Dear Michigan Bulldogs Parents & Teammates, We would like to take the time to formally welcome you and your child to The Michigan Bulldogs Youth Athletic Association. We are excited about the opportunity to get to know you and are looking forward to having you as part of the Bulldogs family. The Bulldogs are dedicated to instilling the fundamental values, skills and knowledge your child will use throughout the rest of his or her life. We are proud members of the American Youth Football & Cheerleading Association, which is the fasted growing youth organization in America. Our goal is to (1) enable your child to benefit from the participation in team sports and activities in a safe and structured environment; (2) provide your child with the encouragement and guidance for achievement he or she needs; (3) create an environment that rewards academic achievement; and (4) to teach your child integrity, character and improved decision making skills. As the season progresses, we will keep you informed on any and all relevant information (for example, equipment and uniform distribution dates, practice times and locations, game schedules, transportation car pools, certification dates, registration document deadlines and much more). We will use newsletters, s, parent and team meetings and online website updates to effectively communicate throughout the season. Within this packet, we have included the 2012 Registration/Certification Information Sheet. Please read this packet in its entirety as it provides important information, including fees and deadlines. Once your child has been officially registered (payment in full) and assigned to a Bulldogs team, you will be notified who their coaches are. In the meantime, please feel free to contact one of the board members to address any questions or concerns that you may have. 3 Registration: 2012 Registration/Certification Information Sheet The Michigan Bulldogs registration DEADLINE is June 30 th. All Bulldogs players and cheerleaders are required to complete registration prior to being certified by AYFA, placed on a team, or given equipment/uniforms. Registration involves completion and submission of the entire registration packet and payment of all applicable registration fees. Certification requires submission of the documentation on the checklist below and completion of the league certification process. Certification: ALL PARTICIPANTS IN AMERICAN YOUTH FOOTBALL & CHEER ARE REQUIRED TO PARTICIPATE IN THE MANDATORY PLAYER CERTIFICATION PROCESS. Certification is an AYF requirement which mandates that the parent/guardian provide certain forms and documents in order to complete the process of verification and validation of the participants (age, weight, grades, etc.). The certification process is directed by the AYF Michigan Regional Director and is conducted prior to the start of the season. Certification is completed in person, at a specific time and place. The AYF Regional Board will publish a schedule of dates and places where the certification process must be completed. Any participant that is not certified by a given deadline will not be allowed to participate for the season. Certification Checklist: Completed and signed Michigan Bulldogs Registration Form Completed AYF Participant Contract and Parental Consent Form Completed Waiver & Release of Liability Image Release Form Emergency Medical Treatment, Consent & Information Form Must be completed by the participant s parent or guardian. Contains emergency medical information, emergency contacts and consent to treat in the event of a medical emergency, to be used in the event that a parent/legal guardian is not available. Birth Certificate a copy of the participant s birth certificate MUST be submitted as part of the registration packet. An ORGINAL must be presented at face-to-face league certification. NO HOSPITAL CERTIFACTES WILL BE ACCEPTED. June 2012 Report Cart or Scholastic Fitness Report Must be an ORIGINAL report card indicating at least 2.0 GPA. ALL SEMESTERS MUST BE ON REPORT CARD. Participants with less than a 2.0 GPA will be required to sign a Scholastic Waiver (obtained from the Bulldogs Register). If your child does not receive a report card, the guardian must write a letter, signed by the guardian, to AYF stating that the child is too young to receive a report card. The letter should state the child s name, school, age, and date of birth. Medical Clearance Form Must be completed by a medical professional and dated after January 1 st of the current season. All participants must submit the Medical Clearance Form before participating in any AYF programs, including preseason practice. Note: After the season begins, the Medical Clearance Form will become void in the event of an injury, accident or illness that is attended to by a licensed medical professional. The Resume Participation Medical Clearance Form will be required in order for the participant to resume active participation.

Families of 3 or more children will receive a $25 discount when registered together. Spots are limited and are based on a first come first serve basis.
