3 CONFINED SPACE 3 Levels of Confined Space Rescue Capabilities Non-Entry: Entry-Basic: Entry-Operations: Entry-Technician: Rescue performed by confined space attendant. This training is usually provided during entrant/attendant training, but may be covered at a later date. Should be practiced before each entry. Rescuers size up and enter a confined space with all hazards controlled in response to a medical emergency. Low angle rope systems, if any, are used. Rescuers size up and enter a non IDLH space that may present other hazards. High-angle rope systems are used to move rescuers and the patient within the space and to a place of safety. Rescuers size up and enter a potentially IDLH space under supplied air and wearing PPE. High-angle rope systems are used to move rescuers and the patient. Entry-Basic Entry-Operations Entry-Technician Duration: 2-4 hours depending on class size, experience, and the nature of the spaces. Prerequisites: Entrant/attendant training. Basic Rescue Process Low-angle Operations Pre-plans Size-up Entry and Patient Packaging Debrief and Continuous Improvement Duration: 3 days depending on Prerequisites: Entrant/attendant and fall protection training. Duration: 5 days depending on Prerequisites: All entry operations pre-requisites in addition to respiratory Entry Rescue Basic Process protection (supplied air) and hazmat training as applicable. Safety and Response to Stress Entry Operations Curricula Incident Command Systems Advanced Rigging Size-up Respiratory Protection Mainlines and Belays HAZMAT and Rope Systems Equipment and Knots Confined Space Operations Pre-plans and Debriefs Team Leader Definitions: High Angle: Ropes support the patient/ rescuer. Low Angle: Patient is supported by the ground or is carried. Duration: 2-5 days depending on students experience. Prerequisites: All entry operations or technician pre-requisites. See Advanced Training: Confined Space Rescue Team Leader on page 7.

4 SURFACE MINES 4 The purpose of this five-day* course is to meet the requirements for a surface mine search and rescue team. Teams capable of providing rescue services at these locations must be able to access and remove patients from a variety of hazardous settings including confined spaces, elevated areas, haul trucks, and dumps. This program develops the technical and safety skills required by surface mine rescue teams. This includes initiating an emergency response, establishing a command structure, sizing-up hazards, ensuring the team safety, and accessing/transporting the patient. Teams should undertake some level of refresher training at least quarterly. Formal refresher training should be provided annually. * The scope, length, and content of this training may vary based upon site-specific conditions, including the student s current skill levels. Students attending this class must have previously completed confined space and fall protection safety training. 5 HIGH ANGLE 5 Types of High Angle Rescue Operations Towers: High Voltage Towers: Structural Steel: These types of structures include lattice and monopole cell and telecommunication towers, radio antennas, wind turbines, and water towers. Rescue techniques must often be accomplished with small (2-3 person teams) with limited or no outside support. Annual retraining is required. Best Practice: Quarterly drills with annual refresher. Tower rescue techniques are adapted to reflect the hazards of operating near energized conductors. Rescue techniques reflect the equipment available to linemen such as truck-mounted capstans and material handling systems. Annual retraining is required. Best Practice: Quarterly drills with annual refresher. Rescue techniques are adapted to the structural steel environment. Examples include bridges, conveyors, cranes, trusses, and stage rigging. Annual retraining is required. Best Practice: Quarterly drills with annual refresher. Towers High Voltage Towers Structural Steel Duration: 2-3 days depending on class size, experience, and the types of towers being accessed. Prerequisites: Fall protection user training. Fall Protection Review Tower Types and Construction Mainlines and Belays Tower Assisted Rescue Techniques Equipment Selection, Inspection, and Use Pre-plans and Emergency Planing Duration: 2-5 days depending on class size, experience, and the types of transmission towers being accessed. Prerequisites: Fall protection user training, outside lineman electrical apprenticeship training. Fall Protection Review Tower Types and Construction Rope Systems and Electrical Safety Transmission Tower Rescue Techniques Patient Care Duration: 2-3 days depending on class size, experience, and the types of structures being accessed. Prerequisites: Fall protection user training. 6 TRENCH/EXCAVATION 6 Levels of Trench Rescue Capabilities Awareness: Operations: Technician: Team understands basic safety requirements for trench and excavation safety. Team members can identify the hazards and list each step of the rescue process. Team members can act in a supporting role in the incident command system. No re-training requirement. Team is trained to size-up and control hazards associated with emergencies in non-intersecting trenches up to eight to ten feet deep. Team can use both engineered protective systems and traditional shoring. Team can create safe zones, access the patient and lift the patient to a place of safety. Should be re-trained every year with quarterly drills. Team is trained to shore intersecting, L, and T trenches greater than eight to ten feet deep. Team can enter excavations with hazardous atmospheres or substantial water intrusion. Should be re-trained every year with quarterly drills. Awareness Operations Technician Duration: 2-4 hours depending on Prerequisites: Excavation Safety Competent Person training.* Basic Rescue Process Rescue Hazards Incident Command System Hot, Warm, and Cold Zones Logistics and Support Duration: 3 days depending on Prerequisites: Excavation Safety Competent Person training. Awareness Curricula Hazards and Size-up Ventilation and Monitoring Pneumatic, Hydraulic, Mechanical Shores. Cribbing, Air Bags and Lifting Duration: 5 days depending on Prerequisites: Excavation Safety Competent Person training. Operations Curricula Waler Systems X, T, and L Trenches Respiratory Protection Use of Powered Equipment Patient Care and Transport * Competent persons can identify hazards and take corrective action. They have been trained to classify soil, specify appropriate construction/installation of protective systems, manage hazardous atmospheres and water instruction, ensure utilities are located, and conduct routine inspections of the excavation.

7 ADVANCED TRAINING Confined Space Rescue: Team Leader Training Confined Space Rescue Team Leader: Schedule 7 This 5-day program provides qualified rescue team leaders with essential tools and skills for developing and maintaining team efficiency and safety. Students review high angle rope skills and then rotate through a series of skill demonstration stations. Each station tests the students abilities to communicate this information effectively, answer questions, ensure safety, and manage a class. The focus of the class is usually on confined space operations but the principles apply to almost any industrial rescue scenario. Elevated Anchor Systems February 29-March 4, 2016 Tempe, AZ November 14-18, 2016 Kennewick, AZ To register: Call us or go online. Establishing an elevated anchor increases the effectiveness and safety of most rope systems. This three-day course focuses on the physical principles of all types of elevated anchors (monopods, bipods, tripods) including the Arizona Vortex and the TerrAdaptor multi-pod. Field exercises ensure that students can rig and deploy these systems as designed. Prerequisites: Students must have basic rope skills including knot-tying, constructing anchors, and operating mainlines and belay lines. Rigging Physics This two-day course examines the physical principles that govern rope rigging. Given a variety of rope rigging systems, simple calculations are performed to estimate the effects of friction, torque, tension, compression, shock loads, and mechanical advantage. These effects are then measured. The first day consists of lecture while day two is conducted at a testing lab in Phoenix, Arizona. This class is designed for rescue and rope access instructors and practitioners. Prerequisites: Students must have intermediate rope and math skills. For a Quote : Rescue for Rope Access Rope access is a form of vertical work positioning that employs rope systems to descend/ascend to the work location. This technique has been evolving for several decades in Europe, Australia, and Canada. It has now gained a large following in the United States. In the near future rope access will be specifically addressed by OSHA. Unfortunately, much of the current training does not adequately address pre-planned, and engineered rescue contingencies for this industrial activity. D2000 Safety and Rescue is a leader in providing proven, and sound training in rigging work positioning rope systems that guarantee immediate extrication of sick or injured workers that are suspended in a vertical environment.

8 STANDBY SERVICES 8 When undertaking special projects or non-routine confined space entries, organizations often hire professional emergency responders to supervise operations or augment their existing team. The level of preparedness your project needs is a function of the environment and the associated hazards. By analyzing your situation, we can develop a tailored solution to provide both safety oversight and a rescue capability, if needed. This may consist of one rescue technician working with your employees or a full team, trained and equipped for almost any contingency. SAFETY TRAINING* Confined Space: Classes offered include entrant/attendant, entry supervisor and train the trainer. The training is tailored to reflect site-specific hazards and hazard control measures in effect. Fall Protection: We provide authorized user, competent person, and formal equipment inspection classes. We also offer a train the trainer class designed to ensure the competent persons can train users. Excavation: Students who complete our Excavation Safety Competent Person training are able to fulfill the OSHA requirements governing excavations. This includes conducting inspections, classifying soils, and installing protective systems. Forklift: Supported Scaffolds: Other Services: We can train your employees to operate forklifts or we can train your experienced operators to provide training and re-training at your facility. Our competent person training focuses on supported (as opposed to suspended) scaffolds.
