2 GENERAL SAFETY INFORMATION Under Penalty of Law These User Instructions are not to be removed except by the user of this equipment. Current User Instructions must always be available to the user. WARNING 1. Compliant fall protection and emergency rescue systems help prevent serious injury during fall arrest. Users must read and understand the User Instructions provided with the product and be properly trained by their employer prior to use per OSHA 29 CFR and or applicable local standards. Misuse or failure to follow warnings and instructions may result in serious personal injury or death. For proper use, see supervisor, User Instructions, or call Technical Service at Failure to follow all instructions and limitations on the use of the Ladder Climb Vertical Lifeline may result in serious personal injury or death. 3. Minors, pregnant women and anyone with a history of either back or neck problems should not use this equipment. 4. Never use combinations of components or subsystems that may affect or interfere with the safe function of each other. 5. Not all fall protection components are rated for the same user weight capacity. Only use components rated for the same weight capacity. 6. Before using a personal fall arrest system, employees must be trained in accordance with the requirements of OSHA 29 CFR in the safe use of the system and its components. 7. Personal fall arrest systems, including the Ladder Climb Vertical Lifeline, must be inspected prior to each use for wear, damage, and other deterioration and defective components must be immediately removed from service, in accordance with the requirements of OSHA 29 CFR and Do not use or install the Ladder Climb Vertical Lifeline without proper training from a competent person as defined by OSHA 29 CFR (f). 9. The Ladder Climb Vertical Lifeline must only be installed on OSHA compliant fixed ladders. 10. Over tensioning the Ladder Climb Vertical Lifeline will result in damage to the Energy Absorber at the top of the system. 11. The Ladder Climb Vertical Lifeline must only be connected to the front D-ring of a Full Body Harness with the distance between the cable and attachment point not exceeding 9". 12. Do not add additional connectors to extend the distance between the 128 Cable Fall Arrester and the front D-ring. 13. Caution must be taken when using the Ladder Climb Vertical Lifeline near moving machinery, electrical hazards, sharp edges, or abrasive surfaces. Contact with these elements may cause equipment failure, personal injury, or death. 14. Do not expose the Ladder Climb Vertical Lifeline to chemicals or harsh solutions which may have a harmful effect. Contact 3M Technical Service with any questions. 15. The Ladder Climb Vertical Lifeline is designed to be used in temperatures ranging from -40ºF to +130ºF (-40 C to +54 C). 16. When not installed, store the Ladder Climb Vertical Lifeline in a cool, dry, clean environment, out of direct sunlight. 17. Any equipment that has been subjected to the forces of arresting a fall, or that has a deployed 2

3 Load Indicator must be removed from service until a qualified person can determine the need for authorized repair or disposal. 18. If inspection reveals any defect, inadequate maintenance, or unsafe condition, remove from service and destroy. 19. Only 3M, or persons or entities authorized in writing by 3M, shall make repairs or alterations to the equipment. CAUTION 1. A secondary fall arrest system may be required if the user is exposed to a fall hazard while entering or exiting the Ladder Climb Vertical Lifeline system. 2. Wear proper Personal Protective Equipment when performing Inspection, Cleaning and Maintenance procedures. Safety glasses & gloves are recommended. FALL ARREST SYSTEM COMPONENTS System Components A complete fall arrest system consists of the following components: Anchorage, Body Support, and Connecting Devices. Note: For continuous protection, more than one system may be needed. Anchorage An anchorage, as defined by OSHA 29 CFR shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kn) per employee attached, or shall be designed, installed, and used as follows: as part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. Body Support A body support is the component of a personal fall arrest system that is worn on or around the body. Per OSHA 29 CFR effective January 1, 1998, body belts are not acceptable as part of a personal fall arrest system. Note: The use of a body belt in a positioning device system is acceptable. Full body harnesses must be used for all fall arrest systems. Connecting Devices A connecting device is the link between the body support and anchorage. Connecting devices will vary depending on the application. USE INSTRUCTIONS AND LIMITATIONS Important Before use, the user must read and understand these User Instructions. Keep these User Instructions for reference. Purpose The 3M Ladder Climb Vertical Lifeline is a pre-engineered flexible vertical lifeline system, designed as part of a complete personal fall arrest system, to help limit the fall arrest forces in the event of a fall. 3

4 Use Instructions 1. The complete fall protection system must be planned (including all components, calculating fall clearance, and swing fall) before using. 2. Users must have a rescue plan, and the means at hand to implement it, that provides for the prompt rescue of employees in the event of a fall, or assures that employees are able to rescue themselves. Use Limitations 1. The Ladder Climb Vertical Lifeline is designed for a single user with a capacity up to 310 lb (141 kg) including clothing, tools, etc. 2. Only approved 3M components may be used with the Ladder Climb Vertical Lifeline. 3. Only full body harness that meet the ANSI Z standard may be used with the Ladder Climb Vertical Lifeline. 4. For custom applications of the Ladder Climb Vertical Lifeline not addressed in these User Instructions please contact 3M Technical Service. ANCHORAGE REQUIREMENTS Anchorages All anchorages in which the Ladder Climb Vertical Lifeline attaches must meet the requirements of OSHA 29 CFR and ANSI Z OSHA states: Anchorages to which personal fall arrest equipment is attached shall be capable of supporting at least 5,000 pounds (22.2 kn) per employee attached, or shall be designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two, under the supervision of a qualifi ed person. ANSI Z states that anchorages in a personal fall arrest system must have a strength capable of sustaining static loads, applied in all permitted directions by the system, of at least: (a) two times the maximum arrest force permitted on the system when certification exists, or (b) 5,000 pounds (22.2kN) in the absence of certification. The strength in (a) and (b) must be multiplied by the number of personal fall arrest systems attached to the anchorage, when more than one personal fall arrest system is attached to the anchorage. Anchorage Connectors Anchorage connectors are components that couple the personal fall arrest system to the anchorage. The end anchorage connectors are designed to resist and transfer to the end anchorage at least two times the maximum arrest load. CONNECTION REQUIREMENTS Compatibility Limitations All connecting subsystems must only be coupled to compatible connectors. OSHA 29 CFR prohibits snaphooks from being engaged to certain objects unless two requirements are met: it must be a locking type snaphook, and it must be designed for making such a 4

5 connection. Designed for means that the manufacturer of the snaphook specifically designed the snaphook to be used to connect to the equipment in question. The following connections must be avoided, because they can result in rollout* when a nonlocking snaphook is used: Direct connection of a snaphook to horizontal lifeline. Two (or more) snaphooks connected to one D-ring. Two snaphooks connected to each other. A snaphook connected back on its integral lanyard. A snaphook connected to a webbing loop or webbing lanyard. Improper dimensions of the D-ring, rebar, or other connection point in relation to the snaphook dimensions that would allow the snaphook keeper to be depressed by a turning motion of the snaphook. *Rollout: A process by which a snaphook or carabiner unintentionally disengages from another connector or object to which it is coupled. (ANSI Z ) Snaphooks and Carabiners Snaphooks and carabiners used in the Ladder Climb Vertical Lifeline, marked with the ANSI Z or ANSI Z standard, are self-locking with a minimal tensile break strength of 5,000 lbs, and a 3,600 lbs gate rating. Snaphooks and carabiners marked with the ANSI Z and/or CSA Z standards, incorporate self-locking snaphooks and carabiners with minimal tensile break strength of 5,000 lb, a minimum gate rating of 220 lb, and a minimum side load gate rating of 350 lb. SYSTEM COMPONENTS Compatibility Limitations All components and subsystems used with the Ladder Climb Vertical Lifeline have been tested as part of a pre-engineered flexible vertical lifeline system. Only 3M approved components and subsystems are to be used with the Ladder Climb Vertical Lifeline. RC M400-XXX Cable Assembly The RC M400 Cable Assembly has three components; the Cable vertical lifeline, Energy Absorber, and Back-Up Cable. The Cable is ⅜" 7x19 galvanized or 304 stainless steel Aircraft cable. The Energy Absorber is constructed of 10 gage powder coated steel measuring 8½" long and 3¼" wide. It is designed to limit the maximum arrest load to less than 900 lbf on the end anchorage, the Top Bracket, when used with the 128 Cable Fall Arrester at a fall factor of two. The Back-Up Cable is constructed with ¼" 7x19 galvanized Aircraft cable with a 1 16" vinyl coating, and is 13⅜" long. The Energy Absorber and Back-Up Cable are swaged to one end of the Cable (thimble and two swages), and the other end unfinished for system length adjustment with two M400-3 Fist Grips and a M400-8 Thimble. 5

6 M400-2 Bottom Bracket The M400-2 Bottom Bracket of the Ladder Climb Vertical Lifeline is constructed of hot dipped galvanized 2" square tube steel. The 18" long M400-2 Bottom Bracket comes with a ⅜" x 6" eye bolt to connect the unfinished end of the cable. M400-5 Cable Guides The M400-5 Cable Guides are a neoprene rubber guide with galvanized steel brackets and hardware. The M400-5 Cable Guides are to be placed at varying lengths to help reduce harmonics and chatter in the cable. The M400-5 Cable Guides are required at staggered intervals starting at 25'. M400-6 Round Rung Clamps or M400-7 Square Rung Clamps Rung Clamps are 2¾" wide ⅜" galvanized steel bolts with two flat washers and two ⅜" nuts. M400-6 Round Rung Clamps are for round rungs up to 1¼" and M400-7 Square Rung Clamps are for square rungs up to 2" (M400-7). M Extended Top Bracket The M Extended Top Bracket of the Ladder Climb Vertical Lifeline is constructed of hot dipped galvanized 2" square tube steel. The 7' long M400-7 Extended Top Bracket extends 54" above the top rung, allowing users to exit the ladder before disconnecting from the system. M400-1 Top Bracket The M400-1 Top Bracket of the Ladder Climb Vertical Lifeline is constructed of hot dipped galvanized 2" square tube steel. The 40" long M400-1 Top Bracket extends 8" above the top rung. 128 Cable Fall Arrester The 128 Cable Fall Arrester used on the Ladder Climb Vertical Lifeline is self trailing and constructed of stainless steel and brass, with a 0.78" connector for the carabiner. Hardware Carabiners used with the Ladder Climb Vertical Lifeline are marked with the ANSI Z or ANSI Z standard, and are self-locking with a minimal tensile break strength of 5,000 pounds and a 3,600 pound gate rating. INSTALLATION Before Each Use Users of personal fall arrest systems must have a rescue plan in place, if the user cannot rescue themselves, as well as the means to carry out the rescue. The user must read and understand these User Instructions, as well as the User Instructions for every component/subsystem of the personal fall arrest system. The entire Ladder Climb Vertical Lifeline system, and its subsystems, must be inspected prior to each use for wear, damage, and other deterioration. All snaphooks and carabiners must be able 6

7 to self-close and lock. Cable must be inspected for kinks, broken strands, corrosion, abrasion, or other signs of wear and damage. Swaged terminations should be secure with the thimble tight and not visibly damaged. System must be properly tensioned. No load indicators shall be deployed. Damaged and other deteriorated and defective components must be immediately removed from service, in accordance with the requirements of OSHA 29 CFR and Swing Falls To minimize the possibility of a swing fall, work as close to the lifeline as possible. Striking objects horizontally, due to the pendulum effect, may cause serious injury. 8 Step 1. Install M Extended Top Bracket Attach the Extended M Top Bracket to the front of the top three ladder rungs, see Fig. 1B, with U-bolts passing around the ladder rung through the M Extended Top Bracket and secured with washers and nuts. Nuts are to be torqued to evenly 15 ft-lbs. The end of the M Extended Top Bracket with the cable, energy absorber, and Back-Up cable attached must be at the top, and the entire bracket must be square and centered to the ladder. Step 1a. Install M400-1 Top Bracket Attach the M400-1 Top Bracket to the front of the top three ladder rungs. Clamp plates securely to the top two rungs by passing the square U-bolts around the M400-1 Top Bracket and through the clamp plate capturing the ladder rung. Secure with washers and nuts. The third ladder rung is captured with the rung U-bolt passing around the ladder rung through the bottom holes in the M400-1 Top Bracket and secured with washers and nuts. Nuts are to be evenly torqued to 15 ft- lb. The end of the M400-1 Top Bracket where the cable, energy absorber, and Back-Up cable attach must be at the top, and the entire bracket must be square and centered to the ladder. Step 2. Connect RC M400 Cable Assembly (if needed) A new system will have the RC M400 Cable Assembly already attached to the M Extended Top Bracket. To attach the RC M400 Cable Assembly to the top of the M Extended Top Bracket, pass the ½" bolt through the front of the M Extended Top Bracket, through the eye of the energy absorber, through the loop of the back-up cable, and through the back of the M Extended Top Bracket and secured with washer and nut. Allow the cable to hang freely down center of the ladder. To attach the RC M400 Cable Assembly to the top of the M400-1 Top Bracket, use a carabiner to connect the eye of the energy absorber and loop of the back-up cable to the eye bolt on the M400-1 Top Bracket. Allow the cable to hang freely down center of the ladder. Step 3. Install M400-2 Bottom Bracket Attach the M400-2 Bottom Bracket to the front of the two ladder rungs, see Fig. 1A, low enough to connect to the system while standing at grade. Clamp plates securely to the top rung by passing the square U-bolt around the M400-2 Bottom Bracket and through the clamp plate capturing the ladder rung. Secure with washers and nuts. The lower ladder rung is captured with the rung U-bolt passing around the ladder rung through the bottom holes in the M400-2 Bottom Bracket and secured with washers and nuts. Nuts are to be evenly torqued to 15 ft-lb. The eye bolt of the M400-2 Bottom Bracket must be facing upwards, and extend leaving ¼" of threads below the nut to tension the system. The entire bracket must be square and centered to the ladder. Step 4. Attach Cable to the M400-2 Bottom Bracket The unfinished end of the cable is for system length adjustment. Take the M400-8 Thimble and open by twisting the ends apart enough to fit over the eye bolt on the M400-2 Bottom Bracket. Do not attempt to pull the ends of the M400-8 Thimble directly away from each other. Twist back to original position. Turn back cable over the M400-8 Thimble until the desired length is reached and apply first M400-3 Fist Grip 5¼" from the M400-8 Thimble. Use torque wrench to tighten fist grip evenly, alternating from one nut to the other until reaching 45 ft-lb of torque. Apply the second M400-3 Fist Grip as near to the M400-8 Thimble as possible. 8

9 Step 5. Tension the System Tension the system by tightening the nut on the eye bolt of the M400-2 Bottom Bracket. The cable should be taut to eliminate slack. Do not over tension the system. Warning: Over tensioning the Ladder Climb Vertical Lifeline will result in damage to the Energy Absorber at the top of the system. Step 6. Install the 128 Cable Fall Arrester Remove carabiner from 128 Cable Fall Arrester. Support body of 128 Cable Fall Arrester and lift handle to open cams. Holding in the open position, rotate 90 and slide the slot over the M400 cable. Rotate back to vertical with cable laying in grooves of the 128 Cable Fall Arrester. The arrow on the 128 Cable Fall Arrester must be pointed upward. Release the cam handle to close cam against the cable. Insert carabiner into hole at the bottom of the handle to secure the 128 Cable Fall Arrester onto the cable. Step 7. Install the M400-5 Cable Guides The M400-5 Cable Guides, see Fig. 1A are required at staggered intervals starting at 25'. Attach the M400-5 Cable Guide to the ladder rung at 25', and stagger the remaining M400-5 Cable Guides through out the system so that they are not equal distances apart. Snap the cable lifeline into the neoprene rubber slot to secure the cable. Each Ladder Climb Vertical Lifeline System comes with the required number of M400-5 Cable Guides to be installed for each length system. All M400-5 Cable Guides are required to be installed. CONNECTION Number of Users The Ladder Climb Vertical Lifeline is designed for a single user with a capacity up to 310 pounds (141 kg) including clothing, tools, etc. Front D-ring of the Full Body Harness Only full body harness that meet the ANSI Z standard may be used with the Ladder Climb Vertical Lifeline. The carabiner for the 128 Cable Fall Arrester must attach directly to the front D-ring of the full body harness. Warning: Do not add additional connectors to extend the distance between the 128 Cable Fall Arrester and the front D-ring. INSPECTION Frequency All components of the Ladder Climb Vertical Lifeline must be inspected prior to each use, and annually by an OSHA defined competent person other than the user. 9

10 Criteria Warning: If inspection reveals any defect, inadequate maintenance, or unsafe condition, remove from service until a qualified person as defined by OSHA (m) can determine the need for authorized repair or disposal. Warning: Any equipment that has been subjected to the forces of arresting a fall, or if any part of the load indicator warning is showing, must be removed from service. All components and subsystems of the Ladder Climb Vertical Lifeline must be inspected. All markings must be legible and attached to the product. All equipment must be free of corrosion, chemical attack, alteration, excessive heating or wear. Orientation lockout feature of the 128 Cable Fall Arrester must be functioning properly. Cable must be inspected for kinks, broken strands, corrosion, abrasion, or other signs of wear and damage. Swaged terminations should be secure with the thimble tight, and not visibly damaged. All snaphooks and carabiners must be able to self-close and lock. All hardware must be free of cracks, sharp edges, deformation, corrosion, or any evidence of defect. All components of the Full Body Harness and 128 Cable Fall Arrester must be inspected. See user instructions supplied with the product. CLEANING, MAINTENANCE, STORAGE Caution: Wear proper Personal Protective Equipment when performing Inspection, Cleaning and Maintenance procedures. Safety glasses & gloves are recommended. Cleaning The Ladder Climb Vertical Lifeline can be wiped down with a mild detergent and clean water solution, and rinsed with a dampened clean cloth to remove detergent. The hardware can also be wiped down to remove grease, or dirt with a clean dry cloth. Maintenance Any Ladder Climb Vertical Lifeline component requiring maintenance must be tagged unusable and removed from service. Warning: Only 3M, or persons or entities authorized in writing by 3M, shall make repairs or alterations to the equipment. Cleaning maintenance may be performed by the user. 10

11 Storage When not installed, the Ladder Climb Vertical Lifeline should be stored in a cool, dry place out of direct sunlight. Do not store in areas where damage from environmental factors such as heat, light, excessive moisture, oil, chemicals and their vapors, or other degrading elements may be present. Do not store damaged equipment or equipment in need of maintenance in the same area as product approved for use. Equipment that has been stored for an extended period must be inspected as described in these User Instructions prior to use.
