1 Rocky Mountain Cup AAA Spring Hockey Tournament JASPER ALBERTA 2015 Manager s Package AAA OIL KINGS ROCKY MOUNTAIN CUP SPRING HOCKEY TOURNAMENT The AAA OIL KINGS is pleased to invite your team to participate in the Rocky Mountain Cup AAA Spring Hockey Tournament. The tournament will be held in Jasper Alberta games to be played at The Jasper arena APRIL APRIL APRIL April 29 May Gold May Welcome to the third annual Rocky Mountain Cup Tournament This year, the tournament will feature teams for the age groups 2006 to Maximum 20 player s only on a roster. There will be a minimum 2 referees per game and games will have an allotted two hour time slot with fifteen minutes in-between games. Flooding will be every two periods or the ice keeper s discretion. The tournament will be a 7 team draw with each team guaranteed 5 games. A round robin followed by, semi Finals and a championship game. We will also have a six team schedule prepared in case.

2 There will be custom gold, silver and bronze medal awards for 1 st, 2 nd and 3 rd participants. Championship trophy awarded to gold medal team. Game 3 star selection awarded after each game. The entry fee for the Rocky Mountain Cup AAA Tournament is $2100. A 500$ deposit holds your spot. The total fee is due March 1, Please note that all teams are required to carry their own insurance for the tournament. Tournament waivers are to be carried by the Manager of the team at all times. Copy of Player s birth Certificates is to be carried by the Manager of the Team at all times. There are 60 rooms reserved at the Chateau Jasper and 60 rooms reserved at the Lobstick Lodge as well; they have given us a generous tournament rate. 6 The team manager or coach on behalf of his/her team releases the sponsors of the above named tournament, its officials, and all concerned with the tournament for all liability for injury or accident that may be incurred by any player, team official, or parent while participating in the tournament. 7 Waiver and Release of Liability In consideration of being allowed to participate in any way in the Rocky Mountain Cup AAA hockey tournament, (team name) athletic/sports program related events and activities, the undersigned acknowledges, appreciates and agrees that: 1. The risk of injury from activities involved with this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce the risk, the risk of serious injury does exist; and, 2. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation; and, 3. I willingly agree to comply with the stated and customary terms and conditions for participation. If however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless the AAA OIL KINGS Organization, their officers, officials, agents and/ employees, other participants, sponsoring agencies, sponsors, advertisers and if applicable, owners and lessors of premises used to conduct the event (Releases) with respect to any and all injury, disability, death or loss or damage to person or property, whether caused by the negligence of the releases or otherwise, I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it and sign it freely and voluntarily without any inducement. Tournament Day Teams are required to carry their own insurance for the tournament and provide proof with certificate of insurance to Tournament Administrator Tournament Day Proof of Birth (Birth certificates) and signed waivers are required for all players. (See waiver form included) Manager to keep these during the Entire Tournament to be ready for viewing at any time during the tournament.
