We have established a drug-free workplace program that balances our respect for individuals with the need to maintain an alcohol and drugfree environment. Behavior Based Safety 35 Behavior Based Safety (BBS) initiative is an education and observation process used to improve safety and reduce risk in the workplace. 4

Fit for Duty 198 Employees are expected to report for work fit for duty, which means able to perform their job duties in a safe, appropriate and an effective manner free from the adverse effects of physical, mental, emotional and personal problems. 7
8 Hazard Communication 234 The purpose of this program is to ensure that the hazards of all chemicals and substances are evaluated and the information concerning their health and physical hazards is communicated to employees when working with these chemicals. Lockout Tagout 304 The purpose of this program is to establish procedures for affixing appropriate lockout/tagout equipment to energy isolating devices and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy to prevent injury or incident. Manual Lifting 317 COMPANY is committed to providing a safe and healthy working environment for all employees. Musculoskeletal disorders (MSD) account for a majority of reported injuries and we must minimize the risk and incidence of MSDs. To achieve this goal, COMPANY requires each worksite to establish and maintain a MSD, Lifting and Handling Loads Program with the following elements: 8
9 Personal Protective Equipment and Assessment 333 The purpose of the Personal Protective Equipment section is to set forth the procedures for the use, care, and maintenance of personal protective equipment required to be used by employees for the prevention of injuries. R1 Respiratory Protection 346 COMPANY shall provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace. 11 Aerial Lifts 20 The purpose of this program is to define the requirements for safely operating an aerial lift device. Compressed Gas Cylinders 50 The purpose of this program is to prevent injury from failing or failure of compressed gas cylinders and to establish requirements for handling, lifting and storing compressed gas cylinders safely. Fire Extinguishers 192 The purpose of this program is to provide fire extinguisher procedures to ensure equipment is operable and employees have the knowledge to safely operate in case of a fire incident. 10
12 Fire Extinguishers 192 Forklift and Industrial Trucks 202 The purpose of this program is to establish requirements for the safe operation and use of Powered Industrial Trucks. Hand & Power Tools 225 The purpose of this program is to provide establish requirements for the safe operation of hand and power tools and other portable tools, including proper guarding. All hand and power tools shall be maintained in a safe condition. 11
13 Ladder Safety 286 The purpose of the program is to prescribe rules and establish minimum requirements for the construction, care, and use of the common types of ladders. Ladder Safety 286 Ladder Safety
14 Rigging/Material Handling 373 The purpose of this training program is to ensure a safe and incident free lifting operation. Scaffolds 375 The purpose of this program is to prevent injuries due to falls from elevated work areas and ensure employees and contractors are able to inspect scaffolding materials and erected scaffolds. Tips Included on your flash drive is a Microsoft Word version of the, along with a PDF copy. The has been created as a Microsoft Word document so that each company using it, can pick and choose which sections they wish to use for particular jobs and can customize the document with their company information, logo and other pertinent details. Each individual section of the manual has a header at the top of the page where you can insert your company logo, company name, date and project information as needed. The document also includes a Table of Contents and a glossary of key terms. 13
15 Tips HOW TO INSERT YOUR COMPANY LOGO Go to the header of the section you wish to customize (as shown below). Double click in the box that says Insert Your Logo Here. This will open up the header box and allow you to modify text and insert your logo within the header for that section of the. Open your company s logo graphic, right click on it and select copy. (Or you can go to file and select copy.) Place your cursor in the box where you will insert your logo, right click and select paste. This should insert your logo into the header at the top of each page within that section. Tips HOW TO INSERT OTHER HEADER INFORMATION You can click on Insert Date and type in the date to update this information within each section. You can also click on COMPANY and type in your company name to update this information within each section. Once you are done making edits in the header box, click the close button.
