1 Conducted By: Date of Inspection: Location: Safety and Risk Management Office Biosafety Level 2 (BSL2) Laboratory Safety Inspection Checklist Principal Investigator/Supervisor: General Lab Housekeeping A M S NO NA Comments Laboratory surfaces appear to be cleaned and disinfected with appropriate decontaminant after work is performed. General laboratory, storage areas, and benchtops appear uncluttered and orderly. No slip, trip or fall hazards are present. Aisles and exits are free from obstruction No glass containers are stored on the floor. Trash bags, glass/sharps, biowaste containers are removed when full. Personal Protective Equipment (PPE) A M S NO NA Comments Protective gloves are available, matched to hazards, and in use. Eye protection is available and in use. Lab coats are available, in use, and remain in the lab at all times. Lab coat laundry service is available. Lab personnel are wearing appropriate attire (no shorts, tank tops, open toed shoes, etc.) PPE is not being worn outside of the lab to reduce risk of contamination. 1

2 Emergency Equipment A M S NO NA Comments Eye wash (required) and safety shower are in good working order, inspection dates recorded, covers are in place, and pathway is unobstructed. Personnel are aware of the eyewash/shower location and know how to operate the equipment. Fire extinguishers available, appropriate for type of hazard, inspection dates recorded, and 3 unobstructed clearance. A biological spill kit with appropriate decontaminant is available for minor spills. A chemical spill kit is available for minor spills. Chemical Fume Hood (CFH) and Local Exhaust Ventilation (LEV) A M S NO NA Comments CFH is not being used as storage (excess materials/chemicals). Air foil and rear baffle unobstructed. Sash at or below minimum height. Kept closed when not in use. Access to CFH is unobstructed. Certification is current (annually). _ Biological Safety Cabinet (BSC) A M S NO NA Comments BSC is being used when potential for infectious aerosols/splashes exists. BSC is not used as storage (excess materials), front grill and exhaust filters are unobstructed. Hazardous chemicals/volatiles are not used inside the BSC. Open flames are not used inside the BSC. Vacuum lines have in-line filters and disinfectant traps. HEPA filters in place and replaced as needed. Certification is current (annually). _ 2

3 Gas Cylinders A M S NO NA Comments All cylinders are properly chained and secured. Cylinder caps are in place on reserve cylinders. Cylinders are properly marked with contents and empty cylinders are labeled and stored separately. Incompatible cylinders are stored separately (oxygen/flammable gas). Chemical Storage and Hazard Communication A M S NO NA Comments Inventory (chemical & biological agents) and SDSs are complete and available to lab personnel. Primary and secondary chemical containers are labeled with identity, appropriate hazard warnings, and expiration dates. Peroxide formers are labeled with date opened and removed as hazard waste when expired. Chemical storage is well organized and incompatible materials are segregated (oxidizers/flammables, acids/bases). Liquids and hazardous chemicals are stored below eye level (not on the floor) and are stored in secondary containers. Chemicals are stored in compatible containers and cabinets (acids in nonmetal cabinets, flammables in approved flammable cabinets, refrigeration). Chemical containers are kept closed. Unnecessary, unused, or outdated materials are removed for hazard waste disposal. Flammable liquid storage cabinets are properly labeled, kept closed, and have no materials stored on top of them. 3

4 Waste Management A M S NO NA Comments Chemical waste containers are labeled with contents, date, and generator name Chemical waste containers are kept closed to prevent off-gassing discharge. Chemical wastes are not accumulated for longer than 60 days in the class lab or 6 months in research areas. Secondary containment is used to store waste collection bottles. Syringes and other sharp waste are disposed of in biohazard sharps container. Autoclave is available to decontaminate biowaste before disposal. Clear, autoclave bags are used. Regulated biowaste is collected in red biohazard bags and stored in the biowaste accumulation area. General Practice and Procedures A M S NO NA Comments Food & household products used in the laboratory (baking soda, vinegar, corn syrup, tin foil, etc.) are labeled for Lab Use Only. Household appliances (refrigerators, microwaves, blenders, grinders, etc.) are labeled for Lab Use Only. No evidence of eating, drinking, smoking, applying cosmetics, or mouth pipetting inside the lab space. Laboratory design includes non-porous furniture, no carpeting, and a handwashing sink near the exit. Only needle-locking syringes or disposable syringe-needle units are used. Proper signage on the lab entry door and equipment for any specific hazards (UV, Laser, Radiation, Biohazard, etc.). BSL 2 sign must indicate agents in use and PI contact information on the door. Lab doors are kept closed at all times and securely locked when BSL-2 work is in progress and when the lab is unoccupied. 4

5 Biosafety Manual, BSL-2 SOP, Lab Specific CHP, CHP, and other applicable SOPs are available to lab personnel and training is documented. BBP ECP & HBV Vaccination Program for labs working with human derived materials (blood, body fluids, unfixed tissues, etc.). Annual BBP training is documented. Vivid online safety training for lab personnel is current. rdna research is registered with IBC Animal (vertebrate) research is registered with IACUC Fire Protection and Egress A M S NO NA Comments Sprinkler heads are unobstructed with an 18 clearance below sprinkler heads around entire lab. ( 24 non-sprinklered) All fire-doors are self-closing and kept closed. Lab registration & emergency contact numbers/location details are posted. Emergency evacuation routes are posted in common hallways and the nearest fire alarm indicated. Electrical A M S NO NA Comments Equipment does not have frayed or damaged wiring. Extension cords are not being used as permanent wiring. Power strips are suitable for the load involved and are plugged directly into the outlet (not daisy-chained ). Safety Office Comments 5
