3 RAWKIN HAWKS STUDENT PARENT COMMITMENT FORM Rawkin Hawks are expected to be committed to the program and to Birdville High School. They are expected to participate in all scheduled activities. Inability to participate in all activities may affect future involvement in the organization. This form must be signed by the student and parent/guardian and returned during your audition with the completed application packet. Signature of this page is an agreement to follow the BISD Student Code of Conduct. Failure to abide by these rules will result in disciplinary action as set forward by the Code of Conduct. They are expected to participate in all scheduled activities. Inability to participate in all activities may affect future involvement in the organization. We will also encourage our child to be a leader that BHS will be proud to have as a representative of the school. I/We also understand the financial obligations that I/we must fulfill, if my/our child qualifies for the team, will be between $ Such items include spirit wear and other items used by my/our child during the year and I/we are willing to provide these things as they become necessary. I/We understand that the bill must be paid by the first day of school in August. An outstanding balance after the first day of school will cause my/our child to be removed from the squad. A financial breakdown is available on the following page. I/We understand that failure to follow the BISD Extracurricular Honor Code and BISD Grading Procedures will result in suspension or dismissal from the team. 4 PHYSICAL ACKNOWLEDGEMENT/MEDICAL RELEASE CANDIDATE NAME GRADE PHYSICAL INFORMATION All Rawkin Hawks will be required to obtain a physical exam. This physical must be completed no later than May 15, Physical exams will be given at Birdville High School on May 2 nd from 4-7 pm. Tickets are $10 and can be purchased from a BHS athletic trainer. Tickets will be available for sale starting April 2 through April 27 ($10) If you do not get a physical from BHS you will need to get one from an outside clinic or doctor and bring the paperwork to Coach Clifton or Coach Naehritz or a BHS athletic trainer. If a Rawkin Hawk does not have a physical by May 15 they will be removed from the squad. Does your child have a health condition such as asthma, diabetes, seizures or a medical concern such as a severe allergy to peanuts or any other medical concerns the sponsors need to be aware of for the safety of your child? 10 Birdville High School Rawkin Hawks Constitution PURPOSE The purpose of Rawkin Hawks is to promote good sportsmanship, good citizenship, and wholesome and enthusiastic school spirit. Rawkin Hawks are first and foremost representatives of BHS. MEMBERSHIP 1. Rawkin Hawks will be composed of a select group of Juniors and Seniors 2. Rawkin Hawks are expected to set a good example. The following characteristics are required and must be displayed at all times: a. honesty/truthfulness b. good sportsmanship c. high moral standards d. positive school spirit (especially on dress up days and game days) e. positive attitude toward the Rawkin Hawks program. f. respectfulness toward fellow members, coaches, administrators, and other school teams and groups. 3. Rawkin Hawks are considered leaders of the student body. Certain responsibilities accompany any position of leadership. Drug and alcohol use or any immoral behavior is not acceptable for any member of Rawkin Hawks. 4. Rawkin Hawks must participate in and give freely of their time for any activity deemed appropriate by the sponsor throughout the year. This includes all fundraisers, summer practices, camp, games, sign painting, etc. 5. Rawkin Hawks must be in good physical condition can quickly and efficiently move supplies, run flags, etc. 6. Rawkin Hawks are not allowed to have any face or tongue piercings. 7. Rawkin Hawks must show respect at all times to coaches and captains. *** If a Rawkin Hawk becomes unable to meet the membership requirements at any point during the year, he/she may forfeit squad membership. ATTENDANCE 1. Rawkin Hawks are expected to have above-average school attendance. 2. Rawkin Hawks must attend all practices/meetings/events and games. ONE absence will be excused. 3. In addition, Rawkin Hawks may be allowed to attend certain school events without penalty with PRIOR APPROVAL at the sponsor s discretion. 4. Rawkin Hawks must notify the sponsor before third period of any absence from practice/meetings. The following are considered excused absences: personal illness, injury, funeral or death in the family, special school activities with prior notice to the sponsor, and religious holidays. All other reasons for being absent or leaving early from a Rawkin Hawks activity will be unexcused. 5. In the event of an injury, the Rawkin Hawk must either bring a doctors note, or be evaluated by a BHS athletic trainer and attend morning treatment. 6. Rawkin Hawks must be ready at the time set by the sponsor. Rawkin Hawks will be considered late if not ready to perform at the set time. (not being ready includes eating, drinking, being out of uniform, etc.) 7. Rawkin Hawks will ride the bus to and from all away events if a bus is provided. 8. Any Rawkin Hawks who fails to appear in the FULL uniform will not perform and will receive demerits. COST 1. Rawkin Hawks are required to purchase all necessary spirit wear, uniforms, other supplies by the due dates. If payment deadlines are not met, administration will reserve the right to bench the Rawkin Hawks 2. The entire balance must be paid for by the first day of school. If this balance has not been paid for by the first day of school then the Rawkin Hawks will not perform until it has been paid in full. If a Rawkin Hawks is removed, the Rawkin Hawks is still responsible for payment and will be placed on the fee and fine list. Approximate cost per Rawkin Hawks: $ ***NOTE: The above listed amounts do not include miscellaneous expenses such as extra playoff shirts, etc..
11 UNIFORMS 1. Uniforms consist of black short sleeve Dickies coveralls 2. Students will purchase their own black Nike style shorts, vans old skool yellow and white shoes and socks. 3. Rawkin Hawks will purchase three club T-shirts, fleece jacket, raincoat (new members), flag and a megaphone (new members) to be paid in full before receiving gear. 4. Rawkin Hawks must wear designated uniforms at all pep rallies, during school on game days and at all games. 5. Rawkin Hawks will not wear their spirit gear at any other time GRADES 1. Rawkin Hawks are expected to maintain a passing grade of 70 in each class. 2. Rawkin Hawks will follow the No Pass-No Play rule as established by UIL. 3. A Rawkin Hawks who fails two (2) six weeks in any class will be removed from the squad. CAPTAIN/CO-CAPTAINS 1. Rawkin Hawks Captain must be a senior with prior Rawkin Hawks experience 2. Must demonstrate leadership with creative abilities. 3. Must not have been placed on probation (academic or disciplinary) for any reason. 4. Must not have been placed in Friday Night and/or Saturday school detention more than once (for minor infractions, such as tardies.) 5. Must not have a history of Rawkin Hawks discipline problems or more than two N s in conduct during any reporting period of the current school year. 6. Must not have received a U in conduct during any reporting period of the current school year. Duties of the Captain/Co-Captain(s) will include: 1. Communicating between sponsor and members. 2. Directing the Rawkin Hawks squad. 3. Coordinating activities of the group. 4. Being responsible for helping create, and teaching skits, etc. to all members. 5. Packing throwables/visiting team gifts/other supplies for games. 6. Working closely with the sponsor to coordinate activities, pep rallies. 7. Maintaining and promoting a good relationship with all squad members and the sponsor. 8.
