3 1.0 Purpose This document establishes the requirements, process, and administrative responsibilities for UIC s Control of Hazardous Energy (Lockout/Tagout) Program. The program adheres to the Illinois Department of Labor OSHA, and Federal Occupational Safety and Health Administration (OSHA) Control of Hazardous Energy (Lockout/Tagout) 29 CFR standard. The program is to provide safety requirements and prevent worker injury by isolating the machine or equipment from the energy source and rendering the machine inoperative during servicing or maintenance where the unexpected energizing, startup or release of stored energy could occur and cause injury. 2.0 Scope The Control of Hazardous Energy (Lockout/Tagout) Program applies to personnel (inclusive of UIC employees and contractors,) who are exposed to, work with, or supervise operations involving work with hazardous energies at the UIC main campus, Regional campuses, and related UIC facilities and operations. 3.0 Definitions The following terms used in this Control of Hazardous Energy (Lockout/Tagout) Program are defined as follows: Affected employee - An employee whose job requires him/her to operate or use a machine or equipment on which servicing, or maintenance is being performed under lockout or tagout, or whose job requires him/her to work in an area in which such servicing or maintenance is being performed. (i.e. UIC Painters and Locksmiths). Authorized employee - A person designated by the Chief or Foreman of the department who locks out or tags out machines or equipment to perform servicing or maintenance on that machine or equipment (i.e. electricians, operating engineers, pipefitters). Capable of being locked out - An energy isolating device is capable of being locked out if it has a hasp or other means of attachment to which, or through which, a lock can be affixed, or it has a locking mechanism built into it. Other energy isolating devices are capable of being locked out, if lockout can be achieved without the need to dismantle, rebuild, or replace the energy isolating device or permanently alter its energy control capability. Employees- Persons that work as an employee and receive regular and planned compensation from UIC for tasks performed. Energized - Connected to an energy source or containing residual or stored energy. Energy isolating device - A mechanical device that physically prevents the transmission or release of energy, including but not limited to the following: A manually operated electrical circuit breaker; a disconnect switch; a manually operated switch by which the conductors of a circuit can be 3

4 disconnected from all ungrounded supply conductors, and, in addition, no pole can be operated independently; a line valve; a block; and any similar device used to block or isolate energy. [Push buttons, selector switches and other control circuit type devices are not energy isolating devices.] Energy source - Any source of electrical, mechanical, hydraulic, pneumatic, chemical, thermal, or other energy. Hot tap - A procedure used in the repair, maintenance and services activities which involves welding on a piece of equipment (pipelines, vessels, or tanks) under pressure, in order to install connections or appurtenances. It is commonly used to replace or add sections of pipeline without the interruption of service for air, gas, water, steam, and petrochemical distribution systems. Lockout (LOTO) - The placement of a lockout device on an energy isolating device, in accordance with an established procedure, ensuring that the energy isolating device and the equipment being controlled cannot be operated until the lockout device is removed. Lockout device - A device that utilizes a positive means such as a lock, either key or combination type, to hold an energy isolating device in the safe position and prevent the energizing of a machine or equipment. Included are blank flanges and bolted slip blinds. Magnitude The total amount of energy present. Normal production operations - The utilization of a machine or equipment to perform its intended production function. Other Employees- Persons whose work activities are or may be in an area where energy control procedures may be utilized. Other employees must be instructed about the procedure and about the prohibition relating to attempts to restart or reenergize machines or equipment that are locked or tagged out. Servicing and/or maintenance - Workplace activities such as constructing, installing, setting up, adjusting, inspecting, modifying, and maintaining and/or servicing machines or equipment. These activities include lubrication, cleaning or unjamming of machines or equipment and adjusting or tool changes, where the employee may be exposed to the unexpected energizing or startup of the equipment or release of hazardous energy. Setting up - Any work performed to prepare a machine or equipment to perform its normal production operation. Tagout - The placement of a tagout device on an energy isolating device, in conjunction with a lockout device where capable of being locked out, to indicate that the energy isolating device and the equipment being controlled may not be operated until the tagout device is removed. 4

5 Tagout device - A prominent warning device, such as a tag and a means of attachment, which can be securely fastened to an energy isolating device in accordance with an established procedure, to indicate that the energy isolating device and the equipment being controlled may not be operated until the tagout device is removed. 4.0 Roles/Responsibilities The University of Illinois at Chicago Administrative Services Senior Management Ensure the University follows applicable Federal and State Occupational Safety and Health regulations regarding the Control of Hazardous Energy 29 CFR Provide needed resources to assure the programs implementation and continuance. Chiefs, Assistant Chiefs, Foremen, Supervisors Attend scheduled LOTO Training Communicate to employees they must meet the requirements of UIC s Control of Hazardous Energy (Lockout/Tagout) Program. Ensure employees receive the required training before allowing employees to conduct hazardous energy control/loto procedures. Ensure retraining is conducted whenever an employee demonstrates an inadequacy of knowledge, or if there is a change in program or procedure that requires additional training. Assist in completing Equipment Specific LOTO Procedures for required equipment. Review the Equipment Specific LOTO Procedures to ensure their accuracy. Maintain the Equipment Specific LOTO Procedures for your department. Ensure employee access to the most recent version of the Lockout procedure developed. Make lockout tagout procedures available for periodic inspections, safety and health audits or regulating agency inspections. Assist in conducting periodic audits of LOTO procedures. This may be completed during LOTO operations Inform all new employees of the need to follow all posted LOTO procedures. Designate employees as authorized employees and ensure these authorized individuals are provided training on lockout/tagout in their work area at the time of initial assignment, upon addition or changes of new machine(s), equipment processes, departmental procedures, or this program. Provide each authorized employee their own LOTO lock and key and show them the location of the safety station for their work area containing specialized LOTO supplies. Maintain appropriate levels of LOTO supplies and notify EHSO if assistance is needed. Provide authorization for emergency/abandoned lock removal. 5

6 Project Managers/UIC Personnel Hiring External Contractors UIC employees responsible for procuring contractors that will conduct work that may involve de energization of energy sources on UIC property are responsible for the following: Ensuring the Chief Engineer of the area where the work is going to be performed is informed of the following: the date and time of the work to be performed energy sources needing to be locked out. Informing contractors, they are NOT to lock-out energy sources (i.e. pneumatic, electrical, hydraulic) in any UIC building. This communication can be made via distribution of the UIC Contractor Safety Guidelines. Contractors are to notify the Project Manager or appropriate UIC Representative of the equipment needing to be locked out. An authorized UIC employee will lock out the energy source. Contractors should then apply their own lock(s) and tag(s). Environmental Health and Safety Office (EHSO) Develop and provide overall administrative support for the Control of Hazardous Energy (Lockout/Tagout) Program, including interpretation of the regulation. Review the Control of Hazardous Energy (Lockout/Tagout) Program on an as needed basis based on regulatory and UIC internal needs. Provide guidance for equipment-specific lockout/tagout (LOTO) procedures. Assist UIC department personnel in selecting appropriate equipment for LOTO, as requested. Manage and schedule UIC Control of Hazardous Energy Control (Lockout/Tagout) safety training. Ensure periodic audits are conducted of lockout-tagout program and adherence to the program. Maintain a master file of documentation and records associated with the Lockout/Tagout Program, including but not limited to: Training records; Equipment Specific Lockout/Tagout procedures Periodic Audit Forms. Authorized Employees Attend training and follow the requirements of the Lockout/ Tagout regulations, UIC Lockout/Tagout, and any departmental procedures on energy hazards and control. Know and understand the associated energy, related hazards, and procedures for the equipment that you will be service and/or maintain. Inform affected employees and other employees who work in the area where LOTO procedures are to be utilized they must not attempt to restart or reenergize the affected 6

7 equipment/machines Complete lockout/tagout on equipment, using required LOTO equipment to control hazardous energy sources. Follow departmental and Equipment Specific LOTO procedures. Use appropriate Personnel Protective Equipment where hazards may be present during lockout/tagout and servicing of equipment. Affected Employees: Attend training on the purpose and use of hazardous energy control/loto procedures. Do not attempt to restart or reenergize machinery when informed and labeled by an authorized employee that equipment/machines have been de-energized and for LOTO purposes. Inform your supervisor of accidents, conditions, or work practices relating to Lockout/ Tagout as you believe to be a hazard to your health or to the health of other individuals. Contractors/Vendors Must have a written safety program that complies with the requirements of OSHA CFR (lock-out/tag-out) and submit this document for review. Provide expected dates/times for work requiring de-energization of energy source(s) to the Project Manager or UIC Representative. Must have employees trained to meet the requirements of the lock-out/tag-out standard and provide proof of this training. Contractor must supply their own lock and tag. Contractors are NOT to lock-out energy sources (i.e. pneumatic, electrical, hydraulic) in any UIC buildings. They are to notify the Project Manager or appropriate UIC Representative of the equipment needing to be locked out. An authorized UIC employee will lock out the energy source. Contractors should then apply their own lock(s) and tag(s). Never operate equipment that has a DANGER-DO NOT OPERATE tag or lock on it. Never remove Energy Securing Devices or Hold Tags. 5.0 Training LOTO Training is a two-step process for Authorized Employees, and a one step process for affected employees (i.e. painters, most carpenters, locksmiths) and other personnel. UIC affected employees are to receive classroom or online training. Documentation of training is required. Authorized Employee Training Contents: The location and availability of the UIC Control of Hazardous Energy (Lockout/Tagout) Program. Requirements of the UIC Hazardous Energy Control Program and the Code of Federal Regulations OSHA Recognition of applicable hazardous energy sources, the type and magnitude of the energy 7

8 available in the workplace, and the methods and means necessary for energy isolation and control. Types of Energy Isolating devices, locks, tags and other applicable equipment of LOTO operations. Purpose and use of the energy control procedure and related steps of the process. Limitations of Tagout Systems Location and use of Equipment Specific Lockout/Tagout Procedures for their department In addition, on an annual basis, the Employee On-The-Job Training checklist will be used for annual employee hands-on review (Appendix C) Affected and Other Employees The location and availability of the UIC Control of Hazardous Energy (Lockout/Tagout) Program. Requirements of the UIC Control of Hazardous Energy Program and the Code of Federal Regulations OSHA as it relates to Affected and Other employees. Attempting to restart or re-energize machines or equipment which are locked out or tagged out is strictly prohibited. Employee Retraining Retraining shall reestablish employee proficiency and introduce new or revised control methods and procedures as related to one or more of the items below; Whenever a periodic inspection or work practices reveals there are deviations from, or inadequacies in the employee s knowledge, use of the energy control procedures, or regulatory requirements of the Control of Hazardous Energy program requirements. Change in job assignment, change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedure. Authorized employees must be retrained if there are changes to OSHA regulation 29 CFR or the UIC Control of Hazardous Energy program. Affected employees must be reminded on an annual basis via , on-line, or classroom training not to remove another employee s lock or tag. 6.0 Control of Hazardous Energy Procedures and Process UIC personnel are only permitted to work on de-energized equipment following the UIC Control of Hazardous Energy (Lockout/Tagout) Program. Documented Equipment Specific Lockout Tagout Procedures are to be developed initially by an authorized employee and EHSO representative. The procedures must be approved by a Chief or Assistant Chief before they are posted on the equipment. A new procedure must be updated and approved by the department when there are process changes that impact the current LOTO procedure. These procedures must be maintained, made available, and accessible for the authorized employees 8

9 and auditors of this program. Appendix A Equipment Specific Lockout Tagout Procedure Template must be used. Equipment-Specific Lockout-Tagout Procedures are required for equipment or processes if one or more of the following are identified: The equipment or process has a potential for stored, residual, or re-accumulation of energy. The equipment or process has more than one energy source. If the equipment or process has one energy source, the isolation and locking out of that energy source will NOT completely de-energize and deactivate the unit. The equipment or process is NOT isolated from its energy source and locked out during servicing or maintenance. (Tagout Operations require documented procedures as they are not locked) A single lockout device will NOT achieve a locked-out condition. The lockout device is NOT under the exclusive control of an Authorized Employee. The service or maintenance creates hazards for other personnel. There have been accidents involving the unexpected activation or re-energization of the unit. EXCEPTION - Minor tool changes and adjustments, and other minor servicing activities, which take place during normal production operations, are not covered by this program if they are routine, repetitive, and integral to the use of the equipment for production, provided that the work is performed using alternative measures which provide effective protection Equipment Specific Lock-Out/ Tag-Out Procedures must include and are not limited to: A specific statement of the intended use/purpose of the procedure. Specific procedural steps for shutting down, isolating, blocking, and securing machines or equipment to control hazardous energy. Specific procedural steps for the placement, removal, and transfer of lockout devices or tagout devices and the responsibility for them; and Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures Additional elements as listed on the UIC Equipment Specific Lockout Tagout Procedure Template In the event a documented Energy Control Procedure does not exist for equipment that will be worked on, an authorized employee can conduct a review of the equipment that is to be Locked and Tagged Out and can develop a procedure in order to service the equipment. This procedure should be reviewed with a supervisor or foremen prior to the work being initiated. In the event the authorized employee needs assistance with the Lockout Procedure, they can contact their supervisor or foreman for help. (See Appendix C) 9

10 Lockout/Tagout Required Process Whenever it is necessary for UIC personnel to be involved in the maintenance or servicing of equipment or machines with a potential exposure to hazardous energy, the following steps must be followed, in order. 1. Notifications 2. Preparing for shutdown 3. Performing shutdown 4. Isolating equipment 5. Discharge of residual energy 6. Locking/tagging devices 7. Energy verification 8. Maintenance/Servicing 9. Restoring power Details on each of these steps is included in the Authorized Employee training. Abandoned Lock/Tag or Removal by Someone other than Authorized Employee Placing the Lock If the authorized employee is not available or the lock has become abandoned, the device may be removed under the direction of the Chief and appropriate Foreman (if applicable), provided that all measures are taken to ensure the following: Verification via phone or text with the authorized employee who applied the lock to confirm he is not at the facility. All reasonable efforts to contact the authorized employee have been made to inform him/her that the lockout or tagout device has been removed. The authorized employee has knowledge that the lock/tag has been removed prior to resuming work at the facility. Temporary Removal of Lockout /Tagout Devices: In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment, or component thereof, the following sequence of actions shall be followed: Clear the machine or equipment or tools and materials Remove employees from the machine or equipment area. Remove the lockout or tagout devices. Energize and proceed with testing or positioning De-energize all systems and reapply energy control measures per the documented LOTO procedure to continue the servicing and/or maintenance. 10

11 Energy Control Procedures Energy Control Procedures are developed to outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to: A specific statement on the intended use of the procedure; Specific procedural steps for shutting down, isolating, blocking, and securing machines or equipment to control hazardous energy; Specific procedural steps for the placement, removal, and transfer of lockout and tagout devices and the responsibility for them; and Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. Group Lockout/Tagout: When more than one trade or an external contractor works on a machine that needs to be locked out, each employee and contractor (if applicable) must put their lock and tag in or on the group lock box following the procedure below. Group lock boxes are provided at each safety station. Lock boxes are a convenient way to manage multiple individuals working on a piece of equipment that is locked out. An authorized UIC employee must take primary responsibility for all UIC employees and contractors working under the protection of a group lockout or tagout device (i.e. the Operating Engineer would lock-out all required energy sources). The employee taking primary responsibility for the lockout is to lock out each energy source and then place the keys to the locks in the lockout box. Each authorized employee working on the equipment shall affix a personal lock and tag to the group lockbox when he or she begins work, and shall remove those devices when he or she stops working on the equipment being serviced. When all authorized employees remove their personal lock and tag, the UIC employee who took responsibility for the group lockout may then remove the key(s) from the box and restore energy to the equipment. 12 Shift or Personnel Changes: The Chief or Foreman is to assure specific procedures are utilized during shift or personnel changes to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout device protection between off-going and oncoming employees. If an employee has not finished a job at the end of the shift, he/she may leave their lock and tag on the equipment. However, they should inform their Chief or Foreman they are leaving their lock and tag on the piece of equipment and confirm they will be assigned to finish the job the next day. If the same employee cannot be assigned to finish the job the next day, he must remove his lock the next day in the presence of the employee now assigned to the job. The new employee assigned to the job will then apply their own lock and tag. Lockout Locking Device Requirements: Durable: Lockout devices shall withstand the environment to which they are exposed for the maximum period of time that exposure is expected. Standardized: Lockout devices shall be standardized within the department in at least one of the following criteria: color; shape; or size. Locks used for lockout at UIC should be red in color. Substantial: Lockout devices shall be substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools. Identifiable: Lockout devices shall indicate the identity of the employee applying the device(s) The Authorized Employee has the only key for his or her lock (No Duplicates) Use of Locks Chiefs, Assistant Chiefs, Foreman and Authorized Employees shall maintain a supply of lockout devices specific for their trade and area of expertise. Locks that are used for lockout operations are to be kept with the employee and at the departmental designated safety station(s) and used only for lockout operations. Locks shall remain on equipment during the duration of the service and maintenance operations except for shift changes and troubleshooting. Locks are required to be applied and removed by the authorized employee who applied the lock. Each authorized employee must have his/her own lock. Tagout Tag Equipment Requirements Tagout devices print and format shall be standardized for each department. Tags are to have the authorized employee s contact information identified on the tag. 12

13 Tagout devices shall be constructed and printed so that exposure to weather conditions or wet and damp locations will not cause the tag to deteriorate or the message on the tag to become illegible. Tags shall not deteriorate when used in corrosive environments such as areas where acid and alkali chemicals are handled and stored. Tagout devices shall warn against hazardous conditions if the machine or equipment is energized and shall include a statement such as: Do Not Start, Do Not Open, Do Not Close, Do Not Energize, or Do Not Operate. Tagout devices, including their means of attachment, shall be substantial enough to prevent inadvertent or accidental removal. Tagout device attachment means shall be of a non-reusable type, attachable by hand, selflocking, and non-releasable with a minimum unlocking strength of no less than 50 pounds and having the general design and basic characteristics of being at least equivalent to a one-piece, all environment-tolerant nylon cable tie. Tag Out Operations Requirements An approved tag will be placed on the hasp of each padlock used for the lockout. Tags are placed or applied by the authorized employee conducting the LOTO operation to notify affected and other personnel of the inherent danger and including but not limited to the following information: Authorized employee name Date and Time of Lockout Operations Authorized employee cell phone Tagout Operation Only (Used in Lieu of a Lockout/Tagout Operation) NOTE: WHERE THE EQUIPMENT CAN ACCEPT A LOCK, A TAG MUST BE UTILIZED. A tagout alone must be approved by the Chief or Foreman, or their designee of the department and the following requirements are to be met: Procedure(s) are written, available, communicated, and used that provide equal protection as a lockout operation against potential start- up of energy and for the authorized employee(s) conducting the LOTO operations. The tagout device is attached at the same location that the lockout device would have been attached, and the authorized employee and supervisor demonstrate that the tagout procedure will provide a level of safety, equivalent to that obtained by using a lockout. The authorized employee and supervisor must demonstrate full compliance with all tagoutrelated provisions of the control of hazardous energy standard, AND provide one of the following or equivalent safety measures: o removal of an isolating circuit element, 13

14 o blocking of a controlling switch, o opening of an extra disconnecting device, o or the removal of a valve handle to reduce the likelihood of inadvertent energization. Periodic Audits The Chief, Assistant Chief, Foreman, or Supervisor is to ensure authorized employees participate in the annual audits of LOTO procedures coordinated by EHSO. The purpose of the audits is to ensure the procedures are still accurate and are well understood by employees. The audit shall identify the equipment or process on which the energy control procedure was being utilized, the date of the audit, the employees included in the audit, and the designated authorized employee performing the audit. The point of this audit is to ensure the employee is able to properly lockout an energy source and the procedure is still accurate. The LOTO Audit form in Appendix B can be used for these audits. The designated personnel completing the audits are to comply with and complete the following: The procedure review is to be conducted by a person knowledgeable about the lockout/tagout standard other than the one(s) utilizing the energy control procedure being inspected. Complete review on an annual basis to assure the requirements of the procedure are being followed. Like equipment with similar energy sources can be grouped and counted as one inspection. Document corrections or deviations identified during the audit on the procedure audit form. Provide documented corrections or deviations to the Chief, Assistant Chief, Foreman, or Supervisor for review, approval, and notification to authorized employee. Communicate to the Chief, Assistant Chief, Foreman, or Supervisor and EHSO the need for Onthe-Job Training to retrain authorized and affected employees (if applicable). Lockout Procedures audits include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. Annual procedure audits are to be filed by the Environmental Health and Safety Office. 14

15 Recordkeeping Recordkeeping shall be performed by individual departments that conduct or have responsibility for oversight of LOTO. The following records are to be kept: Records to be Filed Retention Department Departmental Authorized Employee list Equipment Specific Lockout Tagout Procedures Current Active Authorized Employees Length of Equipment operation Individual and EHSO Lockout Tagout Audit Forms 3 years EHSO Department Individual Dept. responsible for equipment and EHSO will keep the master electronic file of each procedure Formal classroom training for Authorized, Affected, and Other employees Length of employee s employment plus 3 years EHSO Program Enforcement Failure to follow the UIC Lockout/Tagout program may result in life threatening or serious injury situations. Failure to lockout or tagout or otherwise not follow the Lockout/Tagout procedures will result in disciplinary action up to and including termination. References 29 CFR OSHA Control of Hazardous Energy Electrical Safe Work Practices Program Review The Control of Hazardous Energy (Lockout/Tagout) Program will be periodically reviewed and updated by the UIC Environmental Safety and Health department to reflect UIC process and procedure changes and external regulatory changes that impact its content. Changes will be indicated upon program revisions made. Changes will be communicated to affected UIC departments. 15
