2 HAZ-MAT- PRODUCT ID & SCENE OPS NFPA CFD SOG 8.5.4, 9.8 Practical Station ONE The candidate must identify a given hazardous material. They should respond at the operational level ONLY. All safety hazards to personnel should be identified and precautions taken to reduce the risk of exposure. The candidate shall direct a team in a defensive operation on a hazardous materials scene. They should respond at the operational level ONLY. All safety hazards to personnel should be identified and precautions taken to reduce the risk of exposure. The candidate shall assist a team in a defensive operation on a hazardous materials scene. They should respond at the operational level ONLY. All safety hazards to personnel should be identified and precautions taken to reduce the risk of exposure. The candidate shall assist a team in a defensive operation on a hazardous materials scene. 6 OVERPACK DRUM- Nail Puncture NFPA Practical Station FIVE patch and overpack a leaking drum. Set up equipment to be used for operations. 20 Control leak by rolling drum or getting hole in the vapor space FAIL Patch the puncture hole. 10 Properly place drum into overpack drum by accepted method. 20 Upright overpack drum. 10 Locate overpack drum out of spilled material 10 Notify entry team leader that the operation is complete. 10 Exit out to the decontamination sector 10 Page 6 of 19

7 OVERPACK DRUM- Bung Leak NFPA Practical Station SIX control a bung leak on a drum. Set up equipment to be used for operations. 20 Control leak by rolling drum or getting hole in the vapor space FAIL Properly tighten the bung on the drum Upright the drum. 20 Locate drum out of spilled material Notify entry team leader that the operation is complete. 10 Exit out to the decontamination sector. 10 Page 7 of 19

8 OVERPACK DRUM- Forklift Puncture NFPA Practical Station SEVEN stop a large puncture and overpack the leaking drum. Set up equipment to be used for operations. 20 Control leak by rolling drum or getting hole in the vapor space FAIL Patch the puncture hole. 10 Properly place drum into overpack drum by accepted method. 20 Upright overpack drum. 10 Locate overpack drum out of spilled material 10 Notify entry team leader that the operation is complete. 10 Exit out to the decontamination sector 10 Page 8 of 19

9 CHLORINE A-KIT- SIDE PATCH NFPA , 4-3.2, A-4-3.1, A Practical Station EIGHT apply an A-kit for a leaking 150lb Chlorine cylinder. ALL actions shall be in accordance with the kits manufacturers specifications/ reccomendations. Set up equipment to be used for operations. 10 Control leak by actuating a valve if possible 15 Prepare kit to be placed on cylinder... 5 Prepare Cylinder for patch Secure side patch Ensure leak has been completely stopped Secure cylinder from tipping 10 Notify entry team leader that the leak was stopped. 10 Exit out to the decontamination sector. 10 Page 9 of 19

10 CHLORINE A-KIT- Valve Leak NFPA , 4-3.2, A-4-3.1, A Practical Station NINE apply an A-kit for a leaking 150lb Chlorine cylinder. Set up equipment to be used for operations. 10 Control leak by actuating a valve if possible 15 Prepare kit to be placed on cylinder... 5 Prepare Valve Clamp.. 5 Roll onto base... 5 Secure clamp... 5 Shut down vent on bonnet... 5 Ensure leak has been completely stopped Secure cylinder from tipping Notify entry team leader that the leak was stopped. 10 Exit out to the decontamination sector. 10 Page 10 of 19

11 CHLORINE A-KIT- Valve Seat NFPA , 4-3.2, A-4-3.1, A Practical Station TEN reseat the valve stem for a leaking 150lb Chlorine cylinder. Set up equipment to be used for operations. 10 Control leak by actuating a valve if possible 15 Prepare kit to be placed on cylinder... 5 Prepare Cylinder for bonnet.. 5 Roll onto base... 5 Secure bonnet.. 5 Shut down vent on bonnet 5 Ensure leak has been completely stopped Secure cylinder from tipping Notify entry team leader that the leak was stopped. 10 Exit out to the decontamination sector 10 Page 11 of 19

12 CHLORINE B-KIT- Valve Leak Practical Station ELEVEN NFPA , 4-3.2, A-4-3.1, A apply a B-kit for a leaking 1- Ton Chlorine cylinder. Set up equipment to be used for operations. 10 Control leak by actuating a valve if possible 15 Prepare kit to be placed on cylinder... 5 Prepare Cylinder for bonnet.. 5 Secure bonnet.. 10 Shut down vent on bonnet... 5 Ensure leak has been completely stopped Secure cylinder from movement. 10 Notify entry team leader that the leak was stopped. 10 Exit out to the decontamination sector 10 Page 12 of 19

13 CHLORINE B-KIT- SIDE PATCH Practical Station TWELVE NFPA , 4-3.2, A-4-3.1, A apply an B-kit for a leaking 1 ton Chlorine cylinder. Set up equipment to be used for operations. 10 Control leak by actuating a valve if possible 15 Prepare kit to be placed on cylinder... 5 Prepare Cylinder for patch Secure side patch Ensure leak has been completely stopped Secure cylinder from tipping Notify entry team leader that the leak was stopped. 10 Exit out to the decontamination sector. 10 Page 13 of 19

14 CHLORINE C-KIT- Valve Leak Practical Station THIRTEEN NFPA , 4-3.2, A-4-3.1, A reseat the valve stem for a leaking Chlorine rail car. Set up equipment to be used for operations. 10 Control leak by actuating a valve if possible 15 Prepare kit to be placed on rail car Prepare rail car for bonnet Secure bonnet.. 10 Shut down vent on bonnet... 5 Ensure leak has been completely stopped Secure cylinder from tipping Notify entry team leader that the leak was stopped. 10 Exit out to the decontamination sector. 10 Page 14 of 19

15 PIPE PATCH Practical Station FOURTEEN NFPA stop a leak from a fixed pipe leak. Set up equipment to be used for operations. 20 Control leak by actuating a valve if possible 20 Place patch over leak Ensure leak has been completely stopped Notify entry team leader that the leak was stopped. 10 Exit out to the decontamination sector. 10 Page 15 of 19

16 DOME COVER CLAMP Practical Station FIFTEEN NFPA stop a leak from MC-306 dome cover. Set up equipment to be used for operations. 20 Ground the tank prior to operations. FAIL Safely operate around tank. FAIL Apply dome cover clamp Ensure leak has been completely stopped Notify entry team leader that the leak was stopped. 10 Exit out to the decontamination sector. 10 Page 16 of 19

17 HAZMAT- VICTIM DECONTAMINATION Practical Station SIXTEEN NFPA CFD SOG 8.5.4, 9.8 The candidate shall identify the proper type of decontamination system to set up to decontaminate victims, responders, tools, and equipment. SETUP Identifies product(s) involved. FAIL Sets up a decontamination cooridoor for incident FAIL Identify type of decon system to set up. (Wet or Dry)... 5 Identifies all parts to the decontamination system.. 5 Identifies a decontamination team... 5 Properly set up a decontamination system... 5 S Communicates status with the IC. 10 Coordinates the victims through the system 5 Decontaminates the victims. 5 Coordinates the responders through the system 5 Properly decontaminates the responders.. 5 Ensures all equipment is decontaminated 5 Ensures that all decon personnel go through the system 5 Coordinates with EMS for evals TAKE DOWN Coordinates the take down of the system.. 10 Identifies that ALL equipment has been properly deconed Properly completes all paperwork 10 Page 17 of 19

18 HAZMAT- PPE Practical Station SEVENTEEN NFPA The candidate shall identify the proper type of Personal Protective Equipment (PPE) For a given situation. The canidate shall identify and inspect each type of PPE available. SCBA SCBA Donning & Doffing - Extend all straps before donning 1 - Check for full tank gauge reading full.. FAIL - Donning Did the examinee per manufacturers recommendations: - Turn on tank valve or main valve as necessary 1 - Compare tank gauge with regulator gauge 1 - Acceptable procedure for donning backpack,. 1 - Fasten all harness straps properly. 1 - Extend the coat collar free of straps Don facepiece, pull spider straps back, not sideways Check facepiece seal (pressure test). FAIL - Complete donning within 60 seconds 1 Sub Total (8) APR APR Donning & Doffing Extend all straps before donning.. 2 Identify Proper cartrage and exposure limits... FAIL Donning Did the examinee per manufacturers recommendations: Don facepiece, pull spider straps back, not sideways Check facepiece seal (pressure test).. FAIL Complete donning within 60 seconds 1 Sub Total (3) LEVEL D Identify types of Level D Protection 2 Identify potential points that a chemical can breach the suit 10 Identify a potential chemical exposure through the following method Penetration 5 Permeation 5 Inspect the suit for any defects, cracks, tears, Etc.. FAIL Properly Don and Doff the suit FAIL Sub Total (22) Page 18 of 19
