3 Eaton Community Schools Athletic Hall of Fame Dixon-Israel / Eaton Athletic Hall of Fame Nomination Form for Team, Athlete or Coach Mission Statement: This athletic hall of fame committee will use the bylaws in place to recognize the most outstanding athletes, coaches and teams who have performed at the varsity level for Eaton High School or Dixon-Israel High School. Any high schools that consolidated to form Eaton High School or Dixon Israel High School will also be part of the consideration process. Hall of Fame Merits: Select most outstanding varsity athletes. Select most outstanding varsity teams. Select most outstanding varsity coaches. Consideration of the era in which a coach, team or athlete participated should be taken into account during nomination process. Criteria of a varsity athlete- merits will be based on number of sports in which a varsity letter was earned, overall tenure as a high school athlete, league and state tournament recognition awards, rankings and placing in the state tournament, etc. Criteria of a varsity coach- merits will be based on time served in position, personal awards, team awards, player awards, rankings and place in state competition, etc. Criteria of varsity team- merits will be based on awards, rankings in state competition, etc. Please attach verification of nominee merits and statistics that the executive committee will consider for nomination/ induction to the hall of fame. (This could include Xerox of certificate, plaque awarded, yearbook, newspaper articles, etc.)
