3 1) INTRODUCTION This document outlines the Fatigue Management and Emergency Response Policies and Procedures of Camlin Investments. 1.1 OBJECTIVE The objective of the Fatigue Management and Emergency Response Manual is to ensure all employees within the company are aware of their roles and responsibilities with regards to: (a) (b) (c) (d) Avoidance of Fatigue Management of Fatigue Emergency Response in the event of an incident Transport Emergency Response Plan 2) FATIGUE MANAGEMENT Camlin Investments identifies its role in managing the effect of fatigue on its employees as: Identifying the hazards that can contribute to fatigue Assessing the risks of these hazards Implementing and maintaining risk control measures, and Reviewing the effectiveness of the control measures. Fatigue is an acute, ongoing state of tiredness that leads to mental or physical exhaustion and prevents people from functioning within normal boundaries. It is more than feeling tired and drowsy, it is a physical condition that can occur when a person s physical or mental limits are reached. Fatigue is one of the main causes of accidents on Australian roads. Further, Australian surveys have shown that 45% of long distance heavy vehicle drivers have experienced Fatigue. During , heavy vehicles were involved in 15% of fatalities on Australia Roads. It is the Policy of Camlin Investments to remain cushioned from these statistics by engaging drivers who commit to following established policies to prevent fatigue. It is our ongoing obligation to ensure all drivers are aware of the signs of fatigue; the causes of fatigue and how to manage fatigue, if and when it does appear. 2.1 Warning Signs The signs of fatigue include: Yawning Heavy or sore eyes Blurring Vision Seeing things that aren t there Daydreaming and not concentrating Becoming impatient Feeling hungry and thirsty Slowing of reaction time Feeling stiff or cramped Fluctuating driving speed Poor gear changes Wandering over the centre line, or over the road edge Any one, or a combination of these signs is a warning that fatigue has commenced and action needs to be taken immediately. 4 2.2 Avoidance of Fatigue It is the responsibility of Camlin Investments Pty Ltd to develop schedules and trip plans to attend to the requirements of the customer. These schedules are developed by the Transport Operations Manager, who co-ordinates company drivers in line with relevant State based legislation and NHVR regulations and requirements regarding maximum driving hours, as outlined in Table 1. Table 1 Operating Limits as per Heavy Vehicle (Fatigue Management) National Regulation 2012 Total Period Maximum Working Time Minimum Rest Time In any period of. A Driver must not work for more than a total of To remain out of Risk Breach, must have 5 ½ hours 5 ¼ hours 15 mins continuous rest 8 hours 7 ½ hours 30 mins rest, in blocks of 15 mins continuous rest 11 hours 10 hours 60 mins rest, in blocks of 15 mins continuous rest 24 hours 12 hours 7 continuous hours stationary rest * 7 days (168hrs) 72 hours 1x24 hours continuous stationary rest 14 days (336 hrs) 144 hrs 2x24 continuous hrs stationary rest *. 1 st 24hrs rest must be taken after a max of 84hrs work. 4 nights off, includes 2 consecutive nights rest 2.3 FATIGUE MANAGEMENT RESPONSIBILITIES MANAGING DIRECTOR The Managing Director is responsible for overseeing the following: (a) (b) (c) (d) (e) (f) (g) The roadworthiness of all vehicles in the Camlin fleet, including the functionality of the GPS monitoring equipment, as per the NHVR Maintenance Management Manual. Communication with Coregas regarding future planned work, and attending to staffing needs to ensure adequate coverage with respect to Fatigue Management Communication with the Transport Operations Manager to ensure all drivers are rested and fit to drive Overseeing the scheduling of routes, to ensure Operating Limits are managed Overseeing of LogChecker entries by Administration Manager to monitor driver operating limits compliance, and attendance to any breach behaviour Continual review and development of the Emergency Response Policies and Procedures Overseeing employment and training/induction of new drivers to ensure they are complicit in procedures of Fatigue Management and Emergency Response TRANSPORT OPERATIONS MANAGER The Transport Operations Manager is responsible for the overall operation of Fatigue Management and Emergency Response Procedures, and in particular for: Fatigue & Emergency Camlin V(2) Effective: 01/07/2017 Review By: 01/07/2019 Page 4 of 13
5 (a) (b) (c) (d) (e) (f) (g) Ensuring the all vehicles servicing schedules are maintained and faults rectified to ensure the roadworthiness of the fleet, including the functionality of the GPS monitoring equipment, as per the NHVR Maintenance Management Manual. Future planning / scheduling to ensure any upcoming work is managed adequately, with respect to Fatigue Management Overseeing the leave requirements of drivers to ensure drivers are rested and fit for work Scheduling of day to day / week to week work in particular line haul routes, to ensure Operating Limits are adhered to. Working with the Administration Manager to monitor driver operating limits compliance, and attendance to any breach behaviour, as per LogChecker reports Ensuring compliance of the Emergency Response Policies and Procedures Overseeing employment and training/induction of new drivers to ensure they are complicit in procedures of Fatigue Management and Emergency Response ADMINISTRATION MANAGER The Administration Manager is responsible monitoring of Fatigue Management and Emergency Response Procedures, and in particular for: (a) (b) (c) (d) (e) (f) Assisting the Transport Operations Manager to ensure the all vehicles servicing schedules are maintained and faults rectified to ensure roadworthiness of the fleet, including the functionality of the GPS monitoring equipment, as per the NHVR Maintenance Management Manual. Managing any employment requirements with regards to additional upcoming work to ensure adequate staffing Overseeing the leave requirements of drivers to ensure drivers are rested and fit for work Monitoring operating limits compliance, and attending to any breach behaviour, as per LogChecker reports, with the assistance of the Administration Support Ensuring compliance of the Emergency Response Policies and Procedures Overseeing employment and training/induction of new drivers to ensure they are complicit in procedures of Fatigue Management and Emergency Response CAMLIN DRIVERS All drivers are responsible for managing their schedules to avoid fatigue and to ensure adherence to Emergency Response procedures, and in particular for: (a) Adhering to fleet service schedules, and fault repairing, as per the NHVR Maintenance Management Manual. Undertaking Daily Visual Inspections of fleet. (b) Utilising the resources provided to adequately plan their scheduled route, to ensure NHVR Operating Limits are complied with NHVR Operating Limits Break Planner (c) Planning leave to ensure they are fit and rested to driver (d) Completion of the National Driver Work Diary correctly, and submission of these with their weekly time sheet (e) Following any direction or corrective action given in relation to an NCR issued due to a breach (f) Adhering to Camlin Policy of being off the road by 10pm; avoiding 15 min breaks, in favour of 30 min breaks. (g) Ensuring compliance of the Emergency Response Policies and Procedures (h) Following correct Incident / Accident Reporting in the event of such. 6 2.4 LOGCHECKER Camlin is committed to ensuring compliance to NHVR Legislation with regards to Operating Limits for all drivers in their employ and utilising software called LogChecker to enter driver National Work Diary pages to provide a history of each driver. This software is programmed to highlight warnings of potential breaches, and to highlight Minor and Critical Breaches of Operating Limits. This information is used two-fold - Driver Specific To highlight Breaches or potential breaches to an individual driver, with issue of a Non-Conformance Report, requiring attendance to the behaviour. Continual breach of the same Operating Limits Rule will result in disciplinary action, suspension from driving, or in severe cases dismissal. - Company Wide When a specific rule is being breached across multiple drivers, the information is additionally used to educate all drivers through Memos of Awareness and Understanding / Company Newsletter / Update of training and induction tools. Information is entered weekly, and attended to no later than 14 days post the driving time. 2.5 MANAGEMENT OF FATIGUE If an instance of fatigue is recongised, Camlin Investments will review the circustances surrounding the driver in question. The review shall include the following areas: a. Current shift completion time frames b. Prior fortnight s work diary / Log Checker Report c. Prior month s work schedule d. Last instance of leave that was taken e. Personal circumstances that may have been involved for example new baby; family concerns The review shall make amendments to the drivers schedule, and where required, allow for leave to be taken for rest and recovery. Driver schedules shall be reviewed to ensure adequate provision of drivers in similar delviery circumstances to safeguard no future instances of fatigue to arise. 7 3) DRIVERS RESOURCES REGARDING FATIGUE Second Space Cushion State based legislation states that the minimum following distance is 60m between heavy vehicles on single laned, two way roads outside a town area where the speed limit is greater than 60km/h. 1 Driver sees car pass tree 2 Truck needs to take at least 4 seconds to reach the tree 3.2 Health Tips for Long Distance Drivers Know your fatigue symptoms and have a plan to cope with them Rest, rehydrate, renourish. Every time you stop, stretch your body and back in all directions, nice and slowly. Wake up slowly - how you wake in the morning influences your attitudes and behaviour for the rest of the day. Happy, positive thoughts allow you to deal with frustrations and problems much more easily. Learn a positive wake up process that helps generate a positive attitude for the rest of the day. 3.3 REST STOPS Utilise Truck Stops to rest and refresh. Comprehensive Map of Truck rest areas available on Bulk Liquid Resources, on CANVAS. 3.4 TAKE 5 Repetitive work and numbness to the everyday routine can be a cause of fatigue. A systematic approach should be adopted for routine work Use this checklist to serve as a reminder: Before commencing the job: Stop & Think Observe the work area and surroundings Think about what else is happening in the area Identify the hazards Control the hazards Once the job is complete: Observe the work area Control any hazards that may have been created by work Consider how the job went, and what could be improved next time? 8 3.5 NHVR OPS LIMITS BREAK PLANNER TOOL Available on CANVAS BULK Liquid Resources and Bulk Cylinder Resources is an NHVR Operating Limits Break Planner Tool, which allows drivers to manage their breaks effectively, and ensure Operating Limits are adhered to. Exceerpt below. 4) EMERGENCY RESPONSE In the instance of an emergency, there exists several policies and procedures which all drivers are trained in during their Induction at comencement of employment. It is the Policy of Camlin Investments to review and update these Policies as we learn from incidents in the daily running of the business. Employees are updated on a regular basis (minimum 6 monthly) and a review of their awareness of Emergency Reponse is included in the Driver Audit (Refer Policy & Procedures Manual / Section 8 Training). Reaction time of an Emergency Reponse is critical, specifically with regards to dangerous goods, and Camlin takes the welfare of its employees and the working order of its equipment assests seriously - Safety is sacrosanct; it will not and cannot be compromised. It is the policy of Camlin Investments to supply knowledge to company drivers and all staff involved in the handling of dangerous goods, so that in the event of an emergency, appropriate actioon will be taken to minimise damage to lives and surrounding property. 9 4.1 TRANSPORT EMERGENCY RESPONSE PLAN (TERP) Camlin Investments has and maintains a TERP which outlines areas of responsibility in the event of an emergency, with regards to: Plan Activation Internal Alert / Situation Reporting / Resource Mobilisation Response External Alert / Emergency Action Containment Clean Up Resources Communications / PPE / Media Preparation Training / Knowledge Database / PPE Maintenance It is the Policy of Camlin Investments to ensure this TERP is provided to all drivers as part of their training and is made available in hard copy in the Drivers Folder, and is available under Bulk Resources on CANVAS The Core Objectives of the Transport Emergency Response Plan (TERP) are to Minimise adverse effects on employees and the general public, damage to property, or harm to the environment during an emergency Facilitate a rapid an effective response and recovery from an incident Provide assistance to emergency services Communicate vital information to relevant persons- both internal and external to the company This TERP is produced in line with Regulation 14.5 of the Road Transport Reform (Dangerous Goods) Regulations 1997, and the Australian Dangerous Goods Code 7th Edition. Section 1 Plan Activation Internal Alert In the event of an incident / accident or event requiring emergency response, primary contact with the Company is to be made to either, as soon as reasonably possible: Managing Director Gregory Plevey or Transport Ops Manager Michael Anning or Situation Reporting In the event of an incident / accident or event requiring emergency response, the Incident / Accident Report (available on CANVAS) is to be completed in full and submitted as soon as possible, and within 24 hours of the incident Once received, the report shall be discussed between the Managing Director, the Transport Operations Manager, and the Administration Manager to determine whether any modifications are to be made to Camlin s Emergency Reponse Policies and Procedures. The report shall then be discussed with the Supply Chain Manager of the Principle Contractor to recommend any modifications to Contractor SOPs. These shall then be incorporated within Camlin Policy. 10 At all times, Camlin Employee s shall follow Camlin Accident Procedures (See 4.4) and those procedures of Coregas at relevant depots, and the procedures of the customers of Coregas (as contained in Drivers Folder), where specific instructions and procedures have been issued to Camlin. Authority / Resource Mobilisation In a Containable Incident Gregory Plevey or Michael Anning to whom the incident has been reported will make the decision whether to call in external assistance. Drivers shall be advised to follow Action on Spills or Accident Procedures (Appendices 3, 4), and report using CANVAS. In an Emergency Situation, the driver of the vehicle involved with the incident shall be responsible for calling emergency assistance, as the first point of call to 000. (see Section 2 Response Tasks) Section 2 Response Tasks External Alert In a Containable Incident Gregory Plevey or Michael Anning to whom the incident has been reported will make the decision whether to call in external assistance. Quote UN Number of Goods being carried to determine appropriate action. In an Emergency Situation, the driver of the vehicle involved with the incident shall be responsible for calling emergency assistance, as the first point of call to 000, supplying full details about load being carried Specific DG, Class, Volume, UN/Hazchem Number. Emergency Action / Containment / Clean Up All employees are to follow the directions of Emergency Services attending the scene of an accident / incident, providing full details on the load being carried. Prior to Emergency Services arrival on the scene, Employees are to follow Action on Spills and Accident Procedures in the event of an incident. Where the incident involved a toxic spill / venting, the driver is to suit up in full PPE gear before exiting the vehicle (Appendix 5). Where the accident / incident is minor, the employee involved is to use the CANVAS application on the provided ipad to capture details and pictures of the scene, to assist in development of Policy and Procedure to ensure the incident is avoided in the future. 11 Communications Drivers mobile phone should be used in the first instance to make contact to Emergency Services, then to Camlin Management. In situations where mobile coverage is not available, UHF radio is to be used to contact Emergency Services or to contact persons in the area who have telephone service ability. Equipment Media Full PPE gear required during emergency accident or incident Hazard Triangles to be placed, where appropriate to alter nearby traffic or people to the hazard Use of SCBA where toxicity is a possibility for escape purposes only. Torch, as required Safety Bag equipment is audited 6 monthly, as outlined below Fire Extinguisher annual service plan is actioned HazChem Emergency Action Code Drivers are not permitted to talk to media, and all enquires are to be directed to Gregory Plevey. As per Camlin Investments s Social Media Policy (Employee Manual Section 6), no photos, comments or other are to appear on social media initiated by Camlin staff. Section 4 Preparation for Emergency Response Risk Assessment SWMS prepared for all site accessibility Extensive driver training in all product to be carried, including hazards of specific product Bulk Resources app on CANVAS contains valuable information of product MSDS, Coregas SOPs, copy of this Manual Training / Auditing At commencement of employment, a 2 Week on the job training and induction is undertaken with every employee to ensure knowledge of product carried, potential risks and procedure at each customer site, incorporating customer TERP and SOPs for emergency. 6 monthly auditing of procedure and annual product assessment is carried out to ensure performance standards are maintained. Coregas SOPs updated and circulated as provided by Copregas. Maintenance All Camlin and Coregas fleet are enrolled in National Heavy Vehicle Maintenance Management Accreditation, and have service schedules and fault repair undertaken as per rewuirements of this Accreditation. Jemima Craven 4.2 INCIDENT ACCIDENT REPORT The Incident / Accident Report is available on CANVAS, and captures details of Incident / Accident, including facility for photos, GPS capture, third party details. 4.3 ACTION ON SPILLS The action taken within the first few minutes can mean the difference between disaster and the quick return to normal operations. Initial judgement in an emergency situation - Find out what kind of hazardous material is involved - Take necessary action to control the incident When dealing with hazardous materials, there are several threats: 1. FIRE AND FLAMABILITY - There must be three elements present to create a fire- OXYGEN HEAT / IGNITION FUEL 2. REACTIVITY - That is a violent reaction when the hazardous material is exposed to air, water, heat or fire. 3. TOXICITY - Toxicity and the effect on health are as important as the first two threats. Each threat must be dealt with in its own way to obtain a nil threat. 13 4.4 ACCIDENT PROCEDURES Accident Procedures when carrying Dangerous Goods If the vehicle is drivable, the product containment (tanker, ISO, Cylinders) is intact and not damaged, with no visible product leak, or potential for leak, the vehicle must be immediately moved as far away from the traffic flow as possible. Should the vehicle be immobilised or rolled over, and the product containment (tanker, ISO, Cylinders) disturbed or damaged, the driver must take the following steps, provided he is unhurt and capable of doing so: 1. Stop the engine and switch off ignition 2. Switch off all lights (including flashers) 3. Disconnect batteries (isolator switch if fitted) 4. Do everything possible to secure the accident scene and prevent other traffic from becoming involved 5. Notify Police and Emergency Services, quoting the emergency code and the class relative to the DG product being carried, and the condition of the road 6. Move any injured persons to the upwind side of the accident scene 7. Clear the road as much as possible but avoid contact with Dangerous Substances, or the effect of toxic fumes which maybe produced by the DG 8. If the DG being carried are known to produce toxic fumes, use all available means to warn traffic and people away from the downwind side of the accident scene 9. Obey all directions from the Police and Emergency Services 10. Notify Management and carry out accident procedures according to Camlin Policy (i.e.: Information collection from other parties) Breakdown Procedures when carrying Dangerous Goods Get the Vehicle off the road if possible. If impossible, then: 1. Switch on four-way flashers. If not fitted, switch on park and clearance lights 2. Place warning triangles (double sided) as required by law (50-150m to the front and rear and one alongside vehicle) 3. Investigate breakdown and notify Management 4. Notify Police and Emergency Services of class of DG carried 5. If instructed to do so, arrange for vehicle to be towed to safe areas for repair 6. If the vehicle can t be moved into a parking area, then driver must take all precautions to reduce risk of accident and arrange for the vehicle to be towed to a safe area for repair. THE ABOVE RULES APPLY WHILE EVER THE VEHICLE REMAINS ON ANY PART OF THE HIGHWAY OR EDGES OF SAME. THE PROCEDURES ARE IN ADDITION TO CAMLIN POLICY AND PROCEDURE.
