1 2018 Boys National Championship II Check-In Times 9U-14U age divisions and 5 th -8 th grade divisions Location Myrtle Beach Sports Center 2115 Farlow St. Myrtle Beach, SC (Coaches and Players are required to attend) Date Monday, July 16 th Times/Notes 2:00pm-5:30pm Each team will have a specific check-in time below Note: if you have multiple teams and would like to check in together, please arrive at the assigned time of the earliest team Documents Required for both Check-Ins - Proof of Age (copy of a birth certificate or state issued ID with a date of birth on it or another form of ID like military ID or passport) - Proof of Grade player was in (copy of a report card or any official school print out listing the player s current grade and school year such as a progress report or online Power School portal print off) - Roster (please print off a blank NTBA Team Roster posted online and fill out and bring with you to team check-in)

2 Notes for Players & Parents - All players must be in full uniform during check in (either home or away jerseys are fine to wear) - Teams will take a team picture at the end of team check in (see info posted on our website about purchasing team pictures) - Players will wait on Courts 1 & 2 for their team name to be called to get interviewed (NTBA will make sure all players are who the paperwork says they are by asking questions from the birth certificates and report cards) - Parents can come and wait on Courts 1 & 2 as well if they want, plus we will have our 2018 National Championship t-shirts for sale during team check-in very limited # on certain sizes so we highly suggest buying them during the check-in Late Player Check-In Info If a player is going to check-in late the team must turn in their paperwork during the team check in and have them on the roster already. You MUST have all players listed on the roster you turn in even if they are arriving later in the week. All players will be interviewed during team check in. If a player is arriving late they must be interviewed before playing in their first game. Late check-in for individual players will be at the Myrtle Beach Sports Center. Late check-in players must also have a copy of their birth certificate and report card with them to be interviewed when showing up as well.
