O 9 DATE APPLICABLE TO OIL TANKER VESSELS SUBJECT: Revision to Cargo Operation Manual Oil Tanker & Company Form Please note the following revision to the Cargo Operation Manual Oil Tanker & Company Form: Cargo Operation Manual Oil Tanker 1. SECTION OPERATION OF VALVES PAGE 6 3 rd Paragraph add below as last text: Manual hydraulic pump should be kept stand-by in pump-room and at manifold for emergency use and all crew to be trained in its use VOYAGE ORDERS & CARGO STOWAGE PLAN PAGE 12 5 th Paragraph: On oil tankers generally tanks are loaded to maximum 98% volume. At no time vessel shall cross this maximum barrier replace with Nominated cargo quantity against available volume basis maximum temperature expected. Maximum stow shall not exceed 97.8% by volume of the tank capacity 2. SECTION GENERAL PAGE 4 Add new section Dealing with Emergencies in Terminal Dealing with emergencies in terminal 1. Stop cargo transfer operation and raise general alarm. 2. Inform the terminal that all operations are ceased and follow the Contingency plan of the terminal for the applicable type of emergency. 3. Prepare engines for immediate use. 4. Carry out notification to head office, Owners, Charterers and other parties as per Contingency notification procedure. 1

Evaluate whether safety of the vessel is endangered and prepare to case off from the terminal or follow evacuation plan, as per terminal instructions LOADING PRECAUTIONS PAGE 5 New add section Cargo Topping up precautions To include following topping up precautions. - Senior deck officer must be present in CCR, well before commencing top-up. - Reduce cargo loading rate to pre-agreed cargo topping up rate. - Restrict number of cargo tanks to be topped up simultaneously, and topping up more than one cargo tank should be avoided. - In addition to close monitoring of remote gauges, such readout should be compared with local independent monitoring such as UTI and/or MMC tapes. - Monitoring of Topped-up cargo tanks, post top-up, should be recorded in Port Log or Hourly Record. 3. SECTION DISCHARGING PRECAUTIONS PAGE 8 Add this word as last paragraph: For Homogeneous cargo discharge, lower the tank levels to below 95% before commencing full discharge. 4. SECTION LOADED PASSAGE PAGE 7 Replace with as below: It is important to maintain a positive pressure in cargo tanks and pressure should never be allowed to fall below 100 mmwg when in an inert condition. Oxygen content shall be checked atleast two times a week and records maintained onboard. Following safety checks should be carried out on daily basis except when weather conditions render it too dangerous to go onto deck; 2

3 a. Oxygen content of cargo tanks are to be measured atleast twice a week. b. Check the water level in water loops installed in pipework for gas, water or pressure transducers, to prevent the backflow of hydrocarbon gases into gas-safe spaces. c. In cold weather, ensure that the arrangements to prevent the freezing of sealing water in the deck seals, pressure/ vacuum breakers etc. are in order. d. Tank low pressure alarms should be tested at an interval not exceeding one week. 5.9 PURGING PAGE 12 First paragraph - 1.4% replace with 2%. 5. SECTION CARGO TANK OPENINGS AND OTHER FITTINGS PAGE 4 Point 4 replace as below: 4. Pressurize tank using compressed air or inert gas CARGO PIPELINES AND VALVES TEST PAGE 5 1 st Paragraph replace as below: Cargo pipelines and valves should be pressure tested hydrostatically to 100% of their rated working pressure (MAWP) and be marked with the date of test and the pressure. Tests are to be carried out once in Twelve (12) months. The head office is to be informed if the duration extending beyond Twelve months. Furthermore, cargo pipelines and valves should be hydrostatically tested to 1.5 times their rated working pressure (MAWP) at least twice within any five year period. 6. SECTION DISCHARGE OF HOMOGENEOUS CARGOES (PARALLEL PUMPING) PAGE 6 Replace existing guidance with below; 3

4 - Incase of parallel operation of Framo pumps there is a risk of back-flow of cargo into the incoming pump. - The incoming pump should have the hydraulic pressure higher than the running pump (normally 20 bars). This adjusted subsequently to get the desired flow rate from each tank. - The level of the tanks should be below alarm level (95%) so that any back-flow can be detected. - Pump capacity should be controlled by the pump hydraulic pressure, not by throttling the cargo discharge valve or any other valve in cargo piping system. - Generally, we recommend to run as many pumps in parallel as practical, at a reduced hydraulic pressure, rather than just a few pumps at maximum hydraulic pressure. - Monitor the ullage very closely of the incoming tank till it is confirmed that the level is dropping. - If the RPM of the pump is reduced below the parallel pump, part of the cargo from the pump with higher RPM will flow into the pump with lower RPM. 9 Loading / Discharging Operation :- During transfer operations, The terminal and vessel will monitor the cargo transfer rate to verify it does not exceed the agreed max. allowable cargo rate and exchange cargo figures received / transferred. The locations of the shutoff valves or other isolation devices are posted in the Cargo Control Room. d. Procedure for operating the Emergency Shutdown and Communication e. He will check list and trim of the vessel and correct as required. He will keep chief officer informed of any deviation observed to the cargo transfer procedure. He will carry out repeated checks as mentioned in Ship-Shore Safety Checklist. He will hourly monitor cargo transfer rate, stresses and record accordingly. Deck Duty Personnel :- They will assist Chief Officer and Duty officer in executing cargo transfer. They will assist surveyor in taking a sample and ullaging. They will rig required SOPEP/FFA equipment as required for the cargo transfer. Will attend manifold, tend gangway and moorings regularly. 15 10. Safety Information / Requirements / Guidance Notes :- a. All crew members to be briefed about proper cargo handling procedures as required while handing cargo. All officers and crew members must read and familiarize themselves with MSDS of chemicals onboard. (See MSDS board) b. Carry out Pre-Arrival Checks as per checklist and ensure all items are in order. c. Gangway and moorings to be tended at regular interval. d. Keep eye wash and decontamination shower on for ready use. e. Ship-Shore checklist to be strictly complied with at all the times. Duty officers must carry out Repeated Checks as listed in Ship-Shore checklist at agreed interval and log down. f. No open sampling is allowed. Duty officer will accompany surveyor for gauging and sampling. g. Duty officer to calculate hourly cargo transfer rate/sf/bm and maintain a record of same in hourly rate log. h. PV valves to be monitored closely for their correct operation. i. Keep strict ISPS watch and comply with all company/port regulations. j.
