2 Expectations at Sport Uniform: Wear sports uniform to and from school on your sport day White socks must be worn (no ankle socks) Sports shoes need to be appropriate for sport (see uniform policy) No skate shoes, vans, slip-ons or slipper type shoes. Hats must be worn for all outdoor sports / activities. Keep your school hat in your bag at all times.

3 Punctuality & Equipment Travelling sports need to meet outside the Admin building straight after period 6. Non Travelling sports need to meet at your playing area at 1:15 to prepare for your sport. What to bring: WATER BOTTLE Sport specific shoes Shin guards (soccer) No ipads, phones, ipods, electronic equipment to be used at any time during sport.

8 New Sport - Ultimate Disc Sport term for Frisbee Played on a field similar to Gridiron or American Football 2 end zones, aim is to try and move the disc to your end zone Disc cannot hit the ground or will be a change over Score a point by catching the disc in your attacking endzone

9 Step 1: Friday evening February 1 st log onto our school Facebook page or our school website. The ticketing will open at 7pm. Note students can complete this by themselves Parents can complete but must use student name in application.

10 Step 2: Find the link for your year level. Click and it will open up a new website called Eventbrite. This is a ticketing website for our sport selection

11 Step 3: Click on the REGISTER button to take you to the sport selection page.

12 Step 4: Find the sport/activity you would like to participate in Reminder that you can only get ONE ticket in ONE sport

13 Step 5: Compete the registration. Make sure the student s name is placed in this section. A student or parents can be used to send the ticket. (students preferred)

14 Step 6: Completing the medical form. Download the latest version of Adobe Reader This will allow you to use the medical form efficiently. Link to Free Adobe Reader Software Download - t/pdf-reader.html

15 Step 6: Go back to our school website / Facebook page (as shown in a previous slide). Click on the link for the sports medical form and a PDF document will open (image below). Select the corresponding sport via the drop down box as shown.

16 Step 6: Fill in the remaining information on the form. Parents to check the form. Complete digital signature. Save the medical form to your device with the file name format of Lastname_FirstName_Yearlevel_ Chosensport to This is so we can move into the correct folders on our computer.

17 Frequently Asked Questions Can we do a paper copy? Yes but we would like to reduce the use of paper for our environment How do I get the signature to work? need the most up to date Adobe software. Alternatively can possibly use a stylus to sign. Can I change sports? No, unless there is a major injury preventing participation. I missed out on option 1 what do I do? Have an option 2,3,4 if need be and choose that. Can my parents order my sport because I will be at a friend place? Yes I forgot to do the sign on, can I go in? No, choose what is left.
