3 I. Introduction WHY BAND? It s important to understand the effect that band can have on your life. Of course, as a body of musicians, we desire to produce beautiful music together, but as you already know, band is so much more than just a music class. As you meet members of the community, you would be pleased to find that leaders in every field received their start, and often times credit their success, to the time they spent in their high school band programs. Band is a unique experience that allows us to learn not ONLY music, but also the traits sought after by employers, universities, and your future spouses! Band teaches work ethic, discipline, accountability, responsibility, punctuality, interpersonal/communication skills, and leadership. If you were to poll recruiters at any major university, you would find that most, if not all, would tell you that being in band sets you apart from the rest. Recruiters seek out high achieving students who have given their time and energy to band because they know that these students will succeed amongst the rigors of university life. So, what does that mean for you? It means that if you can make it here, you can make it anywhere! Your hard work and dedication will shine to potential employers and universities! Band is designed to teach you the life leadership skills necessary for success in any of your future endeavors! WHY DO I (AND MY PARENTS/GUARDIANS) NEED TO READ THIS? Yes! The information included in this document guides the operation and decision-making in the Bellaire band program. In a sense, it protects you AND the organization equally. It is in your best interest to be familiar with the policies outlined here! By signing the forms at the end of this handbook, both you and your parents/guardians are agreeing to all aspects included in the handbook. Like any binding document, it is important to read and understand BEFORE you sign up! Thank you for spending some time with this document! The following policies were drawn up not to punish anyone or restrict your personal freedoms, but rather to allow us to be aware of the guidelines necessary for keeping the Bellaire band functioning in a smooth and positive manner, protecting and honoring its heritage. NOTE: Please read this entire document. At the end, you will be asked for a few basic things to authorize and confirm identity. Please make sure you know your student ID #. If moving into the district and you do not have a student ID, please use your birth date. -This handbook is in addendum to all Campus, District, State, and UIL policies. Ultimately, they are the bosses! -Parents/legal guardians AND students must read this handbook, and sign and return the acceptance form. Here we go!

4 II. Ensembles Mighty Cardinal Band (MCB) This marching ensemble begins practice July 31st, 2017 and operates until the end of football season. Student leadership, drumline, and color guard may observe extra rehearsals during the summer and school year. During the fall semester, the marching band practices after-school 4pm-6pm. Please see the calendar on the website for details. The band performs at all Varsity pep rallies, football games, and participates in marching band competitions during several weekends in October. Before-school, afterschool, and weekend time is required until early to mid-november, depending on the Bellaire football playoff schedule. The MCB consists of the combination of students from all Bellaire band ensembles. However, ultimate enrollment/placement is at the discretion of the directors. Wind Ensemble This is Bellaire s top concert band ensemble. The Wind Ensemble plays advanced band literature and practices during the school day. Enrollment is a year-long commitment. This ensemble performs concerts, and competes at local and UIL events. Required after-school time -- which consists of a weekly sectional till spring break, and two full band rehearsals after school from Spring Break till UIL (See calendar for details) -- typically begins in November. Members of the Wind Ensemble are also strongly encouraged to enroll in private lessons. Enrollment / placement requires audition / director approval. Symphonic Band This intermediate-level concert ensemble practices during the school day. Enrollment is a year-long commitment. This ensemble performs concerts and competes at local events. Required after-school time -- which consists of a weekly sectional plus occasional music clinics, and occasional extra concert rehearsals -- typically begins in November. Enrollment / placement requires director approval. Beginning Band This class is for students who have never played a musical instrument, have little experience with a musical instrument, or students who wish to switch to a new musical instrument. The Beginning Band practices during the school day. The beginners in this class period may combine to perform as a beginning band ensemble at a concert. Enrollment is a year-long commitment. Enrollment / placement requires director approval.

5 Percussion Ensemble This ensemble practices during the school day. Enrollment is a year-long commitment. This ensemble performs at all football games, Varsity pep rallies, and participates in all marching band competitions. This ensemble also performs concerts and competes at local events. After-school practices with the MCB are required, as is other after-school time throughout the school year. Enrollment / placement requires director approval. There also may be a fee attached for participation in this ensemble. Participants must have at least 1 (one) year of percussion education experience in order to participate in this ensemble. More specific information and policies may be outlined in a supplemental handbook. Jazz Bands (Ensemble and Combos) Bellaire offers an after-school jazz band for students wishing to learn and/or enhance their jazz knowledge and skills. Prior experience in jazz bands is not required. Participation in the jazz band is optional and there is no credit or grade, however attendance at the weekly rehearsals and sectionals is mandatory. The Bellaire Jazz Band will perform occasional concerts and participate in occasional competitions. Enrollment / placement requires director approval. Auxiliary Color Guard members will receive an addendum to this handbook specific to Color Guard. All marching band rehearsals/performances are mandatory. Chair Placement: In May before each school year, each student may complete a chair placement try out. The results of that playing test will determine the initial ensemble, as well as chair and part placement. Students who do not audition will be placed in either the symphonic band. Percussionists will need to visit with Mr. Dudney about placement. Students wishing to move up after the initial chair placement is complete should visit with the director. Additional chair placements through the year may take place at the band director s discretion. This does not apply for percussion. Varsity / Junior Varsity Status: Varsity members perform at all marching performances and contests. Junior Varsity members may perform from the sidelines or in a junior varsity field spot during halftime performances, but will not perform at ANY contests of any kind. The directors will determine who marches in an on-field J.V. spot and who performs from the sidelines. In order to maintain Varsity status a student must do ALL of the following: Be UIL eligible to participate in band

6 Attend all competitions, rehearsals, and performance events. Emergencies will be considered on an individual basis. Documentation of the situation will be required. Failure to plan, jobs, needing to babysit, and problems of this sort are not considered acceptable reasons to not attend/perform. Demonstrate their musical and marching skills by passing off their music and marching fundamentals test throughout marching season

7 III. Classroom and Rehearsal Expectations Summer Band Camp: Every rehearsal is vital for our success. Students are expected to attend all summer rehearsals. If there is a problem with attending, please contact the director immediately. Students that do not attend summer rehearsal may be placed in a Junior Varsity status. IN GENERAL: 1. ALWAYS BE ON TIME. If you are not early, you are late! 2. After-school rehearsals/sectionals are EXTENSIONS of the school day. All school/district policies will be followed. 3. Always be prepared. Students must have all materials needed for a successful rehearsal. Always have your music, instrument, pencil, and mind ready to go! 4. Students must show respect for all band members, directors, and parents at all times! 5. Students must show respect for all property. Every area the band visits should be left in better condition than it was before we entered it. 6. The uniform must be worn correctly at all performances unless otherwise stated. 7. Students must have music prepared. During marching season, this means having all the halftime show music memorized. 8. No food or drinks (besides water) are allowed in the band facilities. They also have no place in rehearsals/performances. 9. Proper language is to be used at all times! 10. Public Display of Affection is not for rehearsal/performances or the band hall. PDA will not be tolerated at any time. 11. Be responsible! Be accountable! Be ready and prepared for rehearsal! 11. Band should be fun! Remember, the more effort you put into the program and making every rehearsal successful, the more fun and worthwhile your experience becomes! BAND HALL POLICIES: 1. The band room should be kept neat and orderly AT ALL TIMES. It should exemplify the pride and organization that reflects the program. Students should clean up around his/her own area and instruments are to be restored in their designated places. 2. There will be NO FOOD OR DRINKS in the band hall at any time (this includes gum and candy). Water is okay. 3. Hats will be removed when entering the band hall.

8 4. Non-band students will not be allowed in the band hall. Band students have earned the privilege to be in the band hall. 5. Students will only handle things that belong to them. No one should touch the instrument or personal belonging of another student. For non-percussionists, this means no touching the percussion instruments unless directed to they are owned by the school. DISCIPLINE: The H.I.S.D. Student Code of Conduct will be enforced at all times. The Extracurricular Code of Conduct will be enforced at all times. The MCB Handbook will be enforced at all times, and will be used as a supplement to the HISD Student Code of Conduct Handbook and Extracurricular Code of Conduct. REPORT DUTY: During the marching season, we observe a "report duty" system in order to keep rehearsals running effectively. "Report duties" are conducted after each rehearsal with a plurality of officers; and director present. The administering officers must ALSO complete the respective exercises with the individual/group. IMPROVEMENT PLANS: If necessary, the director may issue a written improvement plan that outlines the behavior changes needing to be made, and consequences should the changes not be made. ASSESSMENT AND GRADING: The TEA allows for after-school events (such as rehearsals/sectionals) to affect the final grade (see the TEA-UIL Side-by-Side document on the UIL website for more details). The following methods will be utilized in determining the six weeks grades. Grading will follow campus and district policy. -Daily Assessment -Weekly Assessment -Six Weeks Assessments -Performances -After-school Rehearsal -Semester Exams Academic Eligibility: It is the student's responsibility to remain academically eligible. Students ask an upperclassman if you need some extra help in a subject! We want you to be able to play with us at events! Please refer to the HISD Student Code of Conduct handbook for academic eligibility guidelines. ABSENSES Rehearsals and sectionals are EXTENSIONS of the school day. If you are well enough to attend school, you are well enough to attend the rehearsals, performances, events, etc. Work, detention, tutorials, Saturday School, babysitting, vacation, and obtaining drivers licenses are not acceptable reasons for being absent. In order to be excused for class time absences, the student/parent must follow the campus guidelines. In order to be excused for any after-school events/rehearsals/performances, etc.,

9 campus policy must be followed AND the director must receive documentation verifying the need for the absence. Acceptable sickness documentation for after-school events/rehearsals/performances is either a doctor s note or verification from the Bellaire HS nurse confirming the student's illness. Family, parent and/or guardian notes will not excuse a missed performance. Unexcused Tardy to Class / Rehearsal / Performance The campus tardy policy will be followed, and, if during marching season, will be counted as report duty.

10 IV. Parent Participation Parents are encouraged to participate in as many band-related opportunities as they are able. In order to ensure adequate parental support for the band, students must have 10 parent participation points in order to sign up for the Spring Trip. Parent Participation Points for additional fundraisers will be advertised throughout the year. Points may be adjusted according to donation size for fund-raising events (car wash supplies, baked goods, items for UIL concession stand, etc.) Activity Points 3 Alter and fit uniforms for new and returning students (some will be fine from the previous year) you will be assisting the Uniform Chair(s) 1 Serve meals before game/competition/rehearsal about hours. These spots are very limited and will fill up fast. 2 Chaperone games and competitions 4 Chair an approved fundraiser selling product pass out forms, send reminders, turn money in to Treasurer & coordinate pickup of product (there are 5-7 events per year, this point allotment is for 1 event) 8 Chair Lasagna Dinner in February 1 Work a 2-hour shift during the Lasagna Dinner 8 Chair the UIL Solo & Ensemble Concession Stand in February 1 Work a 2-hour shift at the UIL Solo & Ensemble Concession Stand 1 Attend a Mighty Cardinal Band Booster meeting (first Tuesday of month, 7pm) 10 Serve as Uniform Coordinator, Fundraiser Coordinator, Lasagna Dinner Coordinator, Trip Coordinator, Chaperone Coordinator, or MCBB Officer TBD Assist Color Guard points will be determined by color guard coordinator and band director 1(+) Send donations (baked goods, supplies, etc.) for car washes, baked goods sale, Bellaire Teacher Breakfast, UIL Solo & Ensemble concession stand, etc. ***If unexpected opportunities appear during the year, the director will post them and the point allotment for them on the website

11 V. Band Physical The band physical coordinator is Mr. Dudney. All completed physicals will be turned in to Mr. Dudney. Each year a student is required to complete a physical. This can be done by going to Bellaire E.R., or to any doctor/clinic you wish. The physical packet is available on the PHYSICALS link on the website. Your completed physical is to be returned with the other forms in the band packet on Registration Day. VI. Performance Dress Code / Etiquette Stadium / Performance Etiquette: The Mighty Cardinal Band takes our safety and public image very seriously. When in uniform, students are expected to conduct themselves in a safe and professional manner and obey the HISD Student Code of Conduct at all times. 1.) During football games and marching competitions, MCB members enter the stadium, execute the game, and exit the stadium as an organized group. All MCB members must bring their filled MCB water jugs to EVERY game. 2.) When in the stands, band members sit with their sections, and maintain a clean orderly workspace. 3.) At all times, instruments/equipment are to be treated with the greatest of care. 4.) In order to protect uniforms and equipment, no food or drink is permitted in the stands. Members may have water only. 5.) Uniforms are to be worn correctly, and in full, unless the director instructs otherwise. 6.) Students are not to leave the stands (even to use the restroom) without a band parent volunteer. Marching Band: All students must wear full uniform and marching shoes unless otherwise directed plus black calf-length socks. The MCB band shirt MUST be worn under the uniform coat. It is recommended that students keep an additional pair of socks in their uniform bag. Overall straps must always be over the shoulders unity is key! Wind Ensemble: Ladies must wear (and purchase) the approved Wind Ensemble concert dress with black dress shoes. Gentlemen must wear (and purchase) the approved Wind Ensemble tux jacket and shirt, as well as a black bow tie, black dress slacks, calf-length black socks and black dress shoes. A link to order the concert dress or tux is provided in the member section of the band website. Symphonic Band, Beginning Band, and Jazz Band: Ladies must wear solid black blouse with sleeves and solid black pants (no denim) or a solid black skirt, with black shoes. Skirt must be mid-calf or floorlength. Solid black blouse should show no cleavage. Gentlemen must wear white, collared, dress shirt, with black pants/slacks (no denim), calf-length black socks, and black shoes.

12 VII. Communication and Staying Informed There are several ways for parents/guardians to stay informed The MCB website This contains general information about the MCB, updates from the MCB Band Director and a calendar of events. (bellaireband.com) Remind All students and parents are to be current on the Remind system. This system gets information to you by text at no charge to you. Instructions: 1. GO TO THIS LINK ON YOUR PHONE OR COMPUTER remind.com/join 2. ENTER THIS CLASS CODE TO A monthly bulletin from the MCB Band Director (this will also be on the website) communications from the Mighty Cardinal Band Booster (MCBB) Board Charms Every student has a Charms account which will indicate any balances with the booster organization. Monthly MCBB meetings These meetings are OPEN TO EVERYONE; this is a great way to learn and have a say in how the band works and meet other Bellaire band parents/guardians. Facebook and Twitter Yes, we are on social media! These groups are for students and parents and are very fast ways of getting up-to-the-minute information (along with Remind 101, see above). Photos and videos of the Mighty Cardinal Band will also be occasionally posted! Volunteering Besides offering your valued support to the MCB, this is one of the best ways to meet your student s band friends and meet other parents/guardians! It's also a lot of fun!

13 VIII. Explanation of Fees/Expenses Mighty Cardinal Band Fee: $ Instrument Usage Fee: $50.00 If you use a school instrument, a one time, Non-refundable $50.00 fee must be paid in the front office or online through the school. This fee is in addition to the regular band fee. To get the instrument, the online registration must be complete, and a copy of the receipt of payment of the fee must be turned in to the director before an instrument will be checked out to the parent/guardian. Additional Expenses: Additional expenses include concert performance attire,, and the optional Spring Break trip. More information on specific costs will be provided after the start of the school year. NOTE: The Bellaire Mighty Cardinal Band believes every student who has the desire, motivation, and determination to be in band should get a fair shot at participating. If a parent/guardian is unable to meet the scheduled deadline to pay the full band fees due to financial difficulty, it is the parent s responsibility to let a Band Director know BEFORE the deadline so alternative arrangements (replacement fundraising, service hours, etc.) can be made.

14 IX. Student Leadership After UIL Concert/Sight-Reading concludes in the spring, information for leadership auditions will be distributed to those interested. The MCB Leadership Team consists of three subsets: the Officer Team, Section Leaders, and Drum Majors. Officer Team: Officer Team members are elected by current band members. These students are expected to help the booster club plan and execute fundraising and band events. Officers help convey student ideas and concerns to the boosters, as well as relay pertinent information from the boosters to the students. Officers are, just like the rest of the leadership, to abide by the standard of excellence and conduct themselves in an appropriate manner consistent with the expectations set by the directors. BAND PRESIDENT Duties: -Preside as any officer, in their absence. -Help direct the achievement of the Mighty Cardinal Band. -Make sure rules are being abided by members of the band. -Serve as the liaison between the band members, faculty, community and administration and the band director. -Student figurehead/spokesperson of the band for the school -Serve as student representative of the band when called upon. -Plan Officer & Leader Social Activities. -Serve as Band Banquet Committee Chair. VICE-PRESIDENT Duties: -Preside when the president is absent or is no longer in office. -Assist the President in all his/her duties. Serve as student representative of the band when called upon. -Coordinate Birthday Recognition's. HISTORIAN/LIBRARIAN Duties: -Take, order, and save pictures for end of the year slideshow. -Collect/Keep an updated list of all band members and information. -Share in the responsibility of distributing and collecting music. -Make appropriate copies of music. -Notify band director of any band members who request excessive amounts of music. -Keep the music library neat, catalogued, and in order. TREASURER Duties: -Maintain and budget money for student activities and fundraisers. -Oversee equipment needed for such events. -Assist in rectifying financial issues with the Band Boosters. SECRETARY Duties:

15 -Take attendance at practices, games, and officer meetings. Notify students when report duty is assigned. -Take detailed notes (minutes) at officer meetings. -Serve as an assistant for any of the above officers should they need it. Section Leaders: Section leaders (including drum captain and guard captain) are expected to be skilled in their areas of work. They must be able to be present at any and all rehearsals and possess good interpersonal and communication skills. All section leaders/captains are expected to work in conjunction with the other section leaders, drum majors, and with staff. All leadership at this level is expected to provide musical leadership answering questions, helping with technique, etc. and organize sectionals when necessary. Attention to detail and a willingness to take the initiative is imperative of leaders here as they are a direct link to the other members of the band. The "spec ops" captain falls under section leader, as well (see below). SPECIAL OPERATIONS CAPTAIN Duties: -Communicate the duties of Special Operations to members of the crew. -Communicate any hardware/equipment issues with directors. -Oversee, delegate, and take charge for the set-up/breakdown/loading/unloading at all band events. Drum Majors: Drum Majors are student conductors for the marching band during rehearsals and performances. They also carry out the instructions of the director/s in regards to what needs to be done with the band and auxiliary units. Drum majors may be expected to move the band in and out of rehearsal spaces, take the band through warm-ups, assist in teaching marching technique to other band members, and are able to add to the overall showmanship of the band during performance with their own performance as the drum major. The MCB drum major must attend the University of Houston camp or a similar camp that is preapproved by the Bellaire band directors. The band directors will discuss camp expenses with the new drum major. This camp is not optional. If you are not willing to attend, do not try out. Returning drum majors must attend the camp EVERY year. AUDITION PROCESS FOR ALL LEADERSHIP POSITIONS: All those auditioning any of these leadership positions go through the same audition process. In April, the directors will pass out "Leadership Applications" to any students interested. The candidate will then turn this form in during an interview with the directors. Then, the directors will conduct "call backs" for those students that have been "approved" for their respective position(s). These approvals are based on behavior record, grade record, spirit as a bandsman/bandswoman, leadership potential, and skill on their respective instruments. When you are approved as a band officer candidate, the band will conduct a blind vote for these selections. When you are approved as a section leader candidate, or drum major candidate, you will go through an audition process. The details of these auditions will be outlined when the initial leadership application form is passed out. The exact audition contents may change from year to year.

16 *GENERAL NOTES: -A student MAY audition for no more than two simultaneous roles per year (e.g. Band President and Flute Section Leader) -According to the ultimate discretion of the directors, any leadership position may be held by a plurality of students, or may not be held at all! -If you are selected to be a part of the MCB Leadership Team, you MUST be present at the summer leadership camp dates set on the band calendar! X. Fundraising The state of Texas and Houston ISD have laws, regulations, district policies and guidelines on how all student money is to be handled. All campus fundraised monies will be deposited, and spent according to state and district guidelines. All booster fundraising and spending is governed by the booster by-laws, and district/state/federal and IRS guidelines. XI. Transportation to and from Games/Competitions/Events/Trips Travel is a unique privilege that comes with membership in our ensembles. The HISD Student Code of Conduct and the MCB Handbook policy for student behavior will be strictly enforced. When transportation is provided, all members participating in that event must ride the school-provided transportation. Any exception must be discussed with the band director PRIOR to the event. Students will be allowed to leave with a parent/guardian after an event ONLY with a written request submitted to the band director AFTER the performing event by a parent/legal guardian NO EXCEPTIONS. GENERAL TRAVEL POLICIES Students who represent Bellaire High School on trips/events (this includes away-games!) are expected to maintain very high standards in every respect at all times. They are expected to attend and be on time for all sessions/activities scheduled for them. In the event of overnight trips, they are expected to be in their rooms at the times designated by their director and chaperones. You have the opportunity to make a great name for Bellaire and for yourself. Be courteous and polite at all times. If there is any question in your mind as to whether you should do something or not don't do it! All eligibility rules apply for all games/competitions/events/trips. XII. Student Money While Traveling Overnight We highly encourage parents to purchase a prepaid Visa card that can be pre-loaded with spending money. If the student loses the card, they can call VISA and the money is not lost. Taking photos of the back and front of the card will facilitate reimbursement for a lost card. Also, the parent can go online and see what the student is purchasing, eating, etc.

17 XIII. MCB Letter/Jacket Policy Students will earn their letter jacket with white letter if they have not previously earned a Bellaire letter jacket if a student has earned a Bellaire jacket in another activity, the student will receive a band letter and appropriate award(s). Bellaire High School provides only one jacket per student.) Students must accomplish one of the following: Complete three years in the band program plus a fourth marching season First Division on a Class 1 solo at UIL Region Solo and Ensemble Make the TMEA All State Band An unusual accomplishment deemed worthy by the MCB Director and the MCB Boosters. XIV. Bullying, Harassment, and Social Media Policy This policy applies to band members, their parents, all relatives, and friends posting on a band member s behalf. Both the Federal and State courts have upheld that Freedom of Speech does not allow bullying or harassment. No one associated with the Mighty Cardinal Band is allowed to bully or harass anyone at any time. This expectation includes any form of internet post, profile, blog, or website connected to a Cardinal Band Member, their Parent, Relative, or Friends posting on a members behalf. You may not at anytime make a post of any kind that can be deemed bullying or harassing of anyone, including anyone associated with the band, the school, or the community. You are responsible for your profile. Your profile is an extension of you and directly affects and reflects upon the band. The following is required of your internet profiles and personal behavior: Do not bully/harass anyone on social media/chat outlets, and do not even participate in such harassing or negative conversations. Do not post negative comments related in any way about any school, district, band, or anyone. ***Violating this policy can result in immediate removal from the band program. If a student or parent violates these expectations, the student or parent may be removed from the band program and the MCB Boosters organization immediately. XV. Equipment Usage Policies Instruments owned by parents/students (NOT HISD instruments): -HISD is not responsible for personal owned instruments.

18 -It is the owner s responsibility to keep records of purchasing cost, serial number, make, and model. -It is the owner s responsibility to make sure that their insurance company has all required information should a claim be necessary. -It is wise to have a photo of the instrument that also includes the serial number. This will facilitate insurance claims. Usage of MCB instruments: -Please see section XVI for specific information. XVI. The 8 Hour Rule UIL Marching Regulations No student may be required to attend practice for marching band for more than eight hours of rehearsal outside the academic school day per calendar week (Sunday through Saturday). This provision applies to students in all components of the marching band. On performance days (football games, competitions and other public performances) bands may hold up to one additional hour of warm-up and practice beyond the scheduled warm-up time at the performance site. Multiple performances on the same day do not allow for additional practice and/or warm-up time. Examples of activities subject to the UIL Marching Band Eight Hour Rule: Marching Band rehearsal (both full band and components) Any Marching Band group instructional activity Breaks Announcements Debriefing and viewing Marching Band videos Playing off Marching Band music Marching Band sectionals (both director and student led) Clinics for the Marching Band or any of its components The following activities are not included in the 8-Hour Rule: Travel time to and from rehearsals and/or performances Rehearsal set-up time

19 Pep rallies, parades, and other public performances Instruction and practice for music activities other than Marching Band and its components NOTE: An extensive Q&A for the Eight Hour Rule for Marching Band can be found on the Music Page of the UIL Web Site at: XVII. HISD Musical Instrument Usage Agreement The Houston Independent School District (HISD) provides certain musical instruments and instrument cases. The non-refundable usage fee is $50 per year due at instrument checkout. Instruments are checked out to parents/legal guardians and not to students. The parent/legal guardian is financially responsible for instruments and instrument cases checked out to parents/legal guardians until properly returned and accepted by the Bellaire band director. Parents/legal guardians agree to return the instrument and instrument case to the Bellaire band director at the end of the semester or academic year, or upon Bellaire band director s request, in the same condition it was issued to the parent/legal guardian, less reasonable wear. Damage beyond normal wear or loss of the instrument and instrument case while in the parent/legal guardians possession becomes the responsibility of the parent/legal guardian to pay for repairs or replacement. Replacement cost will be determined by obtaining three (3) quotes from HISD-approved instrument vendors. If, in the opinion of a Bellaire band director, an HISD instrument and instrument case is not being properly cared for at any time during the school year, it is the Bellaire band director s prerogative to remove the instrument and instrument case from the parent/legal guardians care. The parent/legal guardian may also be required to surrender the instrument and instrument case if the band member fails to meet practice and performance requirements, or violates other regulations as set forth by the Bellaire band director. If the instrument and instrument case is to be taken home, HISD encourages parents/legal guardians to cover these instruments on their homeowners or renters insurance policy.

20 XVIII. HISD Band Uniform Usage Agreement The Houston Independent School District (HISD) provides a marching band uniform. Uniforms are checked out to parents/legal guardians and not to students. Parents agree to return the full uniform to the Bellaire Band director at the end of the semester or academic year, or upon the Bellaire band director s request, in the same condition it was issued to the parent/legal guardian less reasonable wear. The parent/legal guardian is financially responsible for uniforms checked out to parents/legal guardians until properly returned and accepted by the Bellaire band director. If the Bellaire band director determines that there is uniform damage beyond normal wear, or if parts of the uniform are missing, or if the uniform is lost while in the parent/legal guardian s possession, the parent/legal guardian is responsible for all repairs or replacement. The replacement cost of a Bellaire band uniform is $400. If, in the opinion of the Bellaire band director, an HISD uniform is not being properly cared for at any time during the school year, it is the Bellaire band director s prerogative to remove the uniform from the parent/legal guardian s care. The parent/legal guardian may also be required to surrender the uniform if the student fails to meet practice and performance requirements, or violates other regulations as set forth by the Bellaire band director. When returning the uniform, the full uniform must be returned clean with a dry cleaners receipt no older than 10 days. If the uniform is returned without a receipt, the parent/legal guardian will be charged the cleaning fee and the student s records will be held until the cost of cleaning is reimbursed by the parent/legal guardian. If the uniform is to be taken home, HISD encourages parents/legal guardians to cover the uniform on their homeowners or renters insurance policy.

21 XIX: HISD Permission to Video Tape / Photograph Consent and Release of Liability I do hereby give the Houston Independent School District and its employees, agents, representatives or employees of legitimate news organizations my full permission to photograph, videotape, copyright, broadcast, reproduce, telecast or cablecast, use on the internet or intranet, publish or otherwise use my child's works or my child s photo or likeness for educational, broadcast or news purposes. This is with the understanding that neither the Houston Independent School District nor its representatives will reproduce said photograph or likeness for any commercial value or receive monetary gain for use of any reproduction/broadcast of said photograph or likeness. I am fully aware that I will not receive monetary compensation for my or my child s participation. It is understood that the Houston Independent School District, its agents, representatives or legitimate news organizations may use this material with or without my name or my child's name at its discretion. I further release and relieve the Houston Independent School District, its Board of Trustees, agents, employees or other representatives from any liabilities, known or unknown, arising out of the use of this material. I certify that I have read the Consent and Release of Liability statement and fully understand its terms and conditions. I understand that the opportunity to participate is given by the Houston Independent School District and I have full legal capacity to sign this Consent and Release for myself and/or my child.

22 XX: Parent/Guardian Release Form Required for Student Participation in a Field Trip/Tour/Excursion/Off-Campus Performance On the next page is the list of events, times, dates, locations, etc. you are approving your student to attend I desire for my child to go on this field trip/tour/excursion and participate in any and all activities of the Bellaire High School Band. I agree that in allowing my child to go on this trip that I will hold the Houston Independent School District, Trustees, Superintendent, Principals, Teachers, Employees, and any other persons assisting with any phase of such trip harmless from any and all liability, claims, and responsibility for making such trip and activities. I further release all of these parties from liability by reason of accident or injury that might occur while on the trip or participating in such activities. I understand that if a time, location, or date changes that the information will be sent home with my student and be posted on the band website and that change does not negate this permission slip in any way.
