Format subsequent AHAs as amendments to the APP. The analysis should be used during daily inspections to ensure the implementation and effectiveness of the activity's safety and health controls. Be aware of surroundings Ware proper PPE Work boots, Hardhat, Safety Eye Ware. L 2b. Struck by Delivery Vehicle 2b. Ensure warning and backup alarms are working properly High visibility apparel shall be worn by all workers Individuals not involved in operation shall stay clear of area 3- Unloading of Ductwork and Accessories 3a. Slips, trips, and falls 3a. aintain a clean and clear area unloading Identify and remove obstructions around the work area Wear proper PPE: Hardhat, proper work boots (Toe Protection) laced properly. 3b. Lacerations,Punctures and Abrasions 3b. Wear proper PPE: Leather Work Gloves, Hardhats Wear high visibility apparel Concentrate on your work 3c. Being struck by materials 3c.. Check shipped loads packing configuration Be aware of load slipping or shifting during unloading Worker not involved unloading vehicles shall stay clear. No one is permitted to stand under a load. No one is permitted to stand on, or be lowered on a load during any lifting operations 3d. Back Irritation / uscle Strain 3d. Have someone help when carrying heavier loads Use a two wheel dolly for larger items Use a pallet jack to move heavier loads Use proper lifting techniques.
3 3-Unloading of Ductwork and Accessories. ( Continued) 3e. Hand Injury / Pinch Points 3e. Wear appropriate PPE ( Work Gloves ) Be aware of surroundings and positioning of loads and equipment. 3f. Operation of Equipment 3f. Before equipment is place into service a onetime inspection shall be done. The results of the inspection shall be furnished to SSHO. Equipment inspection shall be performed daily by the CP. Equipment to be used by a trained/ qualified operator. Operator must be certified if equipment calls for a Certification to Operate ( CTO ) The use of electronic devices is prohibited (Cell Phones, I-pods Headphones) Equipment shall have Load and Weight Charts Equipment shall be equipped with seat belts Workers must have seat belt on at all time. (Idling or in motion) Ensure all motion warning devises work properly Operator to operate equipment. ( NO RIDERS) 4- Separating and staging of Sheet etal Ductwork and Accessories. 4a. Slips, trips and falls. 4a. aintain a clean and clear working area. Identify remove any obstructions from work area. Wear proper PPE: (Work Boots w/ Laces properly tied. Be aware of your surroundings 4b. Lacerations and Abrasions 4b. Wear appropriate PPE: (Work Gloves) 4c. Back Injury / uscle Strain 4c. Use proper lifting techniques Ensure that proper amount of workers are used. Use appropriate equipment on heavier loads

Wear appropriate PPE: (Work Gloves) Be aware of the surroundings Use a tag line to place equipment into position. Be aware of hands and feet positions when lowering and placing materials and equipment in designated area. 4c. Operation of Equipment 4c. Before equipment is placed into service a onetime machine inspection shall be done. The results of inspection shall be furnished to the SSHO. Equipment to be inspected daily Equipment shall be operated by qualified personal. Operator shall be train and certified to operate equipment Operator is prohibited to operate equipment while using electronic devices. (Cell Phones, I-Pods, Ear Phoned devices.) Equipment shall be equipped with seat belts Operator shall have seat belt engaged while operating equipment (Idling or in motion at all times) Equipment shall have Load and Weight Charts Ensure all warning devices works properly when equipment is in use Equipment shall have Instruction anuals 4d. Unclean housekeeping practice. 4d. Stage ductwork and accessories in appropriate manner Clean up packing and equipment crating materials. Sweep and pick up trash and debris daily. 5- Installation of Ductwork. Sheet etal and Accessories 5a. Installing ductwork and accessories hangers. (Concrete, etal Decking) 5a. Appropriate PPE. Wear approved hardhat. Appropriate PPE. Dusk ask when drilling Appropriate PPE. Body Harness and Lintel (Required if work is above 6 AFF) Appropriate PPE. Eye Protection (Wearing safety glasses when drilling)

6 5- Installation of Ductwork Sheet metal and Accessories (Cont.) 5g. Operation of Equipment 5g. Before equipment is place into service a onetime machine inspection shall be done. The results of the inspection be furnished to the SSHO Equipment to be inspected daily by the CP Equipment shall be operated by a trained, experience operator. The use of electronic devices is prohibited ( Cell Phone I-POD, ear phones Radio s) Ensure warning and backup devices work properly Operator shall know the weight of loads being lifted Load Charts shall be visible on equipment. Equipment must be equipped with Operators anual 5h. Unclean Housekeeping Practice 5h. Pipe Vise (Tripod) Note: Once mobilized on site or at the preparatory meeting, all tools and equipment will be added with the specific details, (i.e. make, model, and description). This can only be accomplished once on site to ensure accuracy. Continued periodic inspections shall be made to assure safe operating condition and proper maintenance. 3- Visually inspect ladders for defects and deficiencies before use. Tag and remove from service any ladder that appears compromised. 4- Inspection of first aid kit at least monthly. 5- Inspection of extension cord (Daily) 6- Daily inspection of all Aerial Lifts, Duct Jacks Scissor Lifts and Front End Loaders.
