Dear Mr. and Mrs. Wegel: As requested, Hepworth-Pawlak Geotechncal, nc. has desgned an ndvdual Sewage Dsposal System (1SDS) based on the percolaton rate of the subsols for the proposed resdence to be located at the Yonley Mll Ste No. 575 n Montezuma, Colorado. The study was conducted n accordance wth our wrtten agreement to you dated October 18, The data obtaned and our 1SDS desgn recommendatons based on the proposed constructon and subsurface condtons encountered are presented n ths report. Ste Condtons: The project ste slopes gently down to the west and ste vegetaton Ìlthe area of the proposed SDS conssts prmarly of grass. 111ere s an exstng cabn located on the propert. Water servce wì be provded to the proposed four bedroom resdence by an exstng onsîte water well. The southern porton of the property s comprsed ofwetjands assocated wth the upper Snake Rver. The proposed resdence locaton and orentaton s conceptual at ths tme. Once archtechral plans are developed we should be contacted to reevaluate the recommendatons of ths report. Percolaton Test: Percolaton tests were conducted on June 14,2005 by Walter O. Schultz, P.E. to evaluate the percolaton rate of the nahral sols at the ste. The average percolaton rate s 16.4 mnutes per nch. The percolaton test results are provded as an attachment A profe pt was excavated at the ste and observed by Dan Hendershot of the Sunmt County Envronmental Health Department and Walter Schultz on June 13,2005. A copy of the On- Ste nspecton Form has been provded as an attachment. Groundwater was observed to be present n the profle pt at approxmately 6.5 feet below the ground surace; therefore, cut Glenwood Sprngs ) Parker ø Colorado Sprngs

SDS Analyss: Based on the subsuface condtons encountered and the percolaton test results, the tested area s sutable for an SDS utlzng the shallow sols for the treatment and dspersal of effluent. A ste plan showng the proposed SDS, exstng water well, proposed resdence, buldng envelope, water courses, wetlands and the mnmum horzontal setback dstances of ste features relatve to the SDS components s shown on Fgure. The SDS meet the the current Summt County ndvdual Sewage Dsposal System Regulatons. was desgned for a total of four bedrooms. All materals and nstallaton should requrements of Septc and Dosng Taes: A three chamber concrete 2,000 gallon Front Range Precast septc tank, or equvalent, wl be used for prmary treatment of sewage ÍÌom the resdence. An effuent flter wll be nstalled on the outlet of the second chamber ofthe septc tank Sewage wll gravty feed from the proposed resdence to the septc tank and effuent w be pressure dosed to a dstrbuton box and then effluent wll gravty feed to the absorpton area. f the septc tank access rser lds wll be ntalled at grade, wth 110 backfll cover, then the lds should be nsulated wth foam nsulaton board to provde frost protecton. The sewer ppe nlet tee n the frst chamber ofthe septc tank should be nstalled beneath the access ld of the frst chamber of the septc tank so that t can be reached for servce should t become clogged. A hgh water alarm swtch should be nstalled n the dosng chamber of the septc tank 3 to 6 nches above the upper float poston. The hgh water alarm wl provde audo and vsual warnng of a hgh water condton n the tank and should be placed nsde or on the dwellng where t wll be readly seen or heard. The PVC handle on the effluent flter should be extended up to wthn reach of the dose chamber access rser ld. Ths wu aow for easy access and mantenance of the flter. Approxmately 2 feet of sol backfll cover should be placed over the tops of the septc and dose tanks to allow for easy access and frost protecton. For mantenance reasons deep tank nstallatons should be avoded and we should be contacted to evaluate ths SDS desgn should t appear that a deep tank nstallaton nvolvng more than 4 feet of backfll sol over the top of the septc dose tank may be requred. 4 Mr. and Mrs. Wegel August 1, 2007 Page 3 Dosng Pump: An Orenco PEF 3311, 115 volt, submersble effuent pump wth a 2 nch dameter dscharge assembly, or equvalent, s sutable for ths applcaton. f an equvalent pump s used t must be capable of provdng a total flow rate of 10 gallons per mnute at 10.3 feet of total dynamc head. For desgn purposes t was assumed that the elevaton dfference from the dosng pump ntake to the absorpton the length of the effuent transpolt ppe does not exceed 35 feet. area manfold s not more than 10 feet and that The dosng pump wll be controlled by a conttol panel and a system of float swtches. The float swtches should be set to provde a dose of about 231 gallons. The actual dose volume s about 225 gallons, assumng an effuent dranback volume of about 6 gallons n the transport ppe. At ths dose volume the absorpton area wl be dose about four tmes per day. Sewer and Effuent Ppng: The sewer lne from the proposed resdence to the septc tank should not be less than the dameter of the buldng dran and not less than 4 nches n dameter. Sol backfll beneath the sewer ppe must be adequately bedded and compacted to prevent settlement of the ppe. The sewer ppe should be nstalled n landscape areas wth at least 2 feet of sol backfll coverng the ppe, or the ppe should be nsulated on the top and sdes wth 2 nch thck rgd foam nsulaton board for frost protecton. Beneath drven sufaces we suggest Schedule 40 PVC sewer ppe should be used and the ppe should be nsulated f t wl be nstalled wth less Ùan 4 feet of backfll sol cover. We suggest the use of Schedule 40 PVC ppe be\:veen the proposed resdence and the septc tank, though ths s not requred by local regulatons. The porton of the sewer ppe extendng from 5 feet outsde the buldng foundaton should have a slope of at least 2%. Cleanout ppe(s) should be nstalled the sewer lne adjacent from the buldng foundaton, adjacent to the nlet of the septc tank, where the sewer ppe bends at 90 degrees or more and at least one cleanout should be nstalled at least every 100 lnear feet ofo-pressurízed sewer or effluent ppe throughout ts length. We recommend that a straght secton of ppe be nstaued between the resdence foundaton and the septc tank. Any requred 90 degree bends n the sewer ppe should be accomplshed through the use of a seres of 45 degree smooth elbows or a 90 degree long sweep to help prevent cloggng of the ppe. Job No ~tec

Abso'pton Area: The sol absorpton area s desgned to be approxately 588 square feet and wl consst of three absorpton beds. Each absorpton bed wll consst of two rows of ten nfltrator Quck 4 Standard for a total of 60 chambers for the entre absorpton area..per Summt County gudelnes each chamber was gven 9.8 square feet of absorpton area. Absorpton area calculatons are shown on Fgure 2. The absorpton bed should be excavated to the approxmate depths slmwn on Fgw'e 4. Due to shallow!:l'oldwater at the ste, cut depths must be no deeper than about 2.5 feet n order to mantan the 4 foot separaton from the dstrbuton laterals to groundwater. Care must be taken durng nstallaton to ensure that the sols exposed n the bottom and sdewalls of the bed excavatons are not compacted. Once the bed excavatons are complete they should be left open, f necessary, to allow the sols to ar dy pror to placement of chambers and dstrbuton ppes. Once the sobs have ar dred, f necessary, the bottom and sdewalls of the excavatons should be scarfed pror to placement of the chambers and ppng. The on-ste sols can be used as backfll over the chambers and should be graded to deflect surface water and precptaton away from the absorpton area. We recommend the sol backfll be re-vegetated as soon as possble wth a mountan meadow grass mx or sutable equvalent. t s mportant not to plant large vegetaton or vegetaton wth nvasve roots n and around the absorpton area and manfold ppng as the roots may dan1age ppng. Combnaton ar vent/nspecton ports should be nstalled at each comer of the absorpton beds. The ports should be constmcted of4 nch dameter PVC ppe nstalled nto the knockouts provded on the tops of the fíltratorchambers. 111e ports should extend a mnmum of 8 nches above the fnshed grolmd surface and should have ventlated removable caps. We recommend that the sol beneath the dstrbuton box be adequately compacted to prevent settlement of the box or the box may be set on a small level concrete pad. t s crtcal that the box be nstalled level and that the flow lnes of the outlets are at equal elevatons to ensure that effuent s equally dstrbuted to each trench. A thmst block wll be requred n the dstrbuton box because of the relatvely hgh velocty of ncomng effuent or a 90 degree elbow can be nstalled on the effuent ppe from the septc tank nsde the dstrbuton box to drect effuent nto the bottom of the box. Speed levelers should be nstalled n the effuent manfold ppe outlets wthn the dstrbuton box and should be adjusted to provde even dstrbuton to each outlet. f desred, the dstrbuton box may be nstalled wthn a vault for easy access. The absorpton area and any areas drectly up gradent of t should not be watered by lawn rrgaton systems or other means as the sols may become hydraulcally overloaded. Septc tanks and other treatment tanks should be pmnped whenever the sludge occupes 1/3 of the lqud capacty of the frst Job No ~h

The hgh water alarm float swtch should be checked at least once per year to ensure that t s workg properly. n addton, the effuent flter should be checked for cloggng at least once per year and cleaned as necessay. We recommend that an operaton and mantenance manual be developed for the SDS. The SDS should be operated and mantaned as stpulated n the manual f you desre our servces, we can prepare the operaton and mantenmce manual for the on-ste SDS. Lmtatons: Ths desgn has been conducted n accordance wth generally accepted engneerng prncples and practces n ths area at ths tme. We make no warranty ether express or mpled. The conclusons and recommendatons submtted n ths report are based upon the data obtaned from the profle pt and percolaton test holes excavated at the approxmate locaton ndcated on Fgure 1, the percolaton test results, the attached On-Ste nspecton.form, the proposed type of constncton, and our eh.'lerence n the area. Our fndngs nclude nterpolaton and extrapolaton of the subsudace condtons dentfed at the profe pt and percolaton test holes and varatons n the subsurface condtons may not become evdent untl excavaton s perfoued. f condtons encountered durng constructon appear dfferent from those descrbed n ths report, we should be notfed at once so reevaluaton of the recommendatons may be made. Based on ste reconnassance, t s our opnon that the desgned locaton of the SDS does not volate any setback requrements of the current Summt County EvronmentalHeath Department's 1SDS regulatons. The staked locatons ofthe TSDS components are approxmate and not for constructon. We recommend that the poston ofthe SDS components, proposed resdence, proposed water well and setback requrements be verfed n the feld by a Professonal Land Surveyor lcensed n the State of Colorado. l1s report has been prepared for the exclusve use by our clent for the SDS desgn submttal process. We are not responsble for techncal nterpretatons by others of our nformaton. Dear Mr. and Mrs. Wegel: As requested, Hepworth-Pawlak Geotechncal, nc. has desgned an ndvdual Sewage Dsposal System (SDS) based on the percolaton rate of the subsols for the proposed resdence to be located at the Yonley Ml Ste No. 575 Montezuma, Colorado. The study was conducted n accordance wth our wrtten agreement to you dated October 18, The data obtaned and our SDS desgn recommendatons based on the proposed constructon and subsurface condtons encountered are presented n ths report. Ste Condtons: The project ste slopes gently down to the west and ste vegetatìon n the area of the proposed SDS conssts prmarly of grass. Water servce wl be provded to the proposed four bedroom resdence by an onste water well The northwest porton of the propety s comprsed of wetlands assocated wth the upper Snake Rver. The proposed resdence locaton and orentaton s conceptual at ths tme. Once archtectural plans are developed we should be contacted to re-evaluate the recommendatons of report. ths.percolaton Test: Percolaton tests were conducted on July 20, 2007 to evaluate the percolaton rate of the natural sols at the ste. Three percolaton test pts were excavated at the approxmate locatons shown on Fgure. The test holes were hand excavated from ground surface and were soaked wth water for 8 to 24 hours pror to testng. A proflepìt was excavated at the ste and observed by Dan Hendershot of the Summt County Envronmental Health Department and Walter Schultz on June 13,2005. Groundwater was found to be present n the profle pt at about 7 feet below the ground surface; therefore, cut depths must be no deeper than about 3 feet durng constncton of the SDS to ensure that the requred 4 foot separaton to groundwater s mantaned. The sols exposed n the percolaton test holes appeared to smlar to those observed n the profle pt. Percolaton tests were pedonned at depths rangng from about 21 to 24 nches below ground surface. The average sol percolaton rate s 24 mnutes per nch and the percolaton test results are presented on Fgure 3. SDS Analyss: Based on the subsurface condtons encountered and the percolaton test results, the tested area s sutable for an SDS utlzg the shallow sols for the treatment and dspersal of effuent. A ste plan showng the proposed SDS, proposed water well, proposed resdence, buldng envelope, water courses, wetlands and the mnmum horzontal setback dstances of ste features relatve to the SDS components s shown on Fgure. The SDS was desgned for a total of four bedrooms. All naterals and nstallaton should meet the requrements of the current Summt County ndvdual Sewage Dsposal System Regulatons. Septc and Dosng Tanks: A three chamber concrete 2,000 gallon Front Range Precast septc tank or equvalent, wl be used for prmary treatment of sewage from the resde1ce. An effuent fter wl be nstalled on the outlet of the second chamber of the septc tank:. Sewage wl gravty feed from the proposed resd~nce to the septc tank and effuent wll be pressure dosed to the absorpton area. f the septc tank access rser lds wl be nstalled at grade, wth no backfll cover, then the lds should be nsulated wth foam nsulaton board to provde frost protecton. The sewer ppe nlet tee n the frst chamber of the septc tak should be nstalled beneath the access ld of the fst chamber of the septc tank so that t can be reached for servce should t become clogged. For mantenance reasons deep tank nstallatons should be avoded and we should be contacted to evaluate ths SDS desgn should t appear that a deep tank nstallaton nvolvng more than 4 teet of backfll sol over the top ofthe septc tank may be requred. Dosng Pp: An Orenco PEF 33 1, 115 volt, submersble effuent pump wth a 2 nch dameter dscharge assembly, or equvalent, s sutable for ths applcaton. f an equvalent pump s used t must be capable of provdng a total flow rate of30.2 gallons per mnute at 16.5 feet of total dynamc head. For desgn purposes t was assumed that the elevaton dfference from the dosng pump ntake to the absorpton area manfold s not more than 10 feet and that the length of the effuent transport ppe does not exceed 35 feet. The dosng pump wl be controlled by a control panel and a system of float swtches. The float swtches should be set to provde a dose of about 231 gallons. The actual dose volume s about 225 gallons, assumng an effuent dranback volume of about 6 gallons n the transport ppe. At ths dose volume the absorpton area wl be dose about four tmes per day. Sewer and Effuent Ppng: The sewer l.ne from the proposed resdence to the septc tank should not be less than the dameter of the buldng dran and not less than 4 nches n dameter. Sol backfll beneath the sewer ppe must be adequately bedded and compacted to prevent settlement of the ppe. The sewer ppe should be nstalled n. landscape areas wth at least 2 feet of sol backfll coverng the ppe, or the ppe should be ~ nsulated on the top and sdes wth 2 nch thck rgd foam nsulaton board for frost Job No

Beneath drven surfaces we suggest Schedule 40 PVC sewer ppe should be used and the ppe should be nsulated ft wl be nstalled wth less than 4 feet of sol cover. backfll We suggest the use of Schedule 40 PVC ppe between the proposed resdence and the septc tank, though ths s not requred by local regulatons. The porton of the sewer ppe extendng from 5 feet outsde the buldng foundaton should have a slope of at least 2%. Cleanout ppe(s) should be nstalled n the sewer lne adjacent :f'01l the buldng foundaton, adjacent to the nlet of the septc tank, where the sewer ppe bends at 90 degrees or more and at least one cleanout should be nstalled at least every 100 lnear feet ofnon-pressufìzed sewer or eftluent ppethrouglout ts length. We recommend that a straght secton of ppe be nstalled between the resdence foundaton and the septc tank. Any requred 90 degree bends n the sewer ppe should be accomplshed through the use of a seres of 45 degree smooth elbows or a 90 degree long sweep to help prevent cloggng of the ppe. The 2 nch dameter schedule 40 PVC transport ppe should be sloped at 2% mnmum to dran back to the dose chamber between doses. t s crtcal that the effuent transport ppe be properly bedded to prevent settlement whch could cause standng eftluent to f'eeze n the ppe. Absorpton Area: The sol absorpton area s desgned to be approxmately 706 square feet and wll consst of an absorpton bed consstg of sx rows of 12 nfltrator Quck 4 Standard chambers for a total of 72 chambers. Per Sununt Comty gudelnes each chamber was gven 9.8 square feet of absorpton area. Absorpton area calculatons are shown on Fgure 3. The absorpton bed should be excavated to the approxmate depths shown on Fgure 5. Due to shallow groundwater at the ste, cut depths must be no deeper than about 3 feet n order to mantan the 4 foot separaton from the dstrbuton laterals to groundwater. Care must be taken durng nstallaton to ensure that the sols exposed n the bottom and sdewalls of bed excavatons are not compacted. Once the bed excavaton s complete t should be left the Job No Gètec

Combnaton ar vent/nspecton ports should be nstaued at each corner of the absorpton bed. The ports should be constructed of 4 nch dameter PVC ppe nstalled nto the knockouts provded on the tops of the nfltrator chambers. 'The ports should extend a mumum of 8 nches above the fnshed ground srface and should have ventlated removable caps. l1e absorpton area manfold and dstrbuton ppng layout s shown on Fgure 5. All materals used and nstallaton methods should meet the requrements of the Summt COllty Envronmental Health Deparent's Aprl 25, 1996 SDS Regulatons as amended January 26, 1998 and June 8,2000. The level of mantenance wll vary wth the complexty of the system and water use habts of the users. Septc tanks and other treatment tanks should be pumped whenever the sludge occupes 1/3 of the lqud capacty of the frst chamber of the septc tank. The hgh water alarm float swtch should be checked at least once per year to ensure that t s workng properly. n addton, the eruent fter should be checked for cloggng at least once per year and cleaned as necessary. We recommend that an operaton and mantenance manual be developed for the SDS. The SDS should be operated and mantaned as stpulated n the manual. f you desre our servces, we can prepare the operaton and matenance manual for the on-ste SDS. Lmtatons: Ths desgn has been conducted n accordance wth generally accepted engneerng prncples and practces n ths area at ths tme. We make no warranty ether express or mpled. The conclusons and recommendatons submtted n ths report are based upon the data obtaned from the profe pt and percolaton test holes excavated at the approxmate locatons ndcated on Fgure 1 and to the depths shown on Fgures 2 and n the attached On-Ste nspecton Form the proposed type of constructon, and our experence n the area. Deal' Mr. and Mrs. Hudson: As requested, Hepwoth~Pawlak Geotechncal, nc. has redesgned an ndvdual Sewage Dsposal System (SDS) based on the percolaton rate of the subsols for the proposed three bedroom resdence located at Yonley Ml Ste M.S. No. 574 near Montezuma, Colorado. The redesgn was completed at your request to reduce the number of bedrooms from four n the orgnal desgn dated July 5, 2007 to three bedrooms. The study was conducted n accordance wth our wrtten agreement to you dated November 3,2006. The data obtaned and our SDS desgn recommendatons based on the proposed constructon and subsurface condtons encountered are presented ths report. Ste Condtons: The project ste slopes gently down to the west and ste vegetaton n the area of the proposed SDS conssts of prmarly of grasses. Water servce wl be provded to the proposed resdence by an onstewater well. At ths tme, the proposed resdence locaton and sze s conceptual. Once archtectural plans are prepared, we should be notfed to re-evaluate the recommendatons ofths report. Pel'colaton Test: Percolaton tests were conducted on June 7, 2007 to evaluate the percolaton rate ofthe natural sols at the ste. One profle pt and three percolaton test pts were excavated at the approxmate locatons shown on Fgure 1. The test holes were hand excavated at the bottom of the shallow backhoe pts and were soaked wth water for 8 to 24 hours pror to testng. The sols exposed n the profe pt consst of about Crenwood Sprngs Z'J " 7J

A log of the subsol condtons encountered n the profle pt are presented n Fgure 2. The sols exposed n the percolaton test holes were smlar to those observed n the profle pt. Percolaton tests were performed at depths rangng from about 32 to 35 nches below the adjacent ground surface. The average sol percolaton rate s 5 mnutes per nch and the percolaton test results are presented on Fgure 3. SDS Analyss: Based on the subsurface condtons encountered and the percolaton test results, the tested area s sutable for an SDS utìlzng the shallow sols for the treatment and dspersal of effluent. A ste plan showng the proposed SDS, proposed water well, proposed resdence and the mnmum horzontal setback dstances of ste features relatve to the SDS components s. shown on Fgure 1. The SDS was desgned for a total of four bedrooms. All materals and nstallaton should meet the requrements of Summt County ndvdual Sewage Dsposal System Regulatons. the current Septc and Dosng Tanks: A 2,000 gallon, concrete three chamber Front Range Precast septc tan, or equvalent, wll be used for prmary treatment of sewage from the resdence. The frst two chambers of capacty of at least 1,500 gallons. An effuent.fter wll be nstalled on the outlet of the septc tank must have an effectve lqud second chamber of the septc tank. Sewage wl gravty feed from the proposed resdence to the septc tank and effluent wll be pressure dosed to the absorpton the septc tank detal from Front Range Precast s provded as an the area.
