I further certify that I am experienced and skilled in the proper handling, loading, use, and discharge of all firearms and ammunition that I intend to use at this range. I agree to absolutely and unequiocally release from any liability and hold harmless 3B's shooting range and training center and any and all of its employees, instructors, agents, representaties, officers, and members with respect to my use of the facilities of 3B's shooting range and training center. 2 H26 3B'S SHOOTING RANGE AND TRAINING CENTER WAIVER FORM THIS FORM MUST BE READ AND SIGNED BY EACH PERSON WHO ENTERS THE RANGE BEFORE USE OF THE PREMISES AND FACILITIES * READ CAREFULLY BEFORE SIGNING* The UNDERSIGNED has read and understands the posted safety rules, and knows that anticipated and unanticipated dangers associated with the use of training, firearms and archery equipment on ranges present a risk of DEATH, PERSONAL INJURY, AND/OR PROPERTY DAMAGE. In CONSIDERATION for being permitted entry to and/or use of the premises and facilities of 3B'S SHOOTING RANGE AND TRAINING CENTER and on behalf of himself/herself, his/her family, estate, heirs and assigns, the UNDERSIGNED hereby assumes all risk of DEATH, PERSONAL INJURY and/or PROPERTY DAMAGE and foreer releases, discharges, and agrees to hold harmless 3B'S SHOOTING RANGE AND TRAINING CENTER, its EMPLOYEES, INSTRUCTORS, AGENTS, REPRESENTATIVES, OFFICERS, and MEMBERS from all claims, demands, causes of action, or liability of any kind, including attorneys' fees, for DEATH, PERSONAL INJURY, and/or PROPERTY DAMAGE occurring during the UNDERSIGNED'S presence on or use of the premises and facilities of 3B'S SHOOTING RANGE AND TRAINING CENTER. The UNDERSIGNED agrees to be responsible for any and all damages (including but not limited to the costs of repairs) occurring during the use of, and/or otherwise on, the premises of 3B'S SHOOTING RANGE AND TRAINING CENTER by the UNDERSIGNED and/or any of the UNDERSIGNED'S GUESTS. In addition, CRIMINAL and /or CIVIL CHARGES WILL BE BROUGHT FOR WILLFUL ACTS ENDANGERING LIFE OR PROPERTY. CLOSED CIRCUIT VIDEO SURVEILLANCE IS USED AT THIS FACILITY.

3 WAIVER OF LIABILITY, RANGE RULES, REGULATIONS FOR 3 B'S SHOOTING RANGE AND TRAINING CENTER EVERYO NE who wishes to use this facility, whether obsering or shooting their own firearm, bows, crossbow, air soft firearm, or shooting a rental firearm or bow or air soft firearm MUST COMPLETELY fill out this form. All information supplied will be kept strictly confidential, will not be distributed or made aailable to others for any purpose, and is for the sole use of 3B'S Shooting Range and Training Center. I hereby make application for use, on this or future dates, of 3B'S indoor shooting range and training center facility (hereinafter "range") which use includes, but is not limited to, the use and discharge of firearms, bows (crossbows), air soft guns at the range, the use and discharge of rental firearms, bows and air soft guns proided by the range, and the serices and training by employees of the range. I also understand that if l am in any way unfamiliar with the proper handling, loading, use, and/or discharge of firearms, bows, or ammunition that I intend to use at the range, the range will proide me, at my expense, instruction in the proper handling, loading, use, and discharge of such firearms, bows and ammunition. In the eent that I cause an injury (or death) to myself or another person, or to the property at the range, I agree to indemnify and hold harmless 3B's indoor shooting range and training center, together with its agents, representaties, instructors, members, and employees, from any and all claims for injuries and/or damages brought by the injured person as a result of my acts or omissions, whether negligent or otherwise. I understand and acknowledge that the range is a location within which firearms and bows and arrows discharge deadly ammunition. I further understand that the discharging of firearms, bows and the use of ammunition and arrows hae inherent dangers that no amount of care, instruction, or expertise can eliminate. I further understand that the range is an indoor shooting facility that uses a series of component parts, including a bullet trap to decelerate discharged ammunition, howeer, I understand that these component parts and bullet traps of the range DO NOT, in eery instance, contain bullets or bullet fragments and hae been known to cause ricochets that could cause serious personal injury or death and I expressly and oluntarily assume all risks of personal injury or death sustained while using the range, including the risk of negligence of 3B'S Indoor Shooting Range and Training Center, its employees, agents and/or assigns, ricochets of bullets or bullet fragments, and/or the acts or omissions of third parties, and/or obious, hidden or latent defects in the range facilities, including but not limited to, the bullet traps and the associated and component parts of the range. I further agree that no oral representations, statements, or inducements apart from the foregoing written Agreement hae been made and that any modification of this Agreement will be effectie only if it is in writing and signed by the party to be charged. I hae made an independent ealuation of the risks associated with my use of the range, and based thereon, I re-affirm my assumption of the risks set forth aboe. I hae read and understand these rules and agree to follow and abide by each rule. I understand that eye protection, in the form of safety glasses, and hearing protection are aailable for my use for a nominal fee while I am at the range. I further represent that I am under no kind of restraint against the possession or use of a firearm, including any sort of domestic iolence restraining order, and that I am legally free to own, possess and handle firearms. 4 Must be followed by eeryone who enters the range 1. All firearms, air soft guns or bows (crossbows) brought into the range must be completely UNLOADED! 2. All firearms, bows, air soft guns, arrows, and ammunition are subject to inspection by the Range Officers. Targets may be brought into the range ONLY if approed by the Range Officer. Targets are aailable for sale at the main counter. 3. Rifles - Pistol caliber cartridges ONLY. 4. No one will be allowed to use or otherwise be on the premises of the range if he/she is under the influence of drugs and/or alcohol. Range Officers will bar anyone belieed to be under the influence of drugs or alcohol. 5. Weapons must be discharged only in the booths at the firing line. 6. Persons ages must be accompanied by an adult at all times, both on and off the range. 7. Eeryone must wear eye and ear protection while on the range. 8. No more than two shooters are allowed on a lane without Range Officer approal. 9. No steel jacket or steel core ammunition (armor piercing, incendiary rounds, tracer rounds) 10. Keep weapons pointed down range toward the backstop at all times. 11. Load and unload weapons in booth only. 12. No smoking, food or drinks in the range at any time. 13. Do not bring any loaded or jammed weapons off the range for any reason. If there is a problem, lay the weapon down in the booth, pointed down range, and notify a Range Officer. 14. No rapid-fire is permitted. Allow at least one second between shots. (Exception on training courses). 15. Do not exchange weapons with another lane. Switch shooters, not guns. 16. CEASE FIRE: On this command, unload your weapon, open action, lay weapon down, and step back from the line. Do not moe back into the booth for any reason until cleared to do so by a Range O fficer. 17. Rental weapons must be used with range ammunition only. 18. Any person seen firing at lights, target hangers or other range property or equipment will hae his/her membership and/or other priileges suspended, and will be reported to the proper authorities for appropriate action. Firing at boxes, cups, or objects other than approed targets is prohibited. All shooters will be held accountable for and will pay for any damage he/she (and/or his/her guests) causes, including replacement and installation charges. 19. No pregnant women will be allowed on the range at any time for the baby's safety. 20. Clean up after yourself. Brush brass forward of the firing line. 21. No open toe shoes or low cut shirts allowed. 22. No cross shooting allowed. 23. No photos of the interior allowed. 24. Neer cross oer the firing line. 25. No broad heads allowed - field tips only permitted. 26. No bows may be remoed from the case until at the firing line. The Range Officer will ban from the range any person handling a firearm, bow (crossbow) or air soft gun in an unsafe or careless manner. Your membership or priilege may be oid should such conduct w arrants. Vandalism: Any person seen firing at lights, baffles, carriers, or any other range property or equipment will hae their membership suspended, and will be reported to the proper authorities for appropriate action. Firing at anything that is not approed by the Range Officer is prohibited. All shooters will be held accountable and will pay for any damage you cause including replacement and installation charges. Firing line occupants: 1. All persons using the facilities shall be of good character. Appropriate dress is required at all times. 3B'S Shooting Range and Training Center is a family-oriented facility and we hae the right to refuse serice to anyone. Persons smelling of alcohol on their breath or who appear impaired will be required to leae the facility. 2. All persons utilizing the range are required to receie, understand and sign the rules, regulations and disclaimer and waier of liability notice. A responsible adult must accompany persons under twentyone (21) years of age. This adult shall act as a coach, and MUST remain with the minor(s) at all times while the minor(s) is utilizes the range. 3. Any equipment or other personal items in/on a shooting lane left unattended are at the Owner's risk. 3B'S Shooting Range and Training Center is not responsible for lost or stolen items. 4. All range brass is to be pushed forward - do not enter the range to retriee any brass. 5. Safety glasses are MANDATORY when entering the range. 3B'S sells safety glasses for a nominal fee or you may bring your own. Hearing protection is MANDATORY when entering the range. 3B's sells hearing protection for a nominal fee or you may bring your own.
