3 HOW TO ENTER - ADDENDUM A Starting Monday, June 5, 2018, WIXX will take the 11 th caller at after golf-sound effects "cue-to-call" plays weekdays at approx. 8:40am, approx. 11:40am and approx. 4:40pm. This person will instantly win a pair of single-day entrance passes for the LPGA Classic at Thornberry Creek at Onedia. (Passes are good only for ONE of the following days: Mon. 7/2/18 thru Sun. 7/8/18) Each winner will be qualified for a random drawing to be picked as the one contestant who will attempt the Grand Prize (1) putt from 50' on a pre-determined hole/green at Thornberry Creek at Oneida on Sat. 7/7/2018. te: You can only win tickets / qualify for the Grand Prize Putt for $10k. -3- A-1 Official Rules NE Wi

4 ADDENDUM B METHOD OF SELECTION OF WINNER #1: By being the 11th caller after the "cue-to-call" weekdays at approx. 8:40am, approx. 11:40am and approx. 4:40pm the contestant will win a pair of single-day event passes. (See Addendum A) For Grand Prize: They will then be assigned a numbered ping-pong-ball. Each ping-pong ball will be uniquely numbered and dropped onto a "target" from the 2nd - story balcony inside the Midwest Communications Atrium at approx. 11am on Mon. 7/2/17. Whichever ball lands closest to the bullseye on the "target" will be deemed the winner. In the event of a tie, each ball that is tied (measured in distance from the "bullseye" on the target) will be re-dropped from the balcony and will continue until one ball is deemed the winner. A video of this event will be posted to the WIXX Facebook page for public viewing. #2: The Grand Prize Finalist will be eligible to take a 50' putt on Saturday, July 7 th, Prior to the event the finalist will meet with Midwest Communications personal to go over final details for the event and answer any questions. If for any reason the Grand Finalist cannot participate in the $10,000 putt contest the activity cannot be transferred to another individual. In this situation they will be disqualified/voided from grand prize putt with no furthur prize consideration. At that time it will be to the discretion of Midwest Communications, Inc to choose another qualifier. #3: Grand Prize Finalist must follow all course rules and regulations per LPGA and Thornberry Creek. ADDITIONAL INFMATION: The contestant deemed the "Grand Prize Finalist" will then need to abide by the following Terms & Conditions as put forth by SCA Promotions, which include but are not limited to: -The following classes of people are ineligible to participate: a. Current or former touring or club professionals b. Current collegiate golf players at any level (freshman, junior varsity and varsity for men or women) c. Current or former poressionals or current or former members of the Professional Putter's Association -Contestant must agree to complete a "Conteatnt to Agreement to Participate" as put forth by SCA Promotions. -Contesant will not be permitted any practice, warm-up or qualifying putts. -The distance of the putt must not be less than: seventy feet (50') - blocking or grooving of the target path will be permitted. -Any attempted putt where the ball leaves the playing surface will be counted as an unsuccessful attempt. -If, after being hit by the contestant, the golf ball lands in and remains in the designated hole without coming into contact with any surface, person, or obstacle other than the green, pin, or whole and the putt would otherwise be counted as a successful putt under PGA rules and there is compliance with the terms and conditions of SCA Promotions contract #, the winner will receive a check valued at $10,000 from Midwest Communications/WIXX. te: due to the nature of our programs that are heard on-line over the Internet, contests heard on the radio station s Internet audio stream are slightly delayed from the time the contests heard on the broadcast station and may be running behind the broadcast signal. Thus, all contests heard are delayed. This delay can last for 30 seconds up to several minutes. This means that when the station announces winning names in this contest, listeners to the on-line audio stream may be at a disadvantage in participating over those listeners who hear the contest on conventional radio.
