7 VAULT PIPELINES COMMITMENT AND POLICIES Vault Pipelines policies are part of the Orientation Handbook given to each worker prior to starting work. Copies are also available from your Supervisor or HSE representative.

8 INDUSTRY RULES Obtain authorization before entering a confined space. Protect yourself against a fall when working at heights. No alcohol or drugs while on duty. Obtain authorization before overriding or disabling safety critical equipment. Verify isolation before work begins and use the specific life protecting equipment. Work with a valid permit when required. Do not put yourself in an unsafe position in relation to Mining mobile heavy equipment. (This rule is specific to Oil Sands Mining operations only.) Follow OSSA Driving in the Mine protocols. Life Saving Rules

9 INDUSTRY RULES Wear your seatbelt. Follow prescribed journey management plan. Prevent dropped objects. Position yourself in a safe zone in relation to moving equipment. Conduct gas tests when required. Obtain authorization before starting excavation activities. Supplemental Rules Wear a personal flotation device, when required. Follow prescribed lift plan. Do not walk under a suspended load. While driving, do not use your phone and do not exceed speed limits. Do not work under or near overhead electric power lines. Do not smoke outside designated smoking areas without all required authorization and permits.

Maintain adequate distance to keep you safe. Obtain authorization before starting excavation activities. Excavation activities may contain hazards such as electrical cables, confined space, collapse of walls or excavated material. Speeding or using your phone while driving increases the risk of losing control of your vehicle. A seat belt protects you from injury in the event of an incident while driving and keeps you safe. There is a significant risk of dropped objects when using tools and portable equipment at height. Preventing objects from falling keeps you and people working below you safe. Working in the line of fire of moving equipment and energized equipment is unsafe as this can impact you. Personal flotation devices should always be worn when there is a danger of falling into water. Working or walking immediately under a suspended load is unsafe as the load can fall on you. Smoking or use of matches or cigarette lighters could set on fire flammable materials. Designated smoking areas will keep you safe from causing fire and explosion. A lift plan describes how to lift and hoist safely. For routine lifts, there needs to be a general lift plan. For non-routine lifts, including complex and heavy lifts, the plan is specific. A journey management plan is a plan for you as a driver that will help you to travel and arrive safely. Air is tested to stop explosions and/or make sure you can breathe the air safely

11 Policies, Procedures and Practices are located in the safety meeting location, with your site Supervisor and your HSE Representative. You are required to read and understand these documents. Ask questions and actively participate in their review.

13 INCIDENTS What is an incident? Any unplanned and unwanted event which results in personal injury, illness, environmental release or damage. Who reports an incident? Anyone with knowledge of it. We all have a responsibility to report hazards. How is an incident reported? Immediately contact your Supervisor and / or an HSE representative. What if there is no injury or damage? It s called a near miss and it still needs to be reported. A near miss is any undesired event or incident that under slightly different circumstances could have resulted in personal injury, illness, environmental release or damage.

14 INCIDENT INVESTIGATIONS Incident investigations are not conducted to determine blame. They are conducted to identify root causes and prevent recurrences. Who does the Investigation? Depending on the severity of the incident or near miss, site personnel will conduct the investigation and report the facts and findings to Vault Pipelines management. You may be asked to assist or give a statement. If you witness an incident or near miss, contact your Supervisor and HSE representation immediately then freeze the scene. If there is a medical emergency, render assistance, if possible, and call emergency response.

15 WHAT IF YOU ARE HURT AT WORK? If you are injured or experience work related illness, contact your Supervisor immediately. Vault Pipelines is committed to supporting the prompt recovery and return to work of an ill or injured worker. Participation in the Modified Work Program is a condition of employment. Modified duties will depend on the current work available and will be arranged on an individual basis. Modified work needs to be: Achievable - given the worker's injury, is the worker able to do it physically? Safe - the modified work plan should not endanger the worker's recovery or safety or the safety of others. Constructive - the modified work plan should contribute to the worker's skill development and return to full duties. Productive - the worker's duties should be meaningful to the organization.

16 TWO LEADING CAUSES OF INJURIES Injuries from Slips, Trips and Falls: Since 2011, our industry experienced 18.5% of loss time incidents due to slips, trips and falls. Of this, 46% involved injuries to the back. Prevent injuries by practicing good housekeeping skills, cleaning up spills, remove obstacles, secure floor covering, remove or secure cables, use appropriate lighting and wear proper footwear.

17 TWO LEADING CAUSES OF INJURIES Musculoskeletal / Repetitive Strain Injuries: Since 2011, 14% of loss time incidents were a result of overexertion. Eliminate or reduce injuries by following proper manual material handling practices and safe lifting procedures and by eliminating repetitive work through job design and job rotation. Lifting can mean lifting, carrying, pushing and pulling. Maximum lift it 23 kg (50 lbs) per worker. Use lifting equipment where possible.

18 VAULT PIPELINES GENERAL RULES Workers are to wear the PPE required for their work in accordance with Alberta OHS Legislation, Vault Pipelines and Client rules. All incidents, regardless of their nature, shall be promptly reported. Work with a valid permit, when required. All work shall be conducted in accordance with safe work practices. Obtain authorization before entering a confined space. Verify isolation before work begins and use the specific life protecting equipment. Protect yourself against a fall when working at heights. Position yourself in a safe zone in relation to moving equipment. Do not walk under a suspended load. Hand tools shall not be used for any purpose other than specified by the Manufacturer.

19 VAULT PIPELINES GENERAL RULES Maintain housekeeping in the work area. Possession, consumption or use of intoxicating beverages or unauthorized drugs on the job is strictly forbidden. Smoke and the use of e-cigarettes in designated areas. Horseplay and fighting on the job is strictly forbidden. Wear your seatbelt. Use a spotter, when available. Vault Pipelines takes the safety of our workers and visitors very seriously. Non-compliance with all safety rules could result in disciplinary actions up to and including immediate removal from site and termination of employment.

20 PERSONAL PROTECTIVE EQUIPMENT You must wear the following personal protective equipment (PPE): Long sleeve shirt Long pamts CSA Grade 1 safety boots CSA approved hard hat CSA approved safety glasses Leather gloves High visibility jacket, vest or stripes In some work areas, you may be required to also wear: Fire Retardant clothing Hearing or double hearing protection Respiratory protection Face protection Specialty PPE Let s have a quick audit of your PPE! Talk with your Supervisor about site or task specific requirements.

21 BEHAVIOUR BASED SAFETY Workplace Inspections help prevent injuries and illnesses. Through critical examination of the workplace, Inspections identify and record hazards for corrective action. During an Inspection, both activities and conditions are to be carefully examined. Situations that have potential to cause unsafe acts or unsafe conditions are identified and corrective actions are initiated. Formal Inspections Documented Formal Inspections will be conducted at locations as follows: Office Monthly Shop/ Yards Weekly Worksites Weekly Informal Inspections All locations - will be subject to daily Inspections completed at all worksites on an ongoing basis. Documented Formal Inspections will be conducted by these individuals at the following frequencies: All Site Supervisors - conduct a minimum of 2 Formal Inspections per week preferably with a worker or junior Supervisor Workers participate with site Supervision or HSE in 2 Inspections per month

22 BEHAVIOUR BASED SAFETY The aim of Behaviour Based Safety (BBS) is to reinforce safe behaviours and identify at-risk behaviours. Hazardous situations can be avoided before they occur, leading to a reduction in near misses and incidents. BBS focuses on the safe behaviours with positive reinforcement and encouragement. The benefit of BBS is realized when employees take ownership of the initiative and can actively participate in work safe habits through peer coaching.

24 COMPETENCY EVALUATION AND TRAINING / MENTORING Competency All new workers to Vault Pipelines will participate in a competency evaluation. The workers skills are assessed against the job description for their position to determine if there are any areas requiring training or mentoring (gap analysis). Site Supervision uses the gap analysis to prepare a training or mentoring development plan.

25 Mentorship All Green Hands will participate in the Mentorship program development plan. Site Supervision will select a competent mentor based on the skills the Green Hand requires to be developed. Graduation can take 3-6 months and will vary based on the amount of mentoring and skill development required. Short Service Workers may also participate in a development plan as determined by site Supervision. Through positive processes, feedback and worker engagement, continuous improvement leads to a competent workforce and lowered near miss and incident rate.

26 Training Depending on the scope of work and the worker s training needs, training will be offered as necessary. The expectation is that all workers requiring the training will attend. Some training may be required to be taken outside of work hours and is generally a requirement of employment based on position or scope of work.
