5 Report All Incidents This would be any and all injuries as well as near miss incidents! NEAR MISS - Near misses describe incidents where no property was damaged and no personal injury sustained, but where, given a slight shift in time or position, damage and/or injury easily could have occurred.
6 Do a 360 degree walk around your vehicle before making a move! Are there objects you cannot see from the driver s seat? Think about your swing radius. Is there room to pass safely? Be aware of your surroundings and equipment before making a move.

8 Procedure: SP Safety Whips, Flashing Lights and Reflective Gear 1.1. Vehicle Requirements All trucks and SUVs must have a working FLASHING LIGHT whenever going down a ramp to enter THE PIT All light vehicles (1.5 ton or smaller) when entering the Mine and Mine Services area must have a safety whip with reflective flag that is at least 42 above the cab on the vehicle.

10 Safety Policy: SP 500 Mobile Equipment Operating Rules Pre-shift Start-up Inspections 4.1. Pre-shift inspections shall be completed prior to use of Powered Mobile equipment. (Inspection Sheets R drive/forms/mobile Equipment Inspection Forms.) 4.2. Powered Mobile equipment utilized on the Aurora facility shall have a pre- shift start up inspection prior to use on each shift The pre-shift inspection shall remain in the cab or on the Powered Mobile Equipment for the shift of operation The pre-shift inspection for Heavy equipment that may be subject to an engulfment scenario shall have a check off that an Escape Hand Tool is mounted in place and available to the operator. (See section ) 4.5. Change in operator during the shift requires the current operator to conduct a review of the pre-shift inspection and initial that it is correct and applicable Inspection forms for mobile equipment that is not designed to transport items on its flat surfaces must include a question whether the mobile equipment surfaces are free on any gear, equipment, tools, etc before using the equipment. 11 Safety Policy: SP 500 Mobile Equipment Operating Rules 4.7. Mobile equipment shall be provided with: Adequate brakes Mobile equipment(including pickup trucks) shall be provided with audible warning devices when the Operator of such equipment has an obstructed view to the rear; either an automatic reverse signal alarm, which is audible above the surrounding noise level, or, an observer to signal when it is safe to back up ROPS Roll Over Protection Structure All self-propelled equipment shall be equipped with ROPS (Roll Over Protection Structure) and seat belts meeting the requirements of SAE J4c, J140a, J141, J333a, and J386. 12 Safety Policy: SP 500 Mobile Equipment Operating Rules The ROPS complies with the Society of Automotive Engineers, SAE Recommended Practice, Critical Zone-Characteristics and Dimensions for Operations of Construction and Industrial Machinery Each ROPS shall have the following information permanently affixed to the structure: Manufacturer's or fabricator's name and address; ROPS model number, if any; and Make and model numbers of the equipment on which the ROPS is designed to fit Gears, sprockets, chains, drive, head, tail and take-up pulleys, flywheels, couplings, shafts, saw blades, fan inlets, and similar exposed moving machine parts which may be contacted by persons, and which may cause injury to persons, shall be guarded. 13 Safety Policy: SP 500 Mobile Equipment Operating Rules Prior to refueling of mobile equipment begins; If powered mobile equipment is fueled by someone other than the operator, the operator s, master or ignition key must be surrendered to the person performing the fueling. If the operator of the mobile equipment is fueling his own vehicle, the equipment must be powered down. If the mobile equipment is unmanned mobile (e.g., Screeners), the equipment must be powered down and key surrounded to the fueling operator. See SP 512 Delivery of fuels to Mobile Equipment for further guidelines. Caution Fueling Heavy Equipment

16 SP Purpose: The Try, Lock, Try Program prohibits the operation of a valve, switch or other equipment, that would endanger personnel. This procedure must be followed by all PCS employees and Contractors.
17 SP Purpose: Falls represent the leading cause of death in industry representing approximately 36% of all work related fatalities. This Fall Protection Safety Practice provides guidelines, protocol, and safeguards for employees exposed to the potential for a fall greater than 4 feet in height. The content of this Safety Practice meets or exceeds the requirements of 29 CFR 1910, 1926, and 30 CFR Part 56. Stirrups SP Full Body Harnesses shall be equipped with manufacturer approved Trauma suspension strap/relief step safety device to be used in a fall arrest situation to relieve the pressure off both legs.
18 SP Purpose: The purpose of this safety practice is to ensure that all persons entering and conducting work within a permit-required confined space at PotashCorp Aurora, are provided with the maximum protection and in compliance with OSHA Standards for General Industry , Permit-Required Confined Spaces. This Safety Practice provides guidance to employees whose job tasks require them to conduct work within such spaces. This Safety Practice also addresses the exposure of contract employees serving in the roles of Attendant, Authorized Entrant or Entry Supervisor while on the property of PotashCorp Aurora.

20 What can you do to mitigate the hazards?
21 Let s start with a Job Hazard Analysis? A JHA shall be completed for every task. The JHA is turned in to your area management.
22 When should you do a JHA? At the start of each shift, and for each new task performed throughout the shift. Do them with your crew. Many eyes will be able to identify more hazards.
23 How should you do a JHA? Step back and take in the entire job and all hazards associated. What can cause a SIF (serious injury or fatality)? Will the weather change?

25 Hazard ID Card Dept./Contractor your department Employee your name (not payroll number) Supervisor your direct supervisor Location of Hazard Note the Department and specific location of the hazard Hazard Type choose one (try to avoid Unsafe Condition and Other) Description briefly tell what the hazard is Immediate Corrective Action Taken what did you do to mitigate the hazard right then? Suggested Long-Term Corrective Action What will someone do to permanently remove hazard? Type of Control Used for Long-Term Choose one for each action needed
26 Hazard ID Card Criteria for Determining Hazard Level This will be used to identity the hazard level of a Long-Term Corrective Action. These are defined on the inside cover of the book. In Additional Information you can further describe the hazard. If there is a Long-Term Corrective Action, a supervisor will need to assign the action to someone and provide a due date for completion. Write down who is responsible and the due date on the card.
27 Task Training Why do we need task training? Operators that are unfamiliar with equipment can learn how to safely run it. MSHA states that: When Miners (You) are assigned to new work tasks involving new, modified, or unfamiliar equipment you must be trained on it.
28 Fatigue Management A state of physical or mental weariness that results in reduced alertness The result of a lack of adequate sleep Fatigue and falling asleep have been identified as significant contributors to incidents and accidents. It has been estimated that between 10 and 40% of all road accidents involve fatigue.
29 Fatigue is A Serious Safety Hazard Fatigue can be the result of a variety of factors: the body s natural rhythms work schedule type of task work environment non work-related issues

32 PotashCorp Aurora Safety Absolutes 1. Violation of Drug and Alcohol Policy Contract Company will be fined $2,500 per employee. 2. Theft of any PotashCorp or Contract Company property or personal property. 3. Willfully disabling safety equipment or bypassing safety systems without proper authorization.

35 PotashCorp Aurora Safety Absolutes 10. Operation of equipment/vehicles in a dangerous or reckless manner. 11. Willful violation of security protocols for entry into the facility or restricted areas. 12. Blatant disregard for properly marked zones of imminent danger (barricades, hot zones, open holes etc.).
36 PotashCorp Aurora Safety Absolutes 13. Use of cell phone while driving or operating equipment (includes hands free devices). 14. Unauthorized crossing properly marked zones of imminent danger (i.e. red ribbon areas or danger do not enter areas) 15. Blatant disregard for your own safety or the safety of others.
37 Failure to Comply with PotashCorp Aurora Safety Absolutes: 1. Each incident is subject to disciplinary action up to and including termination. 2. Each incident could result in loss of rights to enter the plant premises.
