2 Venous Blood Collection Passport (VCP) Student name Instructions: You will use this Passport to record the number of live collections performed in the Simulated Clinic under supervision. It is your responsibility to ensure that you obtain as many collections as possible during your training period. It is your responsibility to use this record only for the assessment and not to be used for any other purposes The completion of this assessment requires observation of your performance during venous blood collections. During the collection procedures, you assessor will be observing the skills in the observation checklist attached at the end of this passport. Your Assessor will inform you when you are ready for assessment throughout the course after approximately 10 to 15 bleeds. Task: You must perform venous blood collection from at least 20 different people comprising adults of varied ages, with at least 5 of 20 collections directly observed in the workplace. The evidence gathered in this Passport will demonstrate your proactive approach in obtaining as much experience as possible to enhance your learning journey. It is also documented proof of the collections you have performed and is used as evidence for deeming you competent to successfully complete this course and obtain the final qualification. ***Venous blood collection techniques will be performed using the following equipment: ES - Evacuated systems (Minimum of 10 collects) WIS - Winged infusion sets (Minimum of 1 collect) * With collection from Basilic Vein (B), Cephalic Vein (C), Median Cubital Vein (MC) ** 18-25(A), 26-30(B), 30-40(C), 40-50(D), 50-60(E), 60+(F) Record Of Venous Blood Collections Assessor Use Only Date (dd/mm/yy) Bleed # **Age of the volunteer ***Equipment used *Type of vein bled (B, MC,C) Demonstrated effective communication Completed reasonable time Followed procedure, Infection Control and Safety Guideline. of attempts to gain venous blood (Max 2) Comments Assessor initials 1 2 3

4 Performance Checklists During the venous blood collections you are required to continuously demonstrate the skills identified in the checklists. Assessor Use Only 1. HLTINF001 Comply with infection prevention and control policies and procedures Hand washing technique and cough etiquette a) Removed any jewellery and watch from hands b) Finger nails maintained in a neat, clean and short condition c) Wore enclosed shoes d) Covered any cuts/abrasions with semi-permeable tape e) Wet hands thoroughly f) Applied sufficient amount of liquid soap g) Rubbed hands with friction for minimum 15 seconds h) Washed all parts of hands and wrists i) Rinsed hands thoroughly j) Dried hands thoroughly k) Turned off tap with clean paper towel or used elbow to turn off clinical tap l) Used a tissue to cover nose and mouth when sneezing/coughing; disposed tissue into a waste bin, washed hands with soap and water or alcohol gel for at least 15 seconds Feedback to candidate: t

6 c) Established any risk of complications by asking the patient questions about allergies and any other issues in previous blood collection d) Interpreted clinical request form and confirmed any pre-test criteria e) Performed alcohol based hand rub before touching the patient, after touching the patient, before the procedure, after the procedure and after touching the patient s surroundings f) Correctly explained the procedure to the client and obtained consent g) identified whether assistance was needed and sought help if required h) identified all required equipment, retrieved required items from trolley/storage area and positioned equipment for ease of access and safe working practices i) Positioned client in a comfortable position j) Applied tourniquet on patients arms about 3 fingers width above the antecubital fossa k) Palpated all the veins and hallmarked it and loosened tourniquet if it took more than 2 minutes l) Cleaned the venipuncture site with 70 % alcohol wipes m) Assembled all the equipment, applied PPE (gloves and goggles) and tightened tourniquet n) Pulled the skin taut to anchor the vein and inserted needle at an appropriate angle depending on the depth of the vein and the needle size o) Collected correct volume of blood in the appropriate collection tubes in the order of draw and removed tourniquet as soon as the blood starts to flow into the 1st tube p) Withdrew the needle safely after collected the required volume and applied direct pressure using cotton wool q) Disposed the needle immediately and safely into sharps container r) Inverted the collection tubes within timeframe required as recommended by the manufacturer t
