the name and reputation of the PPMH Cheerleading program, will be subject to removal with no refund. In addition, I agree to give PPMH full permission to seek medical attention and/or take any actions deemed necessary to ensure the safety and well being of my child and those around them.
