8 Multi-Gas Monitors An H2S monitor is not enough. Conduct atmosphere hazard assessment and workers are aware of the hazards. Need to measure Oxygen and Flammable atmosphere. Ensure the use of calibrated multi-gas monitors by all workers who go on site. Ensure workers know what the monitor is measuring and trained Respond to all alarms.
9 Ground Disturbance Addresses Operators, Excavators & Locators Utilize a written ground disturbance/crossing permit program. Update plot plan and 811 system. Utilize tracers or markers. Isolate and/or remove unused or decommissioned lines Call 811 before you dig. Ensure contractors/excavators are experienced in disturbing ground Notify owners, 811, & state regulators of damage - Do not cover damages

11 Sudden Uncontrolled Pressure Releases AESC members noticed several injuries over the years associated with these events Where stored pressure (natural gas, water, air, chemical, shock wave, etc.) has either struck the employee, and/or the debris (valves, piece of pipe, metal fragments, pigs, hoses, plugs, etc.) has struck the employee as a result of the work being done: equipment failure, banging or pounding on pressurized equipment, improper procedures, lack of LOTO, etc.
12 Sudden Uncontrolled Pressure Not selected Releases explosions that resulted in a fire or a well blowout Majority were classified as struck-by but there were a few chemical burns and falls resulting from being struck by the metal or pressure.
