An undesired event often caused by unsafe action and/or unsafe conditions that has potential for physical harm to persons, damage to property, product, infrastructure or business interruption. 12. Incidents thus costs industry much more than is realised and for that reason effort should be made to investigate incidents in detail and to take action to prevent re-occurrence and similar incidents. 8.
