1 Challenge Module Score Form Note that three trials were not always done even when a child did not score a 4 the first or second time. If assessor decided not to test a second or third time though, this is explained in the notes column. This ONE step forward by assessor is allowed and should be done when the ball is just short as far as dsitance. Assessor could improve accuracy of her throws : Youth instructed to throw ball back harder so just one bounce. Ball generally thrown well by assessor. Make sure to get to left and right side each time! Do not start again though if your throws are not quite perfect throws. :All assessor s throws were really good this time. Do not round up or down Trial 1: 5 steps backwards before stepping out of path twice : Just manages to hold her balance on the backward portion great concentration : Very difficult skill for her. Would have been a good idea for assessor to ask child to stay in position while she marked the distance Trial 1: Looks very natural and easy, just not really fast : : Manages to speed up, but steps on right line at about the 7 m mark so still scores a 2 even though fast enough for a 4. Always watch carefully for this! Trial 1: 2 step overs, 3 jumps : 2 step overs, 8 jumps : All jumps with double bounce 17. Keep timer going util her L foot touches the ground (swinging it in the air still counts as one leg stand) : Touches toes down to floor may be difficult to see : Touches toes down to floor Trial 1: Arms for stability so even though she looks very stable have to score down to a 3. Decided not to do extra trials as expected that child would have to use arms. Third trial not filmed Trial 1: : : Trial 1: Less than 5 hops IN TOTAL (for this item do not have to be consecutive to count) : Touches line after 5m mark but one touch is allowed for a 2 : Great Verbal cuing by assessor allowed. Difficult to see taped lines. The camera is also not at the best angle for viewing line touches. The PT is an a perfect position to see. Has been scored as if she did not step on line in any trial.
