2 OPERATIONAL SIGN-OFF PACKET To: ALL PERSONNEL The purpose of this Operational Sign-off Packet is to provide all-line personnel with a guide to develop and maintain personal skills necessary to perform in a satisfactory manner in both routine and emergency situations. An assistance asked for by the personnel in gaining this knowledge and completing the Operational Sign-offs should be readily given by ALL approved trainers. The personnel will have an approved trainer sign off the listed subjects and skills. The questions asked, either oral or written, should satisfy them that the personnel knows the subject and/or skills without assistance or error, and they must show proficiency. The approved trainer SHALL ONLY sign off that item at that time. The topics need not be performed as arranged on the sheet, but as the member feels he/she is ready for evaluation and demonstrate proficiency. Captains and Engineers should feel free to use the facilities for practice or evaluation whenever needed. Approved Trainers include: Captain Al Kaiser, F/F John Howell II, F/F Ron Forica, Battalion Chief Brian Brown, Deputy Chief Brett Waddell Copies of this Sign-off shall be forwarded to the Training Division for documentation.

5 OPERATIONAL TASKBOOK To: All Apparatus operators 2/07 The purpose of this Operational Taskbook is to provide the water tender operator with a guide to develop and maintain personal skills necessary to perform in a satisfactory manner at both routine and emergency situations. Any assistance asked in gaining this knowledge and completing the Operational Sign- Offs should be readily given by the Captains and Engineers assigned to the station that houses the water tender. Training captains can also be utilized. The prospective water tender operator will have an Approved trainer sign off the listed subjects and skills. The questions asked, either oral or written, should satisfy them that the apparatus operator knows the subject and/or skills without assistance or error. The approved trainer SHALL ONLY sign off that item at that time. Once the entire taskbook has been completed, the approved trainer shall sign this page as verification of completeness. The prospective water tender operator will then submit their completed taskbook to the Training Division where it will be reviewed by training division staff. If it is complete, the training division will sign it off as completed and update their status with staffing. To: Station Captains This taskbook is provided to the prospective water tender operator. It has to be initiated by the training division before they can take it. Please verify that there is a signature below from the training division before signing off items. If there is no signature, do NOT sign off tasks until it is obtained. Once the entire taskbook is completed, the approved trainer must sign below verifying the packet is completed. The prospective water tender operator will then take their taskbook to the training division for final review. Once verification is complete, staffing will be notified as to the member s new status. Feel free to use the Training Tower and facilities for practices or evaluations whenever needed. The task shall be signed off only after you feel competent in the operator s skills. Multiple skill performances may be required. Again, this Operational Taskbook is for the apparatus operators too display their knowledge and skills.
