1 New Hire Safety Orientation Welcome to Flexicrew, our number one priority is you and your safety. 1

2 New Employee Responsibilities As a contract worker with Flexicrew Staffing, I understand by signing this document, that I have read and reviewed the expectations that are a condition of continued employment. 1) If for any reason I decide to end my assignment prematurely, I agree to give a one (1) week notice. If I fail to give notice I understand my pay will be reduced to minimum wage. 2) If I am released from my assignment due to gross negligence, such as continued unexcused absence, disruptive or unsafe behaviors or disrespect to supervisor, I understand that my pay will be reduced to minimum wage. 3) I understand that consuming or possessing alcohol is prohibited on all Flexicrew jobsites and is grounds for immediate termination and ineligibility for rehire. 4) I understand that as an employee of Flexicrew I am subject to pre-employment, post accident, and random drug screenings and that consuming or possessing illegal drugs while on assignment with Flexicrew is prohibited. I understand that I MUST make Flexicrew aware of any prescribed medication which may negatively impact my ability to perform the functions of the position. 5) I understand that once I have completed an assignment, it is my responsibility to contact Flexicrew within 24 hours of my last day worked and at least once a week to update my availability and understand that by failing to do so is considered a voluntary resignation by me and I waive all right to unemployment benefits and claims. 6) I understand that working safely is a condition of employment and agree to wear proper PPE, participate in safety meetings/jsa s, and report any hazards to my supervisor. 7) I will report ALL injuries to my immediate supervisor as well as Flexicrew and follow all Jobsite Injury Reporting Procedures as outlined by Flexicrew. Failure to do so may cause Worker s Compensation benefits to be delayed or jeopardized. Failure to comply with any of the above expectations while on assignment with Flexicrew could result in one or more of the following: Immediate Termination and Ineligibility for Rehire Pay reduced to minimum wage for last pay period worked Unemployment Benefits or Worker s Compensation delayed or denied 2

3 Job Safety Analysis Job Safety Analyses should be completed and presented by your supervisor before starting work. JSA s should also be completed: After an accident or near miss If the work conditions or scope of work changes 3

4 Hazardous Communication Right to Know Employees have the right to know what chemicals are in the workplace, if they are hazardous or not and the effects they can have. Employees have the right to know the location of the Material Safety Data Sheets and how to read them. If you have a question contact your supervisor. 4

5 Personal Protective Equipment Head Protection Workers must wear a hard hat when working in areas where there is a potential for injury to the head from falling objects. Workers must wear hard hats designed to reduce electrical shock hazards where there is potential for electric shock or burns. Wearing a hard hat can reduce the impact from striking low overhead objects. Hard hats must comply with ANSI Z89.1 5

6 Personal Protective Equipment Foot Protection Steel toe boots are required on most client sites. Some sites may require job/site specific safety footwear such as: Rubber Boots Chemical Resistant Boots Slip Resistant Footwear Footwear Resistant to Electrical Current/Static 6

7 Personal Protective Equipment Hearing Protection Employees must use proper hearing protection on all client sites as required. Hearing protection must reduce noise to acceptable levels. Hearing protection is required to be worn while grinding abrasive blasting needle gunning 7

8 Personal Protective Equipment Respiratory Protection If needed, respirators will be available after proper fit testing and training has been provided, a medical evaluation is required for certain respirators. Eye Protection ANSI Z 87approved safety glasses will be worn on all client sites where required. Face shields and safety glasses will be worn while grinding, using a burning tool, working with chemicals or in any circumstance which needs more eye protection than glasses can provide. 8

9 Personal Protective Equipment Protective Clothing Clothing will be in good condition and suited to protect employee, no tank tops, shorts or loose fitting clothing shall be worn. Coveralls Welding Leathers/Aprons Flame Retardant Clothing (FRC) Reflective Safety Vests/High Visibility Safety Shirts 9

10 Personal Flotation Devices Working over water While working directly over water a PFD (Personal Floatation Device) must be worn Prior to each use, personal flotation devices shall be inspected. Defective personal flotation devices shall not be used. PDFs should also be worn and fastened properly 10

11 Personal Fall Protection Before using personal fall protection employees must be trained on: When to use fall protection Proper fit & Inspection How to use and limits of personal fall protection 11

12 Safe Lift Technique While lifting keep your back straight and bend at the knees If the material is too heavy, ask for help! 12

13 Ladders Inspect all ladders prior to use Always maintain 3 points of contact Extension ladders must extend 3 feet above surface and be secured at the top. Scaffolding Scaffolding shall be properly erected and tagged by a scaffolding competent person. Scaffolding shall be tagged with 3 colors: RED no admittance period YELLOW 100% tie off required GREEN no fall protection is required, all components are in place. Ladders & Scaffolds 13

14 Electrical Safety Only qualified electricians shall work on electrical equipment, Inspect all electrical tools prior to use Worn or frayed electric cables shall not be used Tag defective tools and turn in for repair Do not allow cords to be placed in water 14

15 General Safety Never cross red danger tape Adhere to all signs on jobsite Never entered a restricted access zone unless authorized to do so. Never enter a confined space without proper training and permit signed by a competent person that space is safe for entry. 15

16 Crane Safety The safe working load of the overhead hoist, as determined by the manufacturer, shall not be exceeded Only qualified personnel shall operate cranes Never stand under a load If you are not involved in lift operations, stay out of the area 16

17 Forklift Safety Only authorized employees are to operate forklifts Before using a forklift, examine it for hazardous conditions which would make it unsafe to operate Follow safe procedures for picking up, putting down, and stacking loads Seat belts MUST be worn at all times Remove unsafe or defective forklifts from service until the defect is properly repaired 17

18 Slips, Trips, and Falls Housekeeping All lines and leads need to be in trees when possible or to one side out of walkways to prevent tripping hazards Work areas need to be kept clear of debris and flammables If you create debris CLEAN IT UP 18

19 Hazard Recognition Be aware of your surroundings at all times If you see a hazard, let your supervisor know immediately so it can be addressed 19

20 SITE SPECIFIC SAFETY Each client/jobsite will have site specific safety rules. You are required to follow all site specific safety rules on the client s jobsite. Notify your site supervisor or Flexicrew Representative if you observe any hazards not being addressed at your worksite. If you have questions, contact your supervisor or Flexicrew Representative. 20

21 Accident Reporting All accidents and near misses MUST be reported to your supervisor and Flexicrew immediately. No matter how small it seems, it needs to be reported. If you have any questions contact your supervisor or Flexicrew Representative. 21
