1 Heart of Texas All- Star Cheer Half Season Cheer First and foremost, Welcome to Heart of Texas. We are excited for you to become a part of our HOT Cheer Family. Founded in 1998, Heart of Texas has a gym that offers something for every child and teen regardless of age or skill there is literally a place to call home for everyone! Outside of training amazing athletes, we pride ourselves on developing great adults as well. Our goal every day is to teach our athletes values of commitment, hard work, integrity, leadership, self-confidence, positive attitude and a love for the sport! It is our goal to be the very best all-star program in the area. Each and every family is important to our program and us. Half Season teams are Sign up only so there is no need to go thru a formal evaluation. The first week or so of practices we will be evaluating athletes and creating teams. Teams are formed by age and ability. Since they only practices one day a week, you normally will not see teams higher than a level 2 Mini thru Senior Age Sign Up Week Information Starts Monday, October 29 th -Nov. 2 nd. Age s 5yrs thru 18yrs; All Abilities and levels Day of Sign Ups Please turn in your Registration form, Waiver and Athlete Information Form. You will turn these in when you walk in to sign up. You will also be making your 1st payment this day. All families are required to keep a current credit card on file. As long as your invoice is paid by the 15th of each month, the credit card is never run. Should your credit card information change at any point in the season, you will need to fill out a new financial form. Practices Practices will begin the week of November 5th All athletes who have signed up for Half Season will come to a one-day evaluation, where all athletes will be evaluated on their skills. That will be held on Monday, November 5th from 5:30-7pm. During this time we will have sizing for cheer team uniforms, and parents will complete the financial obligation forms as well. You will then receive your practice schedule which will include the day, time and team you will practice with for the remainder of the year. Teams practice one day a week for 2hrs. Normally 6-8pm. Practices can be added on, as we get closer to competitions.

Fees are due on or before the 15th. On the 16th, any outstanding balance will be charged to your credit card that you provided in your packet. If you have any questions regarding your account, please do not hesitate to call the front office. Accounts not paid in full will be charged a $25 late fee and the athlete will be unable to participate in practices, tumbling classes or competitions until the account is brought current. There will be no refunds made to anyone who is asked to leave the program or who quits a team. If there is a financial problem, please contact the office immediately. Your coach will not be able to advise you on these matters.

Based on the starting date of Nov ending April For anyone beginning the financial agreement on a later date. The tuition will be based on your start date: I acknowledge that, under the terms of this Agreement, the above named student will be charge all tuition and will be payable on the 1 st of the month, unless other arrangements have been made If at any time during the season you or your cheerleader is unable to fulfill the one year obligation to his/her team, or has been dismissed from the program by staff, you will be required to pay a termination fee of $200 to be processed immediately Termination fees are due immediately at the time of termination. These fees will not be subtracted from any balance you may have paid to the gym throughout the year. Only two exceptions will be considered in making this agreement void: 1- Moving out of town 2- Year ending injury Medical Release and Policy Acknowledgment I, the parent or legal guardian of the above named student hereafter referred to as student, do hereby permit the student, to participate in tumbling, cheerleading or any other physical activities while a student at Heart of Texas Cheerleading LLC. Hereafter referred to as HOT. By granting permission for student to participate at HOT, I assume full responsibility for student s personal safety and release HOT, its directors and employees from any and all liabilities that may arise due to any injury including death to student by reason of student s participation at HOT or in which HOT is participating elsewhere. I understand there is personal risk involved in any activity that involves motion, height, or rotation and that these activities can result in serious injury, disability, or death. I declare student has been seen by a physician and is cleared to participate in physical activities such as tumbling, jumping and cheerleading. I authorize HOT to use photographs, video, and/or other likenesses of student for use in HOT promotional materials or sales and waive any rights of compensation or ownership thereto. 6 Athlete Code of Conduct Acknowledgement HOT All-Star Code of Conduct (Please initial each) I will not be involved in any matter of disrespect towards any members of the Heart of Texas staff, parents or students. I understand that I am required to wear the appropriate practice attire to every practice. I understand the coaches reserve the right to suspend any athlete s participation in activities at Heart of Texas or at Heart of Texas functions as disciplinary action. I understand the importance of punctuality and will arrive at all scheduled events on time. I will not participate in negative gossip or communications that adversely affect Heart of Texas. This includes but is not limited to Face Book, Twitter, Chat Rooms or other Websites. I will not post any Heart of Texas music, choreography, routines, stunts, etc. on any website. I will maintain and/or improve the skills I performed at tryouts through hard work and dedication. I will conduct myself with respect and integrity, displaying positive sportsmanship at all times. I understand all athletes at Heart of Texas are expected to arrive at practices and competitions on time with a positive attitude and give their best effort in every endeavor. I will not use inappropriate language. I understand it is sometimes necessary for athletes to be moved from one team to another. I understand this is a 6 MONTH commitment I am making. I will honor my commitment. I understand that if I quit or I am removed from the program, I will not be entitled to a refund of any kind. 7 Parent Code of Conduct Acknowledgement I understand that all athletes are required to attend all practices and to wear the appropriate practice attire. If habitual absences occur: athletes may be removed without notice and no refund will be provided. PLEASE NOTE that if any athlete misses practices the week prior to a performance or competition it is at the discretion of HOT and its coaches whether or not they perform. I understand the all athletes must attend all schedule competitions. I will notify the director no later than Aug. 1st of any schedule conflicts I understand the only EXCUSED ABSENCES are school function for a grade, death in the family or contagious illness. Anyone threatening to quit or pull their child from a squad may be dismissed from the program immediately. I will not be involved in any matter of disrespect towards any members of the Heart of Texas staff, parents or students. I understand the coaches reserve the right to suspend any athlete s or parent s participation in activities at Heart of Texas or at Heart of Texas functions as disciplinary action. I understand if me child is injured or and not contagious, they are required to attend practice, but not participate I will conduct myself with respect and integrity, displaying positive sportsmanship at all times. I understand all athletes at Heart of Texas are expected to arrive at practices and competitions on time with a positive attitude and give their best effort in every endeavor. I will not use inappropriate language. I understand it is sometimes necessary for athletes to be moved from one team to another. I understand that the parent viewing area should be a positive atmosphere, and I may be prohibited from using the parent viewing area if I cannot maintain a positive attitude while there. I understand that my monthly billing statement is ed to me as a courtesy. Whether I receive it or not, I am still obligated to make my payments on time. If my billing address changes, it is my responsibility to notify the Heart of Texas front office. I understand this is a 6 MONTH commitment I am making. I will honor my commitment.
