1 Activity: Crate Climbing RISK ASSESSMENT FORM: - CRATE CLIMBING No Hazard Inherent 1 Activity Leaders Supervisor is 18 years old or over. have insufficient of participant. skills to run the No persons under the age of 14 to activity. be involved with the running of this activity. Activity Leaders are familiar with the Assessment and Controls which are to be carried out A list of approved: - Supervisors - helpers aged under 18 years old, held within the Competency Log. 2 Leader Incompetence 3 Incident Management of participant. of participants, Sessions are only run by Approved Supervisors who are present at the activity at all times. The Supervisor is in charge of the activity at all times. Belayer / Supervisor receives training from qualified instructors (SPA or higher). Belayer / supervisor undergoes assessment of skills by qualified assessor prior to running the activity. Ongoing skills review (at least every 12 months) by qualified assessor. Belayer is not under the age of 16. Sufficient breaks to be taken by staff running this activity Activity to be stopped whilst incident is investigated Session Log to be completed City of Coventry Scout County: 1
