RELEASE Whereas, the undersigned desired to release certain parties from any damages that may arise, directly or indirectly, as the result of his participating in activities offer by LONGMEADOW GAME RESORT, fully realizing the inherent danger in any type of sporting event of this nature. NOW, THEREFORE, in consideration of being allowed to participate in said activities, the undersigned does hereby release and forever discharge LONGMEADOW GAME RESORT, its staff, members, guides, and all land owners upon whose land they may be hunting, from any and all manner of actions, causes of actions, claims and demands whatsoever in law or in equity, which may arise now or in the future as a result of, or pertaining to, the undersigned's participation in said hunt or any of its activities. INWITNESS WHEREOF, I have hereunto affixed my hand this date and year written below: I have received (read and understand) the official rules and regulations of LONGMEADOW GAME RESORT and agree to abide by the provisions therein. 2 Hunting Rules and Regulations 1. All hunters will be required to sign a Waiver of Liability prior to hunts. 2. Ear & Eye protection is MANDATORY for all shooters and spectators. 3. No alcohol prior to or during hunts. 4. No illegal drugs at any time. 5. Firearms are to be unloaded with actions opened until hunters arrive at assigned hunting location. Firearms are to be unloaded and actions opened when hunters leave the hunting area. Shot size is limited to #6 or smaller 12 and 16 gauge shells limited to 2¾ inches. Smaller gauges shells restricted to no more than 3 inch. 6. No shooting at or within 500 feet of any building. No shooting within 50 feet of any public roadway. 7. Treat dogs with the same respect as you would treat a person. Any injury to a dog due to abuse or negligent use of a fire arm will be subject to termination of hunt and/or membership and all costs will be covered by offender. Replacement of a Longmeadow Hunting Dog=$5, No trespassing on neighboring property or leaving your hunting area to hunt elsewhere on the preserve without permission.

3 9. No shooting of birds on the ground. 10. Sportsmanlike conduct and proper safety precautions required at all times. Longmeadow reserves the right to remove anyone acting in a manner deemed inappropriate. 11. During non-guided hunts dogs are to be under owners/handlers control at all times. 12. Shooting of non-preserve wildlife is prohibited. 13. The cost of any property damage due to abuse or neglect will be the responsibility of said offender. Sporting Clays & Trap Rules and Regulations 1. No shot size larger than 7-1/2 and no more than 3-1/4 dram equivalent powder load. No magnum loads. 2. All guns must remain unloaded and open when not in the shooting stand and when walking between stations. 3. Ear & Eye protection is MANDATORY for all shooters and spectators. 4. Once in shooting stand, guns must be pointed safely down range. 5. Everyone must remain on pathway and in designated shooting stands at all times. 6. Shoot only from designated shooting stands. 7. Load no more than 2 shells in your gun at one time 8. Each SQUAD is allowed ONE free show pair per station.

4 9. PLEASE CLEAN UP your empty hulls and all other trash and discard in cans provided before proceeding to next station. 10. The shooting of live birds on the sporting clays course is strictly prohibited. NON-COMPLIANCE WITH ANY OF THE RULES ABOVE WILL RESULT IN THE IMMEDIATE TERMINATION OF THE SHOOT AND THE FULL AMOUNT OF THE SHOOT CHARGED! Name (Print): Signature:.

5 Photograph and Publicity Release Form I,, give Longmeadow Game Resort And Event Center permission to use my name, likeness, image, voice, video and/or appearance as such may be embodied in any pictures, photos, video recordings, audiotapes, digital images, and the like, taken or made on behalf of Longmeadow. I agree that Longmeadow have complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with the Longmeadow's permissions. These uses include, but are not limited to social media platforms such as, Facebook, Twitter, YouTube, Linked In, etc.; illustrations; bulletins; exhibitions; videos; reprints; reproductions; publications; advertisements; and any promotional or educational materials in any medium now known or later developed, including the Internet. I acknowledge that I will not receive any compensation, etc., for the use of such pictures, etc., and hereby release Longmeadow and its agents and assigns from any and all claims which arise out of or are in any way connected with such use.
