1 CUSTODIAL Safety Inspection Checklist Priority 1 Any condition which is life-threatening, or may cause injury, or permanent disability Priority 2 Any condition which may cause serious, but non-disabling injury Priority 3 Any condition which may cause slight injury or may not meet established standards or regulations, or result in property loss damage to facilities Date ACCEPTABLE? District: Site: Inspector: 1. Are lights free of loose wires, jagged glass, etc. that could cause injuries? 2. Are fire alarms, switches, wires, etc., in good repair with no bare wires hanging loose that could cause injury? 3. Are plumbing fixtures and outlets in good repair? 4. Is bulk storage of dangerous or flammable materials inside main building prohibited? 5. Is electrical equipment (coffee urns, hot plates, lamps, clocks, refrigerator, etc.) grounded? 6. Do fans with blades located less than seven feet above the floor have guards with openings not more than ½ inches? 7. Are ceilings and ceiling-hung fixtures in good repair? 8. Is there a record system of inspections of back-up generators? 9. Are the HVAC exhaust hood, filter, and ducts cleaned regularly? 10. Are waste receptacles kept emptied and clean? 11. Are hand trucks, dollies, or carts properly loaded? 12. Is machinery shut down before cleaning takes place? 13. Are proper lifting techniques being used.

2 ACCEPTABLE? 14. Is the wearing of loose clothing or jewelry around machinery prohibited? 15. Are tools returned to proper storage? 16. Are corrosive and toxic materials stored in closed containers and properly identified and segregated? Are flammable materials stored in an approved flammable storage cabinet? 17. Are overhead storage areas provided with guardrails, toe boards, and adequate aisles? 18. Are materials piled, racked, or stored in a safe manner? 19. Are tools kept sharp, clean, and safe working order? 20. Are equipment control switches easily accessible to operate? 21. Are ladders equipped with safety feet and maintained in good condition? Are metal ladders restricted from use around electrical currents and labeled? 22. Are ladders frequently inspected and those with defects are tagged Dangerous Do Not Use? 23. No cracked or broken pallets in use? 24. Are grinding wheels equipped with hood guards and shields; are tool rests adjusted to within 1/8 inch of wheel? 25. Is portable power- tools properly grounded and cords in good condition? 26. Are ropes, chains, and slings for materials handling in good condition? 27. Is ear protection provided and worn in areas of excessive noise levels (per 85 db)? 28 Is protective equipment and clothing provided for eyes, face, head, and extremities where a danger of flying objects, splashing, and other hazards exist? 29 Is personal protective equipment maintained in sanitary condition?

3 ACCEPTABLE? 30. Are compressed gas cylinders clearly marked as to content? Are protection caps in place except when in use? Are cylinders separated in racks, securely chained to prevent tip-over, and marked by "Full" or "Empty" sign age? 31. Are pesticides and cleaning materials properly stored? 32. Are pesticides and cleaning materials applied indoors reviewed for safety and proper usage prior to use? 33. Are floor drains and drain gratings in good condition? 34. Are glass doors and windows unbroken and in good condition? 35. Is safety or wired glass provided where required? 36. Are doors and locking mechanisms in good conditions and operating properly? 37. Are locked areas immediately accessible in case of fire? 38. Do doors have a clear opening of no less than 32 inches when open? 39. Are doors operable with pressure or strength which could reasonably be expected from disabled persons? 40. Are utility panels and controls well protected from damage or tampering? 41. Is HVAC equipment cleaned, filter changed, and inspected regularly; is a maintenance log maintained? 42. Are boilers and water heaters inspected, permitted as required and in good condition? 43. Are air compressors well-maintained, properly permitted, and in good condition? 44. Are electrical panels and equipment periodically inspected and in good condition? 45. Are mechanical and electrical rooms free of storage?

4 ACCEPTABLE?? 46. Are circuit breakers labeled in accordance with electrical code requirements? 47. Is the facility free of apparent structural deterioration or damage? 48. Is the facility free of any apparent leaks or water damage? 49. Is lighting adequate to eliminate glare and shadows? 50. Are treads in good repair and of nonskid material? 51. Are handrails provided and in good repair? 52. Are landings free of storage materials, equipment, etc.? 53. Are ramps provided with nonskid surface and in good condition? 54. Are elevation differences between floors clearly defined and in good conditions? 55. Are lockers in good condition and secured to the floor or wall with no sharp projections? 56. Is housekeeping adequate? 57. Are desks, chairs, and tables in good repair? 58. Are floors in good condition and not slippery? 59. Are potential hazards protected (e.g., extension cords across aisle covered, electrical equipment properly grounded, Venetian blinds in good repair, etc.) 60. Is inclement weather protection provided (i.e., mats, safety strips, de-icers, etc.)? 61. Are tripping hazards eliminated (e.g., threshold plates in good repair, absence of cords, etc.)?

5 ACCEPTABLE?? 62. Are proper handling techniques utilized (mechanical lifting devices, two-person lift, etc.)? 63. Are wheels on trash carts in good repair? 64. Are chemicals disposed of properly? 65. Are students/staff instructed to dispose of broken glass, aerosols, chemicals and sharp metal objects properly? 66. Is a spill cleanup program in place? 70. Is trash removed at the end of each day for pest control? 71. Are covered metal waste cans used for oily and paint soaked waste?
