3 Studio Rules and Regulations 1. Fees are payable strictly in advance every month before the first class and thereafter every month. Fees should by paid by EFT or Bank deposit no Cash will be accepted at the studio. 2.Enrolment is for a period of 1 calendar year. Fees are annual devided over 10 Monthy instalments Fess include a R registration fee. There will be no classes held on public holidays or during school holidays unless organized by the teacher/s with written notification. 3.1 Termination of classes can be given in writing but as the classes are for the calender year on receipt of termination notification the remaining balance of the yearly fee will be payable in full. 4.Extra classes may be scheduled at the discretion of the teacher/s for Examinations and Competitions. These classes will be charged at the following rates: R80.00 per hour Group Class R per hour Private Class R per hour Pianist Fees *these rates are subject to change at any time and without prior notification 5.Examination Fees and competition entry fees are not included in the term/trimester fees. A written notification of the fees will be handed out to eligible students at least 1 month prior to the exam/competition. Payment of these fees should be made at least 2 weeks/14days before the examination/competition date. If these fees are not received the teacher/s is within his/her rights to withdraw the student from the Examination/Competition. 6.All Examination/Competition entries are solely at the discretion of the teacher/s. No negotiations will be entered into over these entries and students entered/not entered for the Examinations/Competitions 7.Students should attend classes in the proper dance attire/uniform. Hair should be tied back off the face. No jewellery or other accessories will be allowed to be worn. 4 Uniform List All uniform can be ordered on line via our WEB SITE Ballet: Pink or Burgandy leotard (according to class level) Pink ballet shoes and ballet shoe covers Pink or Burgandy ballet skirt Modern: Royal blue camisole strap leotard (Primary- Preliminary) Bare beige footed or footless stockings Pink or black ballet shoes Unitard students own choice of colour (Elementary and above) Tap: Royal blue camisole strap leotard Bare beige footed stockings Black tap shoes (Fixed taps are preferred for better sound quality) Hip Hop: Black pants/leggings Red T'shirt/vest top Knee Pads Sneakers/takkies (No split sole dance sneakers are allowed) Any breach of the attached conditions / late payments that would incur legal costs will be for the dancer, parent/guardian's costs. Code of Conduct and Decorum For Students Students will: Arrive at least 10 minutes before the start of class to prepare for class and to warm up appropriately. If students are late to class, they may be refused entry to the class. All personal belongings, including handbags, will be neatly stored in the lockers and dressing room. No cell phones are allowed in the dance studio during class The waiting area, bathrooms, changing areas and studio space will be kept neat and tidy at all times Students will be dressed appropriately: leotards and tights or ankle socks, ballet skirts are optional, ballet flats or pointe shoes; for all other dance styles, a leotard with leggings or shorts, footless tights are optional and the appropriate shoes or footwear Ensure that their dance clothing is clean and in good condition at all times Hair will be neatly tied up and pulled away from the face in a ballet bun or ponytail or any other appropriate hairstyle Show respect for staff, parents, siblings and other students Will encourage fellow dancers to learn and achieve in their dance classes Keep to a regular class schedule, except in the case of injury, a family emergency or an illness Should a student be unable to attend their regularly scheduled classes, the student or parent /legal guardian will notify The Studio immediately, and when necessary, will furnish the Studio with a note from a medical professional Will refrain from using inappropriate language in class and around The Studio premises No chewing gum or food is allowed in the dance studio. Water may be brought into class. 4

5 Code of Conduct and Decorum For Parents, Legal Guardians or Carers Parents, Legal Guardians and Carers (including nannies or au pairs) will: Ensure that students arrive on time for each class Not enter the studio space before or during a class, rehearsal or exam, unless explicitly invited by the dance teacher in charge Be respectful of staff, students, other parents and children at all times Will not use inappropriate language on The Studio premises or around students and staff at the Studio Notify The Studio if a student is unable to attend class and will provide a letter from a medical professional, when necessary and at the request of the dance teacher in charge any concerns with the dance teacher in charge after class or via or by making an appointment to discuss the concern Adhere to exam and performance rules and regulations Collect students promptly after the end of any class, rehearsal, performance, exam or any other activity at The Studio. Students cannot remain at the studio for more than 15 minutes after the end of class or any other activity. Be responsible for understanding the class, rehearsal, exam and performance schedules and times, ensuring that the student is prepared for any dance class or activity and regularly check communications from The Studio to ensure that schedules are adhered to Ask for permission before taking any photos or videos, in particular before posting these photographs or videos to any social media platforms 5

I hereby release The Beverly Acquisto Studio of Dancing, its affiliates dance studios, event sponsors, employees and dance teachers from all liability for injuries sustained or illnesses contracted while attending or participating in any dance classes, rehearsals, workshops, or performances. I agree to indemnify, defend, and hold harmless The Beverly Acquisto Studio of Dancing, its affiliate dance studios, event sponsors, employees and dance teachers for liabilities, costs and judgments arising from acts of omissions committed by me or my child which result in injury or damage to any person or property. I also agree to abide by any rules, regulations and policies set forth by The Beverly Acquisto Studio of Dancing. Initials. In case of physical injury or medical emergency, I hereby authorize The Beverly Acquisto Studio of Dancing to make necessary arrangements to transport myself or my child to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. In extreme emergency, or if my child is under 18 years of age, I understand that The Beverly Acquisto Studio of Dancing will attempt to notify the person(s) I have named above as my emergency contact(s) of my condition and how to reach me.
