5 ***Please sign the bottom of this form as well as the backside of this sheet and return with your registration*** MEDAL LEAGUE VOLLEYBALL SPORTSMANSHIP RULES 1. FOLLOW THE RULES OF THE GAME Part of good sportsmanship is to know the rules of the game and play by them. If a player decides to play a sport, it is the responsibility of that player to learn not only how to play by how to play according to the rules that have been established. 2. TRY TO AVOID ARGUMENTS Part of good sportsmanship is anger management. Arguing with officials, opponents, or even teammates is often a misguided effort at letting off steam in the heat of competition. A good sport knows that anger can get in the way of a good performance. A good sport know how to walk away from an argument and to stay focused on the game at hand. 3. SHARE IN THE RESPONSIBILITIES OF THE TEAM Good sportsmanship implies that the player on a team is a team player. In other words, the player understands that his or her behavior reflects on the team in general. A team player does not overlook unsportsmanlike conduct from teammates and reminds players that they all share in the responsibility of promoting good sportsmanship. 4. GIVE EVERYONE A CHANCE TO PLAY ACCORDING TO THE RULES Talented players that are good sports will look out for and encourage the less talented players on the team. They will cooperate with them so everybody gets a chance to play. Sportsmanship Tip: No put downs, be nice to everyone. 5. ALWAYS PLAY FAIR Honesty is an important part of sports. A player with good sportsmanship does not want a hollow victory which comes from the result of cheating. Sportsmanship Tip: If you make a mistake, don t pout or make excuses. Learn from it and be ready to continue to play. 6. FOLLOW DIRECTIONS A player with good sportsmanship listens and follows directions. Everyone should be quiet when the rules are given so everyone can learn them. If a player has disagreements with the teacher or coach, the player should discuss the disagreements privately in a civil manner. 7. RESPECT THE OTHER TEAM S EFFORT Whether the other team plays better or worse, the player with good sportsmanship does not use the occasion to put the other team down. Respect for the other team is central to good sportsmanship. If an opponent out-performs a player that accepts it, learns from it, offers no excuses and moves on. If a player out-performs an opponent, that player enjoys the victory, but does not gloat, belittle, or minimize the opponent. Sportsmanship Tip: If you win, don t rub it in. If you lose, don t make excuses. 8. OFFER ENCOURAGEMENT TO TEAMMATES A sign of good sportsmanship is a player who praises teammates when they do well and comforts and encourages them when they make a mistake. Criticizing teammates in the heat of battle simply distracts from the focus of working together and gives the advantage to the opponent who develops a sense of confidence when seeing signs of weakness or a lack of unity in the midst of competition. Sportsmanship Tip: If a teammate makes a mistake, offer encouragement, not criticism. 9. ACCEPT THE JUDGEMENT CALLS OF THE GAME OFFICIALS Making mistakes is human nature. Arguing with an official over a judgement call simply wastes energy. The player with good sportsmanship knows that errors may be made. This player may be upset, but that player also has learned to focus her energies back on the game and doing the set she can do for the rest of the game. 10. END THE GAME SMOOTHLY When the game is over, pouting, threatening, or trash talking have no place in the life of the players with good sportsmanship, who emphasize the joy of participating, regardless of the outcome. They re not empty of emotions, but they know that their efforts to end the competition smoothly, without bad emotional display, will help ensure that the games will continue in the future. Sportsmanship is the ability to: Win without bragging, lose without complaining and treat your opponent with respect. Player s NAME (printed) Grade: Parent Signature: Player Signature: Date:
6 Concussion Information and Waiver A concussion is a type of traumatic brain injury that changes the way the brain normally works. A concussion is caused by a bump, blow or jolt to the head or body that causes the head and brain to move rapidly back and forth. Even a ding, getting your bell rung or what seems to be a mild bump or blow to the head can be serious. WHAT ARE THE SIGNS AND SYMPTOMS OF A CONCUSSION? Signs and symptoms of a concussion can show up right after the injury or may appear days or weeks after the injury. If an athlete reports one or more symptoms of concussions listed below after a bump, blow or jolt to the head or body, they should be kept out of play the day of the injury and until a health care professional, experienced in evaluating concussions, says they are symptom-free and they are OK to return to play. Signs Observed By Coaching Staff: Appears dazed or stunned Is confused about position or assignment Forgets an instruction Is unsure of game or opponent Moves clumsily Answers questions slowly Loses consciousness (even briefly) Shows mood, behavior or personality changes Cannot recall events prior to hit or fall Syptoms Reported By Athletes: Headaches or pressure in the head Nausea or vomiting Balance problems or dizziness Double or blurry vision Sensitivity to noise Feeling sluggish, hazy, foggy, or groggy Concentration or memory problems Confusion Just not feeling right or feeling down CONCUSSION DANGER SIGNS In rare cases, a dangerous blood clot may form on the brain in a person with a concussion and crowd the brain against the skull. An athlete should receive immediate medical attention if after a bump, blow or jolt to the head or body they exhibit any of the following danger signs: One pupil larger than the other A headache that gets worse or slurred speech Repeated vomiting or nausea Cannot recognize people or places Loses consciousness for any amount of time Is drowsy or cannot be awakened Weakness, numbness, or decreased coordination Convulsions, seizures, or unusual behavior Becomes increasingly confused or agitated WHY SHOULD AN ATHLETE REPORT THEIR SYMPTOMS? If an athlete has a concussion, his/her brain needs time to heal. While an athlete s brain is healing, they are much more likely to have another concussion. Repeat concussions can increase the time it takes to recover. In rare cases, repeat concussions in young athletes can result in brain swelling or permanent damage to their brain. It can even be fatal. WHAT SHOULD YOU DO IF YOU THINK YOUR ATHLETE HAS A CONCUSSION? If you suspect that an athlete has a concussion, remove them from play and seek medical attention. Do not try to judge the severity of the injury yourself. Keep the athlete out of play until a medical professional says they are symptom free and are OK to return to play. Rest is the key to help an athlete recover. Exercising or activities that involve a lot of concentration (studying, computers, video games) may cause concussion symptoms worsen. Recovery is a gradual process that should be carefully monitored by a health care professional. WHY SHOULD PARENTS AND ATHLETES BE AWARE OF THIS INFORMATION? On April 6, 2012, the State of Wisconsin joined 31 other states by enacting formal legislation dedicated to the safety of youth participating in sports. Wisconsin Act 172 relates to concussions and head injuries sustained in youth activities. The law requires all youth athletic organizations to educate coaches, athletes and parents on the risks of concussions and head injuries and prohibits participation in a youth activity until the athlete and parent or guardian has returned a signed agreement sheet indicating they have reviewed the concussion and head injury informational materials. The law requires immediate removal of an individual from a youth athletic activity if symptoms indicate a possible concussion has been sustained. A person who has been removed from a youth activity because of a determined or suspected concussion or head injury, may not participate again until he or she is evaluated by a health care provider and receives written clearance from the health care provider to return to the activity.
