If you have no bespoke seafood policy, please refer to your suppliers sourcing policy. 1.1 General 1.11 Does your company have a specific seafood policy? If no, go straight to section 2) 1.12 Does your company s seafood policy ensure traceability and identification of all seafood products from point of capture or farm origin to sale? 1.13 Does your company have a specific set of guidelines for sourcing farmed and/ or wild caught seafood within your seafood policy or as a standalone policy? 1.14 Is your seafood policy publically available? (If yes, please provide details on how and where this is available) 1.15 Is your supply chain independently audited against your seafood policy? 1.16 Do you have a specific member of staff employed to deal partly or wholly with seafood sustainability? 1.17 Does your seafood policy apply to all seafood sold in store (both branded and own brand)?

6 2 Seafood sales This applies to all own brand seafood and seafood containing products sold in store with the exception of pet food, health products and fish derivatives (i.e. fish sauce, prawn crackers.) To reiterate, the survey is applicable to chilled, frozen, value added and ambient (including tinned) fish as an ingredient (e.g. salads, sandwiches, ready meals and fish pies.) 2.1 Please fill in the attached pro formas with the details of the farms and fisheries that you source from 2.2 What percentage of the fisheries and farms you have listed in the pro forma (by number) are ranked by MCS as Fish to Eat or green rated (1 & 2 rating What percentage of the fisheries and farms you have listed in the pro forma (by number) are ranked by MCS as Fish to Avoid or red rated (5 rating What percentage of the fisheries and farms you have listed in the pro forma (by number) carry a consumer facing eco-label (e.g. MSC or Soil Association)? NB: Eco-label must be shown on pack. 2.5 If you source from any fisheries rated as a Fish to Avoid by MCS, are you engaged in an FIP/AIP within this fishery?
