1 MIDDLESEX POP WARNER 2010 BLUE JAYS SPIRIT CONTRACT Welcome to the 2010 Middlesex Pop Warner Spirit Program and thank you for your interest in our Spirit Squads. Our organization provides a program which safety, dedication; hard work and sportsmanship are instilled. The philosophy of Pop Warner has been that athletics and scholastics go hand in hand. Pop Warner programs seek to develop well-rounded young men and women who learn not only the fundamentals of football, cheerleading, and dance but also the importance of education in an atmosphere conducive to developing sound character while having a good time. Our goal is for your child to walk away with memories that will last a life time. Attached is a copy of the Middlesex Pop Warner Spirit Squad by-laws. If you would like to register for one of our spirit squads, please read this packet carefully and sign the application. Please also include your registration fee, work bond and uniform bond checks. All checks should be made payable to MPAA (Bond checks should be post dated for November of the corresponding year) Space is limited, therefore, acceptance will be on a first come, first serve basis. Bond checks will not be cashed unless obligations are not met by the parents. Please CAREFULLY read the rules and regulations. Our program requires a great deal of time and commitment from both the parent and child. Retain these by-laws for your records. All coaches will abide by these by-laws. Any appeals regarding an enforcement of a rule by a head coach must be done in writing within a reasonable amount of time to the Cheer Coordinator. Decisions made by the MPAA Executive Board will be final. PLEASE READ ALL INFORMATION CAREFULLY. These by-laws have been compiled in accordance with National Pop Warner. All questions or concerns about your individual squad should be referred to your head coach. The Cheer Coordinator will make final decisions. Squad placement for all participants will be announced at a later date.

2 Parent Requirements 1. Assume all financial responsibilities for loss or damage to uniform and equipment issued to your child. 2. Attend all meetings and scheduled events. 3. Participate in all fundraisers run by MPAA for each child. 4. Submit paperwork by date required. 5. Purchase necessary items to complete uniform for both games/competitions. 6. Work Snack bar at designated home games. One home game per season per registered child and one week night during the summer practice season. Specific date will be assigned by team mom in August. 7. Registration fee is non-refundable after the first week in August. 8. Children CANNOT be left at any practice or game unless an adult coach is present. 9. Children must be picked up from practices/games etc. on time. 10. Participation is mandatory for any additional fundraisers. If you choose to opt out the fee will be determined for each fundraiser per registered child. 11. Inclement weather- coaches/team moms will inform you of practice status. 12. All costs associated with the competitions after the local/rvc level are the responsibility of the parents. ATTENDANCE AT PRACTICES AND GAMES 1. ATTENDANCE IS MANDATORY. You made a commitment to be on the squad. YOU MUST BE AT ALL PRACTICES AND GAMES ON TIME. Squad practice for nights a week in August (Friday will be used as our rain or make-up date ), Three nights a week when school starts. 2. Attendance will be taken at every practice and game by the coaching staff. 3. All absences from practices and games require an official absentee form. This form must be submitted to the head coach prior to the absence. Failure to clear an absence with the form in advance will result in the absence being considered unexcused. Make all appointments,

3 gymnastics/dance classes, etc around your squad s schedule on your day off. During the Pop Warner season, your spirit squad is your TOP PRIORITY over any other extra activity. (Dance class is considered an unexcused absence/late) Three (3) or more excused absences=benched ½ a game Four (4) Unexcused absences = benched an entire game Five (5) unexcused absences = we reserve the right to remove participant from the squad Tardiness (for every 15 minutes late) = benched ¼ of a game. Benched means you must attend game in full uniform and sit with the coaches. 4. Squad members must arrive 45 minutes prior to the game 5. When traveling on the bus coaches must be informed as to whether or not you will be returning on the bus. 6. IT IS MANDATORY ALL SQUAD MEMBERS PARTICIPATE IN ALL COMPETITIONS. ILLNESS A call must be made to the coaching staff no later than one hour prior to the practice/game time. An absentee form must be submitted at the first practice/game that you return to. Failure to follow these instructions will result in the absence being considered unexcused. Further instructions will be given on the first day of practice. APPEARANCE 1. Participants will maintain a neat appearance while in uniform 2. Nail polish or jewelry will NOT be worn.

4 3. Nails can not be longer then fingertips 4. Hair must be pulled back off the face and CANNOT touch the shoulders UNIFORMS 1. You are responsible for keeping your uniform clean and in good condition. 2. Uniforms are to be washed in cold water and line dried. NEVER IRON YOUR UNIFORM or DRY CLEAN YOUR UNIFORM 3. Eating and drinking anything other than water is not allowed while in uniform. A tee-shirt must be worn as a cover up. 4. Uniforms are not to be worn any time other than at games and competitions unless instructed by your coaches. 5. All uniforms are the property of Middlesex Pop Warner therefore they may not be altered in any way unless permission is given by the Cheer Coordinator. 6. All uniforms must be returned CLEAN on hangers at the end of the season. Uniforms not returned or returned damaged will be the financial responsibility of the participant s parent or guardian. Failure to meet this responsibility will result in the participant being ineligible for roster to a squad in the future until said obligation is met. 7. The cheerleading uniform consists of: shell, skirt, briefs (lollipops) socks, sneakers turtleneck, warm-up suits/sweatpants, practice t shirt and shorts, and hair accessory. (Shell and skirt are provided by Middlesex Pop Warner) Briefs, socks, sneakers, t shirt, shorts, Game and competition bow, and turtlenecks are to be ordered thru a vendor of our choice (form TBA) ATTITUDE 1. All participants, parents and spectators must maintain a respectful attitude toward coaches, team members, etc. when traveling to games, competitions etc. You are representing Middlesex Pop Warner and a good example must be set by all. Participants who show disrespect will be benched for one full game and a

5 written warning will be issued. Parents and specters will be issued a written warning. 2. Always display good sportsmanship to the organization, team members and opposing teams. Any issues you might have please refer them to the cheer commissioner. DO NOT TAKE MATTERS INTO YOUR OWN HANDS. SAFETY 1. Safety is the utmost importance to our spirit squads. 2. NO stunting unless a coach is present 3. NO gum 4. NO jewelry is to be worn (medical alert bracelets/necklaces must be secure to the body. 5. NO clips, barrettes, or bobby pins 6. NO long nails (nail polish is NOT permitted a practices, games, or competitions). 7. Practice clothes will consist of shorts (boxers or spandex NO jeans of any kind are permitted) sweatpants, white socks; tee shirts and supportive sneakers (preferably cheerleaders sneakers-especially for flyers and tumblers. COMPETITION 1. Spirit squads will compete in eligible sanctioned Pop Warner Competitions. 2. Tiny Mites and Mitey Mites team are exhibition teams 3. Competition will be held at the following levels: a. Raritan Valley Conference local competition-first Saturday in October Sovereign Bank Arena-Trenton

6 b. Garden State Games TBA If your team does not advance to Regionals c. South/West Qualifiers -Date - TBD Sovereign Bank Arena d. Regional Cheer and Dance Championships Date - TBD e. Pop Warner National Cheer & Dance Championship held in Walt Disney World Early December If a squad advances to Nationals, it is the intention of the organization to support the qualifying team competing at the next level of competition. Parents are responsible for fees at the National Level however, will offset the expenses with available funds from the treasury. we 4. Fundraising will take place through out the season to offset the costs of gyms, transportation, fees and room arrangements. If a squad does not qualify to the National Level, funds will be rolled over in the cheer account for the following year. The funds will be used for National competitions, clinics, training, uniforms, etc. 5. The Middlesex Pop Warner Cheer/Dance season concludes when the last squad is done competing. FUNDRAISING 1. All squads will participate in fundraising events even if their squad is not advancing on to the nest level of competition. 2. Each Participant is required to participate in ALL fundraising events set up by our fundraising committee. Parents who do not participate in fundraising project for EACH CHILD REGISTERED opting out arrangements must be made with the cheer fundraising chairperson BEFORE fundraising begins. 3. If a squad member from any squad chooses not to fundraise, or pay the opting out fee after their competition season is over, the opt out fees will be extracted from their work bond. Fundraising is earmarked as follows but is not limited to the below schedule. At the discretion of the

7 cheer staff additional fundraising can and will be added. Lottery Calendars (general organization fundraiser) Candy Sale-each child is required to sell one box money is due at registration. Additional boxes will be distributed to offset costs if you chose to do so. There will be several other fundraisers throughout the season to support the cost of running the cheer program. When Fundraisers are laid out for the 2010 season, a schedule will be communicated Parental/Guardian Permission and Waiver: 1. PERMISSION TO PARTICIPATE I, the parent/guardian of the above-named participant hereby acknowledge that my child is in good general health and I give my approval for my child to participate in any and all Pop Warner national, regional, league/conference, association and team/squad activities, including transportation to and from the activities by a licensed driver with proof of insurance. 2. INTENT TO INFORM I acknowledge that I am fully aware of the potential dangers of participation in any sport and I fully understand that participation in football, cheerleading and/or dance may result in SERIOUS INJURIES, PARALYSIS, PERMANANET DISABILITY AND/OR DEATH. Furthermore, I fully acknowledge and understand that protective equipment does not prevent all participant injuries, and therefore I do hereby waive, release, absolve, indemnify, and agree to hold harmless the local, league and regional Pop Warner organization(s), Pop Warner Little Scholars, Inc., and any and all organizers, sponsors, supervisors, participants, and persons transporting the above named participant to and from activities, from any claim arising out of any injury to my/our child whether the result of negligence or for any other cause. 3. EMERGENCY MEDICAL AUTHORIZATION I hereby grant my permission for any and all emergency medical/dental treatment and/or first aid to be administered to my child/participant, including authorizing any medical treatment facility/hospital to administer emergency treatment, for any illness/injury/accident resulting from participation in any and all Pop Warner activities. 4. EQUIPMENT RESPONSIBILITY I agree to assume full responsibility for any and all equipment/uniforms loaned to the above named participant and I agree to promptly return, upon request, the uniform and other equipment issued to the above named participant in as good condition as when received except for normal wear and tear. If I fail to adhere to this policy, I will be responsible for the replacement cost of such equipment. I agree to furnish an authentic certified copy of a birth certificate of the above-named participant to local Pop Warner officials. 5. INSURANCE DISCLOSURE I am aware that my local Pop Warner organization carries group accident insurance which is considered secondary or excess for medical purposes to any and all valid insurance I possess is considered primary insurance. Furthermore, I agree to notify in writing my head coach and local Pop Warner organization of any medical claim as a result of participation in Pop Warner as soon as reasonably possible. I understand that any registration fee paid does not constitute a direct premium for insurance and that a deductible(s) may apply.

SCHOLASTIC VERIFICAITON I hereby stipulate that [PLEASE CIRCLE EITHER (a) OR (b)] (a) my child is scholastically fit, or (b) that I have completed the scholastic eligibility form and will adhere to all rules and regulations therein. Furthermore, I hereby authorize my child s school to release school grades, report card results, and any and all other pertinent scholastic information to the local Pop Warner organization in order to comply with Pop Warner s scholastic fitness requirements. 7. FINANCIAL RESPONSIBILITY I hereby stipulate that I have been advised by the local Pop Warner Organization of my rights, if any, to a refund in accordance with the local organization policies, and I have also been advised of my fundraising obligations for the entire season and agree to fully comply with those obligations. 8. ADULT CODE OF CONDUCT: S1: In order to uphold the goals of Pop Warner and ensure that all participants have the benefit of a safe and fun learning environment, all parents, guardians and other adults and attendees of Pop Warner events, including but not limited to practices, competitions, and banquets, must behave accordingly in a respectful, courteous and sportsmanlike manner at all times. S2: Any adult who is using alcohol, tobacco or non-prescription drugs and/or appears intoxicated at a Pop Warner event, and/or who is flagrantly rude, attempts to intimidate, verbally abuse, heckles, taunts, ridicules, boos, throws objects and/or uses vulgarity or profane language/gestures with an official, coach, volunteer, staff member, participant or other event attendee, must receive a verbal warning and/or be asked to leave a Pop Warner event. The member organization may also provide a written warning to the individual regarding the misbehavior. The adult s children may also be removed from the event. Any adult who commits one of the above stated offenses a second time, will be banned from any and all Pop Warner events for a period of one year from the date of the second offense, and their children may also be removed from the program(s) for that time period. S3: Any adult who physically assaults an official, coach, volunteer, staff member or participant or threatens grave bodily harm may be banned from any and all Pop Warner events for one year from the date of the offense, and their children may also be removed from any and all Pop Warner programs for that same period of time. After the ban has expired, if the individual commits another offense of the adult code of conduct, the individual will be permanently banned from any and all Pop Warner events and the individual s children may also be permanently removed from any and all Pop Warner programs. RULES & REGULATIONS I hereby understand and acknowledge that as a parent/guardian of a Pop Warner participant it is my responsibility to comply with all rules and regulations stipulated, adopted or recognized by Pop Warner Little Scholars Inc. or any of its member organizations, including but not limited to the Adult Code of Conduct, stipulated in Section 8 above and published in the Pop Warner Rulebook Any non-compliance with any and all rules and regulations may be cause for discipline and/or dismissal of my child/the participant, myself, and/or any spectators or other persons affiliated with the undersigned and the above named participant.
