4 Green Valley High School Cheer Student and Parent Agreement My child,, has permission to participate in clinics and tryouts and to be a cheerleader at Green Valley High School. I understand the tryout process, and that all decisions are final. I understand that by the very nature of the activity, cheerleading carries a risk of physical injury. No matter how careful the participant and coach are, how many spotters are used, or what landing surface is used, the risk cannot be eliminated. The risk of injury includes minor injuries such as muscle pulls, dislocation, and broken bones. The risk also includes catastrophic injuries such as permanent paralysis, or even death, from landing or falls on the back, neck, or head. I understand these risks and will not hold Green Valley High School or any of its personnel including, coaching staff and students, responsible in the case of accident or injury at any time. I shall not use profanity, abusive language or actions toward any coach, official, parent, cheerleader, advisor, or any other person present: before, during or after any practice, game or GVHS function. I will not coach from the sidelines, nor incite or taunt the coaches or anyone acting in an official capacity. I will not harass, ridicule, abuse, use profane actions to said coaches, advisors and administrators, nor second guess their decisions. If I have a suggestion for improvement, I will review this with a coach or administrator at an appropriate time. I agree to support the program as stated in the constitution, and understand that all possible situations that may arise during the year cannot be covered in the Green Valley High School Cheer constitution and that the coach(es) will make final decisions on any and all situations that may come up. I recognize that coaches have invested a great deal of time and energy to ensure my child will have a successful season. I understand that coaches will make decisions for the good of the entire program. I fully understand the time and financial commitment involved in being a cheerleader and that my son/daughter must abide by all rules and regulations as stated in the constitution. 9 Cheerleader Constitution Agreement I,, have read the Green Valley High School Cheerleader Constitution regarding the expectations for members of a cheerleading squad. I understand that it is my responsibility to abide by the rules and regulations as outlined in the Constitution if I am selected as a member. I further understand that any violation of the rules or regulations may result in permanent or temporary suspension or removal from the squad. Signature of Candidate Date Parental/Guardian Permission and Constitution Agreement I have read the GVHS Cheerleader constitution. I agree to my child s participation in the tryout procedures for this performing group. I believe my child to be in good health and to possess the stamina to participate in all cheerleading activities and understand every reasonable precaution will be taken to insure her/his safety; however, I do realize that there is an inherent risk of injury. I understand that should my child become a member of the squad, it will be necessary for me to help them abide by all the rules and regulations contained in the Cheerleader Constitution. I understand that I must purchase uniforms, warm-ups, and accessories and that the squad members may participate in fundraisers to help minimize the out-of-pocket costs associated with being a cheerleader. I also understand that my child and I will be responsible for expenses related to cheerleading that she does not earn through these fundraisers. Furthermore, I understand that any violation of standards, lack of cooperation, unexcused absences from performances or practices, or any violation of CCSD rules or the rules outlined in the constitution may result in temporary or permanent dismissal from the cheerleading squad. I am aware that there is a payment due on May 29 th and additional payments due throughout the months of June-August. I understand that my child s 3 rd and 4 th quarter grades will determine his/her eligibility for tryouts. Should my child fail to maintain a minimum 2.0 GPA necessary for eligibility, (s)he will be dismissed from the Green Valley High School Cheerleading Squad. I also understand that if she receives any F s on a 3-week NIAA eligibility grade checks she will be ineligible to participate and may be replaced and/or removed from the squad. I understand the Cheerleading Constitution is a living document, and changes may be made by the coaches and Green Valley High School administration. Signature of Parent/Guardian Date
