7 Medical & Photo Release Classes begin September 2 nd, 2014 and run through May 22, 2015 Medical Release If you, your student or a family member is involved in a Joyful Soles Dance class, activity or outreach you are hereby advised that our organization does not carry Workman s Compensation Insurance for participants, volunteers or family members. If you, your student or a family member should suffer an injury at our studio, participating in our classes, activities or outreaches, you will be personally responsible for all medical or injury related expenses. I give permission for my student, whose birthdate is, to participate in classes, activities and outreaches at Joyful Soles Dance. I also give permission to the instructor or designated adult supervisor in charge to secure emergency medical treatment for the student named above. I also agree to hold Joyful Soles Dance, and/or their assignees harmless in the event of an injury or accident. Photo Release Joyful Soles Dance may want to use a photo or video of your student participating in classes or outreaches on our website. Should the opportunity arise, we may also choose to use it other forms of advertising. The statement below gives us permission to do so. Please be advised that we have your student s safety in mind. Your student s name will not be published with their photos and we will verbally confirm your agreement before posting. I authorize and consent that Joyful Soles Dance shall have the absolute right to copyright, publish, use, sell or assign any and all photographic portraits or pictures, television spots, movie films, videotapes and/or sound recordings, or any part thereof, they have taken or made of my student: (name) or in which my student may be included in whole or in part. 8 Production Participation and Costumes Classes begin September 2 nd, 2014 and run through May 22, 2015 Our students are encouraged to offer all they are, to serve all they can, because of all He is. God s beautiful truths poured from our hearts, through our dances and into the souls watching -Miss A Thankful Heart DANCE presentations November 15 and/or 16 th Pre-School High school Students will take their dances to area Churches, Senior Centers, and Homeless Shelters. There are NO PRODUCTION FEES for our November Outreach. The Lion, The Witch and the Wardrobe DANCE students K-12 - May 2 nd and Joyfully His Pre-School -May 9 th A $50 production fee per family is due on the week of February 1 st for both of these productions. If you have students in both our pre-school and k-12 programs, one $50 fee is sufficient. The Lion, The Witch and the Wardrobe THEATRE students April 18 th - Production Fee $30 per family. (If your student is also involved in DANCE classes, you do not need to pay the additional Theatre production fee) Costume Fees are Due the week of September 2nd for our November production and The week of February 2 nd for our May production. $45 per class= Pre-School (Mustard Seeds I & II) $45 per class = Ages 5-8 $50 per class = Ages 9-12 $55 per class = Ages 13 to Adult. $15 per student = Home School Theatre In November, we will keep our costume cost lower, using regular dancewear and adding skirts, sashes etc. Our May production will require real costumes. 9 Dress Code At Joyful Soles, our goal is to honor God in all we do. This includes honoring Him with our appearance. Please dress modestly. All female dancers must wear a leotard under their clothes. All shirts and leotards must come up to (or just below) the collarbone. If shorts are worn, bike-shorts or leggings must be worn underneath them. Gentleman, the same rules apply. (Except for leotards ) Please wear regular T-Shirts, athletic or dance wear. Shirts should either be long enough to stay down or tucked in during warm ups and tricks. Required Classroom Attire: : Black leotards, pink tights, ballet skirts or shorts. You may wear a wrap or shirt over your leotard, pink ballet shoes and hair in a bun. Jazz: Form fitting dance-wear (leotards, black jazz pants, Capri pants, shirts, tights) black ballet shoes or jazz shoes and hair worn away from face. Tap: Form fitting dance-wear (dance pants, Capri pants, shirts, leotards, tights), black tap shoes and hair worn away from face. Hip Hop/Break Dance: Dance-wear may be looser/baggier, jazz sneakers (for studio use only), wrist guards, knee pads and hair worn away from face. Mustard Seeds I-II-III: Leotards and tights with a dance skirt or, leggings. Please choose dancewear that won t obstruct movement. You will need Pink ballet shoes and black tap shoes. 10 Classroom Expectations & Attendance Classroom Behavior Please arrive a few minutes early to dress for class. Classes will begin on time. Students must wear proper dance clothing and dance shoes (no street clothes/shoes). Only water bottles are allowed in the classroom. Please leave other beverages, food, jewelry and toys with your street clothes in the cubbies provided or in the seating area with your belongings. The studio is a place of education. Please demonstrate respectful and courteous behavior toward your instructors and other students. Students who cannot do this will be asked to sit out of class for the day and parents will be notified after class. "Do unto others as you would have them do unto you." Luke 6:31 Attendance Creating a memorable production requires commitment. As a group we are working to create a dance that will minister to our audience. The success of your class and the quality of the offering you are presenting to the Lord depend on you. We understand students become ill or the weather makes travel unsafe. Life happens Please do your best to be at every class Students who miss an excessive number of classes may be removed from the class choreography. We will not be able to refund your production or costume fees but the costume will be yours to keep. Thank you!
11 Studio Safety Our studio entrance empties into a shopping center parking lot. We are in a nice neighborhood but have no idea who may enter the shopping center. For this reason we ask that our students remain inside the doors until a parent picks them up. Under no circumstances are students age 13 and under allowed to wait outside. Sharing Spaces We are sub-leasing our studio space from Agape Bible Church. They own the lovely black furniture, stacks of chairs, children s church equipment, nursery equipment and everything in the kitchen. We ask that you help us observe the following requests: 1) Please keep all feet and food off of the Black Furniture. It is for bottoms only. 2) Please keep all food in the Fireside Room. Please do not walk about the church eating. 3) Unless directed by your teacher, please do not prop the front door open. 4) If you make a mess, please clean it up. This request applies to restrooms, kitchen, fireside room and dance floors. Thank you 5) Please use only the lights you need. 6) Please use only the upper-front door so we can insure the safety of students and equipment. 7) Please stay off of the stage. The musical equipment belongs to Agape Bible Church. 8) Siblings are welcome to wait quietly in the Fireside Room for Dancers. They are not allowed to run about peeking Into classrooms or playing on the stairs. Thank you for helping us insure their safety and allowing Dancers to learn without distraction. 9) PLEASE No STREET SHOES on DANCE FLOORS. Dirt and small stones can be dangerous to our dancers. Snow Days / Class Cancellations We are located in the Jeffco School District and follow their lead for snow day closures. As a rule, if Jefferson County schools close, we close. If classes must be cancelled due to bad weather, we will send an and post a closure notice to our website Facebook page. You may also call or text for details. Tuition is not refunded for closures Jefferson County posts. In the event of studio closure due to weather, Students may take a make-up class in any age and level appropriate class within 30 days of their missed class. Weather Emergencies: If a Weather emergency; Tornado or risk of one, blizzard, flood etc arises during a class, students will be ushered into the infant nursery until we have an all clear from the county or until a parent arrives.
