3 Registration Instructions YOU MUST COMPLETE USA HOCKEY MEMBERSHIP AND ONLINE REGISTRATION FOR ALL OF YOUR PLAYERS PRIOR TO ATTENDING OPEN HOUSE. PLAYER NAME ON REGISTRATION MUST EXACTLY MATCH LEGAL NAME ON BIRTH CERTIFICATE. GETTING STARTED 1. Player Registration Paperwork Needed a. If not already on file, turn in Birth Certificate. b. 4 IMPORTANT REGISTRATION INFORMATION Outstanding balances must be PAID IN FULL in order for your child/children to participate in tryouts or evaluations Any player that loses his/her tryout jersey will be charged $40 Any player that doesn t return his/her game jerseys at the end of the season will be charged $100 per jersey If a skater ends up on a lower team after tryouts then what he/she registered for, CYHA will refund the difference via credit card refund or check by 12/1/2018. For example if you register for A Peewee and end up playing B Peewee, you will receive a credit card refund or check in the amount of $150 by 12/1/2018, but your online payments will still be at the A Peewee level. CYHA CAN T ADJUST THE PAYMENT PLAN IN SportsEngine. Tryout Fees are NON Refundable Skaters/goalies that register for C Peewee or C Squirt DO NOT need to attend tryouts The Consent to Treat Medical Form is no longer required by USA Hockey. 3

5 ONLINE REGISTRATION INFORMATION Follow the two-part process and steps below to complete online registration prior to attending an open house: Part 1-USA Hockey registration: 1. Go to and click the REGISTER ONLINE button located in the top right corner. 2. Click on the red REGISTER NOW button. 3. Check the box that acknowledges that YOU are 18 or older (person completing the registration must be 18 years of age). 4. REGISTRATION TYPE = Ice Player/Coach 5. Follow the prompts fill in all data correctly ENTER YOUR CHILD S NAME EXACTLY AS IT APPEARS ON THEIR BIRTH CERTIFICATE (i.e. Samuel Smith not Sam Smith, Kathryn King not Kathy King). Your player s paperwork is not valid unless you use their exact legal name on their birth certificate. 6. Process payment * Visa, Master Card, Discover or American Express (credit or debit card). A Visa gift card will also work. 7. PRINT your confirmation page it will also be ed to you after your registration is complete. (Repeat this process for all players in your family before heading to the CYHA website.) *Each player will be charged the USA Hockey fee of $40 and the MN Affiliate Hockey fee of $10, if birth year 2009 and earlier. If your child is birth year 2010 and younger (6 & under), there are no USAH and MN Affiliate fees, but player still needs to be registered at the USA Hockey site and receive confirm. #. Part 2-CYHA online registration: 1. Go to login upper left corner and click the Register/Tryout tab. 2. Continue the online registration process answering each question. (You will be asked the USAH Confirmation # received in step 7.) 3. 6 Must Haves at the Registration Open House OPEN HOUSE CHECKLIST 1. A copy of player s official government issued Birth Certificate (ONLY REQUIRED for players who DID NOT play during the season or who are new to CYHA.) (You may scan a copy of the birth certificate during the online registration process, attaching it to your registration.) 2. Signed Volunteer Contract and check for 10 hours of service per household, made payable to CYHA for $500 or two checks for $250 each (print player s name and level of play on memo line). 3. Waivers for players who do not live in District 10 are available for signing as needed (see Participation Rule below). If you have a current school waiver, then you will pick it up at the registration open house and turn it in with the rest of your paperwork. 4. Proof of enrollment for ALL open enrolled players MN Hockey is requiring that every player that is open enrolled provide proof of enrollment in an ISD12 school. This can be a printout from the parent portal, registration documentation, etc. It may be uploaded as part of the registration process or ed to the registrar. Copies of USAH Confirmation #, and CYHA registration information can be brought to the Registration Open House, but do not need to be turned in. Only four items listed above are required for each player. And, the waiver is only required by the affected players who do NOT live within ISD12 school district. PARTICIPATION RULE (Do you need a Waiver?) Those registering from outside our Association boundary (ISD No. 12) must have a completed Waiver form before participating in any CYHA activities. Per the MN Hockey Participation policy, a player can play where they live OR with the Association of their school enrollment, should it differ from their residence. Your High School Boundary Area is considered your Home Association. If you live outside the Centennial High School Boundary, but go to a Centennial School, you are eligible to play in the Centennial Youth Hockey Association. You will only be able to choose this option once. If you choose to join our Association, you must FIRST request a signed Waiver from your home Association BEFORE registering to play with CYHA. The signed Waiver can be obtained from your home Association Registrar or President. This completed/signed waiver would then be brought to an open house for the CYHA President to sign. Please start this process as soon as possible before registration. If you live outside of ISD12 AND are outside of District 10 Hockey boundaries, then additional waiver signatures are required. ANNUAL WAIVERS: Annual waivers are only assigned if you live and attend school outside of ISD12 school district. Typically these are only approved to help fill teams where there are shortages. 5

7 PARTICIPATION RULE (Do you need a Waiver?) Cont. New School Waiver: As of hockey season, USA Hockey mandated that players applying for a new school waiver cannot play at the highest level available, they must play down at least one level. Meaning, a new PeeWee waivered player cannot play at the AA level for the first year of the school waiver. New in : The exception is that 2 nd year Bantams who are open enrolling this year are allowed to play at the highest level. Attention to families that completed Waivers during/after the seasons: If nothing has changed with your residence and district of school attendance, you DO NOT need to renew your Waiver every year. CYHA REFUND POLICY All refund requests must be submitted in writing to the CYHA President. Requests received prior to the first tryout session will receive a full refund, less a $50 cancellation fee. The tryout fee will be refunded, if applicable. Requests received after tryouts have begun, and prior to team selection, will receive a 50% refund. Requests received after team selection and prior to November 1st, will receive a 25% refund. No refunds will be given after November 1st. There will be no refund of tryout fees once tryouts have begun. In the event of a season ending injury, a refund may be made at the discretion of the CYHA Executive Board. The amount refunded, if any, will be based on several factors such as the date of injury and the costs incurred by the Association on behalf of the player. This amount will be prorated depending on the percentage of the season played by the injured player. Any player officially being selected for and choosing to participate on the Centennial High School Varsity or Junior Varsity teams, will receive a refund at the discretion of the Executive Board. The refund will be based on several factors including the costs incurred by the Association on behalf of the player. Refunds will NOT be given to players leaving the Association to play for any other school or hockey program. Example based on a B Bantam registration fee of $1200. 8 TEAM ASSESSMENT PAYMENT POLICY CYHA charges a program fee for each level of play. That fee will cover all District 10 league games, any end of the season District 10 tournaments and a predetermined number of practice/home game ice hours. Your program fee will not cover invitational tournaments, hotel/travel costs, and additional practice ice purchased by the team, team apparel or any additional team training at outside facilities. The coach and manager for each team are responsible for setting a team assessment which will cover any necessary costs beyond the CYHA program fee. See the policy below regarding team assessments: Fees assessed by each team for the purposes of playing in invitational tournaments, additional practice or scrimmage ice time, etc., must be paid according to the payment schedule set by the Team Manager. Moving down to a younger age range is not allowed. To determine the correct playing level, locate your child s birthday. Detailed level description information can be found on the Team tab and then by Level at NOTE: Second year Squirt Players who have July or later birthdays can choose to play an additional year of Squirts or to play up to PeeWee with the rest of their classmates. This decision is made only once, and the player must remain 7

9 playing with that age group for the remaining years. Their age will be verified with their birth certificate copy turned in with their registration. Cougar Cub, Mite C and Mite D players can NOT register as goalies. Financial assistance is available for those families that are on the "free or reduced lunch program". Please contact the CYHA Registrar prior to starting the registration 8

10 process. You will need to provide documentation from the school district that verifies you are part of this program. Goalies pay ONLY registration fee if they have their own equipment. ** Skaters and goalies that register for C Peewee or C Squirt DO NOT attend tryouts. An annual Arena Fee has been added to help cover CYHA's financial commitment to Centennial Sports Arena ARENA FEE An Arena Fee has added this year to help cover CYHA's financial commitments to Centennial Sports Arena. The Arena fee will be an annual fee and will be charged to every player registering with CYHA as follows. Mite B and above - $50 per player Mite C - $25 per player Mite D and Cougar Cubs - no Arena Fee These funds will be used for Centennial Sports Arena operational maintenance and facility upgrades. ONLINE VOLUNTEER SYSTEM (DIBS) CYHA now uses the DIBS online volunteer system to assign and track volunteer opportunities. Families can log on to to view and claim available volunteer opportunities. The same member account and password used to register your player(s) will be used with DIBS. (DO NOT CREATE A NEW ACCOUNT.) Additional information on the DIBS system is available on the website. VOLUNTEER POLICY Centennial Youth Hockey Association depends on the assistance of its members to function as a successful organization. It is critical that we all share in the commitment to produce a program that operates efficiently. 10 volunteer hours per family will be required to satisfy this need. Volunteer opportunities can be completed from April through March each year. 9 A separate check payable to CYHA for the $500 volunteer fee (or two checks for $250 each) is mandatory at the time of registration. This check(s) is a deposit and will not be cashed unless the volunteer requirement is not met. After all hours have been completed the check(s) will be shredded. The shred dates will be the end of December and the end of March. If a family has not completed 10 volunteer hours by signing up for online opportunities by March 15 th, the full deposit will be forfeited, and the check will be cashed March 30th. If two checks are written, the 1 st will be cashed March 30 th and the 2 nd check

11 cashed April 30 th. Coaches and managers listed on official team rosters will automatically be given full credit once CYHA Registrar has the official team rosters signed. Teams will only be able to claim up to 4 coach/manager credits. FOR ACCURATE VOLUNTEER CREDIT, FAMILIES MUST SIGN UP FOR VOLUNTEER OPPORTUNITIES UNDER ONE NAME. DO NOT SIGN UP FOR VOLUNTEER HOURS UNDER MULTIPLE NAMES. Cougar Cub, Mite D and Jr Gold players are exempt from volunteer hours and are not required to fill out the volunteer contract or turn in a check. 10

14 CENTENNIAL YOUTH HOCKEY ASSOCIATION MITE PROGRAM Guidelines Time and Cost Expectations Ages 4 8 (As of July 1, 2018) Boys and Girls (Program subject to change at any time due to District 10/CYHA program changes) Objective: To provide a hockey program for children that will teach them the fundamentals of the game, as well as, serve as their foundation for the future. This will be accomplished by providing participants the opportunity to develop the skills needed to play the game of hockey, ranging from skating skills to more advanced team concepts. The Mite program will be divided into 5 (five) classifications in order to serve the varying degrees of age, skill and ability of the participants. Team placement is determined by an on-ice evaluation just prior to the beginning of the season. Cougar Cubs Learn to Skate Program teaches the most basic skills of hockey and is intended for the children that have an interest in playing but have little or no skating experience. Full equipment required (see below for description), with the exception of shoulder pads, which are optional. NO EVALUATION HOUR Weekend Sessions - Nov-Feb. Required Equipment: Helmet with mask/shield, mouth guard (cannot be clear), hockey skates, stick, elbow pads, shin guards, breezers/hockey pants, supporter/pelvic protector and gloves, shoulder pads (optional), neck guard (optional). Recommended Equipment: Shoulder pads, neck guard. CYHA Provided Equipment: A jersey and hockey socks. Minimum skill requirements: None Number of Hockey Events Per week: 1 event per week beginning in November and concluding in February. Schedule is pre-set at time of registration.. SEE Cougar Cubs Program Flyer in Registration Package. U8 Girls This program is designed for girls only that are of Mite age, but at the B/A level (2 nd -3 rd Graders). Girls are not required to play on the U8 girls teams; they can register and play on Mite teams, levels A D. CYHA offers this program due to the interest from girls in this age group to play on teams of all girl players. D10 numbers at the 8U level will determine the number of U8 teams Centennial will roster. If necessary CYHA will form more than one U8 team and CYHA will determine if there are two balanced teams or Upper Level U8 Girls (Cont) and Lower Level U8 teams. If there is only one U8 team, Girls that don t evaluate at the U8 level can also still play the Mite level they were placed at from evaluations (A-D). The program details would be similar to C for the Lower U8 team and A/B for the Upper U8 team. CYHA will determine the overall number of 8U girls teams based on registration numbers. For , CYHA has determined we will have one Upper level 8U team (2 nd /3 rd graders only). However, this won t be finalized until after evaluations when we have a final count on numbers by level. Girls interested in playing U8, may be evaluated with the A/B level mite players. If for some reason we can t field a U8 team, the girls will be placed at the appropriate Mite level (A-D). Mite D This program is designed for children who have some prior experience and will be conducted in a team format. Primarily for Kindergarteners and 1 st graders (ages 4-6). This is an In-House program that follows USA Hockey s American Development Model (ADM). The focus of this program will be continued individual skating/skill development, while introducing basic team concepts. Areas of skill development include: forward skating, backward skating, stopping, shooting and stick handling. Team concepts include: passing, shooting and positioning. There will also be ½ ice games to work on team concepts. Required Equipment: Helmet with mask/shield, mouth guard (cannot be clear), hockey skates, stick, elbow pads, shin guards, shoulder pads, breezers/hockey pants, supporter/pelvic protector and gloves, neck guard (optional). 13

15 Mite D (Cont) CYHA Provided Equipment: A jersey and hockey socks. Minimum skill requirements: Participant must be able to skate forward at a reasonable pace without assistance or the use of walkers or props. Number of Hockey Events Per week: Average of 1-2 events per week beginning in November and concluding in March. D10 guidelines request a 3:1 practice to game ratio. Number of Games for D Mite: 6-8 games + 1 jamboree (subject to change by District 10/CYHA). CYHA Program Fee: The CYHA program fee charged at registration covers the expenses of practice and game indoor ice time, jersey/socks, and 1 end of season jamboree (in-house). Team Assessment: Each team may assess an Mite D (Cont) additional team assessment fee to cover additional items for example: indoor gym rental, additional indoor ice rental, coach gifts, and end of season party (player gifts, cake, misc). This fee should be discussed and agreed to at the first parent meeting. Additional ice hours: CYHA suggests that outdoor ice be used when available (it s free). If necessary, CYHA suggests that teams at this level buy no more than 2 additional hours. All team parents must agree to this expenditure and parents will be assessed for this ice time in the team assessment. Evaluations: 1-hour group skating evaluation. Teams will be formed after the evaluation. Practice and Game Information: Indoor practices will be held with 3-4 teams on the ice at a time. There will be cross ice and ½ ice games/scrimmages scheduled throughout the season, scheduled by CYHA. The focus of these games/scrimmages will be on positioning and will be conducted in a coach-controlled environment. Mite C/B This program is designed for children who have an intermediate level of experience, primarily consisting of 1 st, 2 nd and 3 rd graders. This program will be conducted in a team format following USA Hockey s ADM process (a mix of full ice and ½ ice games). The focus of this program will be continued individual skating/skill development, while advancing the team concepts taught at the Mite D level. Required Equipment: Helmet with mask/shield, mouth guard (cannot be clear), hockey skates, stick, elbow pads, shin guards, shoulder pads, breezers/hockey pants, supporter/pelvic protector and gloves, neck guard (optional). CYHA Provided Equipment: A jersey and hockey socks. Minimum skill requirements: Participant must be able to skate forward smoothly, backward at a reasonable pace without assistance, and be able to stop in one direction without falling. Number of Hockey Events Per week: Average of 1-3 events per week beginning November and concluding in March. D10 guidelines request a 3:1 practice to game ratio. Number of Games for C/B Mite: C mite: play 100% cross ice/half ice games. B mite: Max 10 full ice games + 2 jamborees (subject to change by District 10/CYHA). Cross Mite C/B (Cont) for this ice time in the team assessment. Evaluations: 1-hour group skating evaluation. Teams will be formed after the evaluation. Goalies are introduced at this level. Players in their last year of Mite eligibility are allowed to register as full time goalies. If a team has a full time goalie any player interested in trying the goalie position during the season must still be given an opportunity to do so. Players registering as full time goalies will still be evaluated as skaters during evaluations and then play goalie at the level they evaluate to. Practice and Game Information: Indoor practices will be held with 2-3 teams on the ice at a time. There may be some dry land practices in the early portion of the season, before outdoor ice is available. CYHA will coordinate a game schedule with other D10 associations. Games will be scheduled in December through March. Mite A This program is designed for children who previously participated in Mite B or C with advanced skating skills (2 nd and 3 rd graders only, generally in their 4 th or 5 th year of hockey). This program will be conducted in a team format following USA Hockey s ADM process (a mix 14

16 ice games will also be played (there is no limit on cross ice games). Per D10 guidelines, full ice games are only allowed after December 31 st of each year. CYHA Program Fee: The CYHA program fee charged at registration covers the expenses of practice and game indoor ice time, referees, jersey/socks, and 1 jamboree. Team Assessment: Each team may assess an additional team assessment fee to cover additional items for example: indoor gym rental, additional indoor ice rental, coach gifts, and end of season party (player gifts, cake, misc). This fee should be discussed and agreed to at the first parent meeting. Additional ice hours: Coaches may want to purchase additional practice ice hours. CYHA suggests that outdoor ice be used when available (it s free). If necessary, CYHA suggests that teams at this level buy no more than 5 additional hours. All team parents must agree to this expenditure and parents will be assessed of full ice and ½ ice games). The focus of this program will be continued individual skating/skill development with increased emphasis placed on the team concepts learned in the previous levels. Required Equipment: Helmet with mask/shield, mouth guard (cannot be clear), hockey skates, stick, elbow pads, shin guards, shoulder pads, breezers/hockey pants, supporter/pelvic protector and gloves, neck guard (optional). CYHA Provided Equipment: A jersey and hockey socks. Minimum skill requirements: Participant must be able to skate forward and backwards smoothly, perform forward crossovers while handling a puck, backward stops, and stopping in both directions without falling. Number of Hockey Events Per week: Average of 1-4 events per week beginning in November and concluding in March. D10 guidelines request a 3:1 practice to game ratio. Mite A (Cont) Mite A (Cont) Number of Games for A Mite: Max 10 games + 2 jamborees (subject to change by District 10/CYHA). Cross ice games will also be played (there is no limit on cross ice games). Per D10 guidelines, full ice games are only allowed after December 31 st of each year. CYHA Program Fee: The CYHA program fee charged at registration covers the expenses of practice and game indoor ice time, referees, jersey/socks, and 1 jamboree. Teams will be formed after the evaluation. An additional 1-2 hour evaluation will be held for A players. Players in their last year of Mite eligibility are allowed to register as full time goalies. Players registering as full time goalies will still be evaluated as skaters during evaluations and then play goalie at the level they evaluate to. Practice and Game Information: Indoor practices will be held with 1-2 teams on the ice at a time. There may be some dry land practices in the early portion of the season, before outdoor ice is available. CYHA will coordinate a game schedule with other D10 associations. Games will be scheduled in December through March. 15

19 Centennial Cougar Cubs Learn to Skate Program Fee: $50 Includes jersey, socks, participation medal and 12 one-hour indoor ice sessions at Centennial Sports Arena This program is designed to teach the most basic skills of hockey and is intended for the children who have an interest in playing but little or no skating experience. Sessions will be held indoors and will be taught by CYHA coaches and players as well as other membership volunteers. Required Equipment: helmet with mask/shield, mouth guard (cannot be clear), hockey skates, stick, elbow pads, shin guards, breezers/hockey pants, supporter/pelvic protector and gloves. Recommended Equipment: shoulder pads, neck guard.
