1 SECTION 1: TRUE OR FALSE 1. A player from Team A receives his third minor penalty of the game and a player from Team B also receives a minor penalty. Team A is not required to place a replacement player in the penalty box to serve the minor penalty. 2. Whenever an additional puck is observed on the playing surface, even if not interfering with the game, play must be stopped immediately. 3. For all match penalties, a total of 5 minutes shall be recorded on the Official Scoresheet against the offending player or team official. 4. A team may change goalkeepers to defend against a penalty shot. 5. If a team is unable to place the correct number of players on the playing surface, due to injuries or penalties, the game shall be forfeited and the non- offending team declared the winner. 6. A player from Team A leaves the penalty box prematurely, due to error by the penalty timekeeper, and Team B inadvertently shoots the puck into its own goal. The goal counts. 7. All double minor penalties are treated as two separate minor penalties and recorded on the Official Scoresheet as such. 8. The ensuing face- off location after the puck is shot off the crossbar and out of play by the attacking team in the attacking zone is the center face- off spot. 9. The proper sequence of penalties to call against a team official who continues to verbally abuse an official is: minor, misconduct, game ejection, game misconduct. 10. A major plus a game misconduct or a match penalty must be assessed in every instance where an injury results from body checking. 11. A team shall be allowed to call its timeout in order to warm up its goalkeeper or a substitute goalkeeper. 12. All players, including goalkeepers, are bound by the within five feet of the bench criteria when they are being substituted for by another player. 13. A major penalty cannot be treated as coincidental with a match penalty. 14. The ensuing face- off location after a stoppage of play caused by an attacking player in the attacking zone is always the center face- off spot. 15. A player and an official accidentally collide on the rink and the player uses profane language to the official for being in the way. A misconduct penalty shall be assessed. SECTION 2: PENALTY EXPIRATIONS In the following situations, determine which player, if any, returns to the playing surface once the goal is scored. All numbers after the player refer to the penalty minutes assessed. A. Player A1 returns to the rink. B. Player A1 first minor terminates. C. Substitute for A1 returns to rink. D.
