22 Quince Orchard HS Athletic Department Welcome to the school year. The QOHS Athletic Department has big plans for the upcoming school year and we hope that you will take part in one or more of our 41 different sports, either as a player or as a manager. In your summer mailing you will find information about how to contact any of our Fall, Winter, or Spring coaches as well as information about what paperwork is necessary for participation with a team. Below is the basic information about important dates and requirements for participating in Athletic Events at QOHS. If you have any questions, would like a complete list of eligibility requirements or have any concerns, please feel free to contact our Athletic Director, Lisa Schrader, at Lisa_J_Schrader@mcpsmd.org or ( is the best way to contact her over the summer as she is not always in the office). Eligibility Information: The table below explains what is necessary for participation as an athlete on a team, as a manager, or for participation in a conditioning program. ACADEMIC ELIGIBILITY Have at least a 2.0 with no more than 1 E (Note: ALL Incoming 9 th graders are eligible for Fall Sports) Athlete Participating on a JV or Varsity Team Manager for a JV or Varsity Team Student Participating in a Conditioning Program HEALTH INVENTORY Have a completed and signed current physical on file (physicals are good for ONLY 1 year) PERMISSION FORM Have a completed and signed Student-Parent Permission form on file for each sport/activity MEDICAL CARDS Have a completed and signed Medical Card on file for each sport/activity OBLIGATIONS Students must be cleared of all Athletic Obligations before participating on/with a Team Important Dates: Paperwork Due Date First Day of Tryouts Student/Parent Pre- Season Meeting Sports Awards Night Fall Friday, August 12 th Saturday, August 13 th Thursday, August 25 th Thursday, December 8 th Winter Friday, November 4 th Tuesday, November 15 th Tuesday, November 29 th Wednesday, March 21 st Spring Friday, February 17 th Thursday, March 1 st Tuesday, March 13 th Wednesday, May 23 rd Have a GREAT Summer and we ll see you in the Fall GO COUGARS! 25 Paperwork for Athletes Students MUST have the following on file at QOHS to participate in a Sport. ALL paperwork must be correctly completed, signed and turned in before or during Paperwork Collection: Student-Parent Permission Form (1 necessary for each sport) (completed and signed by Parent AND Athlete) Completed Health Inventory OR Student has Current Physical on file at QOHS (Physicals are good for only 1 calendar year from the date of the exam) Medical Card 1 Page Form Preferred Insurance Form (Football Only) NOTE: All forms can be found online at Fall 2011 Paperwork Collection Information The first day of tryouts for the Fall season is Sat. 8/13, paperwork must be turned in by the end of the day on the 12 th to ensure the student can tryout on the 13 th.
