9 EXPRESS SOCCER CONSENT AND INDEMNITY FORM FOR THE DUBAI UAE CLASSIC FOOTBALL TOURNAMENT AND EXCURSION. 1. I.(Name) of (Address) being the parent / guardian of hereby give consent for my child to undertake a trip to Dubai to participate in the UAE Classic Football Tournament and to take part in extra mural activities while on tour during the period between 28 th Nov 3 rd Dec I request that (person in charge of the football club) be appointed to be in charge of my child and act in loco parentis during the course of the tour s activities. 3. I fully understand and accept that all the activities shall be participated in and undertaken at my child s own risk. 4.
