SETX OILERS VOLUNTEER POLICY Everyone Is Expected To Volunteer! This organization is solely made up of generous adults who volunteer their time and energy so that each and every child has the best experience possible. The Board does not expect anyone to participate any more or less than we do ourselves. However, as mentioned before, we do volunteer our time and expect that each family involved do its share of the volunteering as well. Each football team will need one Team-Parent. Among other duties, the Team-Parent will be responsible for coordinating their team parent s time spent working the league s home game concession stand and the fundraising booth. Of course, we know your child s football game is important to you, and we will do our best to avoid scheduling you during your child s game time. The Team-Parent will coordinate these efforts with the Association Coordinator. The Team Parent will be responsible for finding parents to work on the sidelines at home games and away games. 2 STUDENT-ATHLETE & PARENT/LEGAL GUARDIAN CONCUSSION STATEMENT According to the Centers for Disease Control and Prevention, a concussion is a type of traumatic brain injury that changes the way the brain normally works. Most concussions occur without loss of consciousness. Athletes who have, at any point in their lives, had a concussion have an increased risk for another concussion. Young children and teens are more likely to get a concussion and take longer to recover than adults. The new concussion law is an opportunity to make playing sports safer for Texas' young athletes. Must be signed and returned to school or community youth athletic activity prior to participation in practice or play. The SETX Oilers strive to keep children and adults safe in all of our programs. Student-Athlete Name: Parent/Legal Guardian Name(s): After reading the information sheet, I am aware of the following information: Student-Athlete initials Parent/Legal Guardian initials A concussion is a brain injury which should be reported to my parents, my coach(es) or a medical professional if one is available. A concussion cannot be seen. Some symptoms might be present right away. Other symptoms can show up hours or days after an injury. I will tell my parents, my coach and/or a medical professional about my injuries and illnesses. I will not return to play in a game or practice if a hit to my head or body causes any concussion-related symptoms. I will/my child will need written permission from a health care provider* to return to play or practice after a concussion. Most concussions take days or weeks to get better. A more serious concussion can last for months or longer. After a bump, blow or jolt to the head or body an athlete should receive immediate medical attention if there are any danger signs such as loss of consciousness, repeated vomiting or a headache that gets worse. After a concussion, the brain needs time to heal. I understand that I am/my child is much more likely to have another concussion or more serious brain injury if return to play or practice occurs before the concussion symptoms go away. Sometimes repeat concussion can cause serious and long-lasting problems and even death. I have read the concussion symptoms on the Concussion Information Sheet. * Health care provider means a Texas licensed medical doctor, osteopathic physician or a clinical neuropsychologist with concussion training. 3 SETX OILERS WAIVERS SETX OILERS Parent/Player Code of Conduct Team Posture A team is a small number of people with complementary skills who are committed to a common purpose, performance goals, and approach for which they hold themselves mutually accountable. All our players/parents are role models. They represent our team, league, school, community, and their families. Only positive attitudes and good behavior are acceptable. This creates the type of leadership required to properly motivate each player in becoming a valuable asset to our program and our community. Everyone is required to abide by the following code of conduct: 1.) Do NOT criticize any player in front of spectators, but reserve constructive criticism for later, in private. 2.) Accept decisions of the game officials and judges on the field during competition as being fair and called to the best of their abilities. 3.) Do NOT criticize an opposing team, its players, coaches, cheerleaders, or fans by word of mouth or gesture. It is important that we teach good sportsmanship. 4.) Emphasize that good athletes strive to be good students. 5.) Strive to make every football activity serve as a training ground for life, and a basis for good mental and physical health. 6.) Emphasize that winning is the result of good Teamwork. 7.) Do NOT shout instructions from the side line, this is the coach s responsibility. 8.) All games and practices are closed. This means only players and designated Oilers representatives are allowed on practice and game fields. Parents and fans are not allowed in such areas. 9.) Any fan/parent who becomes a nuisance and out of control will be asked to leave. 10.) Do NOT use abusive or profane language at any time. 11.) Considering this is a youth activity, no vulgar and/or explicit logos and/or attire, etc. will be allowed. 12.) Sweating Down tactics in order for a player to make the team weight is prohibited. 13.) Do NOT incite unsportsmanlike conduct. 14.) Abstain from the possession and drinking of alcoholic beverages and the possession or use of any illegal substances on any associated facility of SETX Oilers. 15.) Uphold all rules and regulations. Please make all guests aware of this code of conduct, for they are your responsibility. 4 SETX OILERS Policies & Procedures: No refunds will be given with the exception of a player who is unable to pass a physical and provides a written statement from his physician stating that he is unable to play football for the SETX OILERS due to a medical reason. All issued equipment and uniforms other than the game jersey and pants are to be returned to the SETX OILERS when requested at the end of the season or if you terminate your involvement with the Association. If you fail to do so, you will be billed $ Parents are responsible for supplying cleats, mouth pieces and practice pants for football players. Photos taken of your child while during practice or on the game field may be used on the website and or for publicity purposes. All complaints or grievances with anyone associated with the SETX OILERS shall be filed in writing with the Board of Directors in order to be properly addressed. There shall be no smoking, tobacco products, or alcoholic beverages allowed on school property as prohibited by State Law. Participants may be disciplined for unexcused absence from practice, being late for practice, leaving practice early, distractions during practice, or behavioral issues during practice. Disciplinary actions will be at the discretion of the Head Coach. Due to restrictions on weekly practice schedules, a participant who misses over 50% of their scheduled sessions that week will not be allowed to participate in that week s game. Excused absences are: death in the immediate family, (mother, father, sibling, and grandparent), church worship, personal sickness. If you are going to be late for practice or miss practice, prior notice should be given to the Head Coach or it will be automatically be deemed as unexcused. Attending practice is one of the single most important parts of being on a football team. It is for his safety as well as those around him.

5 Dear Player and Parent: Let me take this opportunity to welcome you and thank you for choosing to participate in the Southeast Texas Oilers. Our league was created by a group of individuals who wanted to provide a fun and competitive football and cheer experience for kids who have the desire to play the game. The mission of SETX OILERS is to provide young athletes with opportunities to compete and win at some of the highest levels of youth football and cheer in an environment free from discrimination and prepare these athletes for lifelong contributions to society; to embrace equality, academic achievement and diversity through competition of sport. Our players are expected to maintain passing grades throughout the season, but SETX Oilers does not penalize for non passing grades as stated in our By Laws. We are holding the parent/parents responsible for monitoring the player s grades during the season and imposing their own disciplinary action of which they deem necessary. Our coaching staff will be held to the same standards as our Board Members, and I believe that we are building a strong foundation for your children and that this program is therefore a great asset to our community. We are affiliated with the Texas Youth Football and Cheer Association (TYFA) and abide by their rules and regulations. TYFA has Chapters in Austin, Houston, Killeen, the Rio Grande Valley, San Antonio, West Texas, Port Arthur, Orange, and most recently added, Beaumont. In addition numerous small towns in the surrounding areas of those cities are part of TYFA. There will be traveling involved for away games, but it will not be extensive. It will take a small commitment from everyone to make this organization continue to flourish! The definition of teamwork extends from the field to us as parents, so please consider becoming more involved in this outstanding organization by donating, sponsoring and/or volunteering your time. This season we will be offering an 8x10 program comprised of ads placed by local businesses along with good-luck ads placed by parents, grandparents, etc. The program will include team photos, rosters and schedules (Team photos to be taken in early August).

6 If you own a business and want to place a corporate ad please use the corporate sponsorship form, or if you d like to place a good luck ad, please use the parent sponsorship form. Please note that there is also the option to simply make a donation if you do not desire to place an ad in the program. Please feel free to also pass the sponsorship form(s) along to grandparents, employers or anyone else you may know that may be interested in placing an ad in our program. Please be sure to purchase a program whether or not you choose to run an ad as it is a great way to support the SETX Oilers as well as get to know your coaches & teammates! We are proud to be a part of such a great organization within TYFA and we are looking forward to a great season.
